背景技术

发明内容
通过设置套管和闭塞器的组合,本发明克服了现有技术的上述和其它缺陷,该套管和闭塞器的组合通过为许多诊断和治疗过程提供在套管位置确定和免持地通过该套管的连续接近方面的改进,便于进行由诊断成像导向的微创手术过程。

具体实施方式
现在参考附图,其中相同的附图标记表示在几个视图中相同的构件,在图1中,磁共振成像(MRI)相容活组织检查系统10包括导向件、MRI成像标记和流体处理能力,该导向件引导与活组织检查装置本身分开并且有利地包括改进的穿刺部分的套管和导入闭塞器的导向件。安装装置允许沿所需的轨道精确穿刺而不会超射。
MRI相容活组织检查系统10包括控制模块12,该控制模块典型地位于容纳MRI机(未示出)的防护室的外部或至少被隔开,以减轻与它的强磁场和/或灵敏的射频(RF)信号检测天线的有害的相互作用。控制模块12控制并给MRI活组织检查装置14提供动力,该装置14可以紧靠MRI机的方式配合使用。MRI活组织检查装置14的一个例子是前述的MAMMOTOMETM仪器。MRI活组织检查装置14由被连接到乳房线圈18上的定位夹具16精确定位,而该线圈又支撑着病人(未示出)。可在市场上购买到的乳房线圈18的例子包括由Waukesha WI的MRI DEVICE CORPORATION制造的BIOPSYBREAST COIL MODEL BBC。导向组件20、特别是套管22被有利地连接到定位夹具16上以在处置的具体部分中与MRI活组织检查装置14的选择性使用相结合地增加成像和治疗的灵活性和准确性。导向组件20可包括一个或多个闭塞器24,其中的一个是这样的,即它在插入时并且在MRI活组织检查装置14未被插入其中的程序的后续部分中密封套管22。设置与定位夹具16一起使用的限深器26,以有利地防止套管22的过度插入、套管22的无意收回和/或提高将套管22沿Z轴安置到所需位置的准确性。
为了方便起见,在这里使用一个惯常作法来通过笛卡儿坐标在参照定位夹具16的乳腺组织内定位可疑的病变,并且其后将仪器(例如套管22)定位到这个位置,而不需要连续地成像该区域。如将在下面更详细描述的,沿乳房的外侧受压缩的多孔屏障相对于病人胸部的中间平面定义了X-Y平面,而且X轴相对于站立的病人是垂直的(径向的),并且从面向定位夹具16的外露部分的医生的角度看相当于从左到右的轴线。相对于贴近病人皮肤的定位夹具16连接或定位的基准标记(未示出)定义了该平面的原点。垂直于该X-Y平面并向乳房的中间侧延伸的是Z轴,虽然应当理解可以允许相对该Z轴的插入角度有一定的变化,该轴典型地对应于MRI活组织检查装置14插入的取向和深度。这样,为了清楚起见,术语“Z轴”可以与“穿刺轴线”互相替换地使用,尽管后者可以或不可以垂直于用于在病人身上定位插入点的空间坐标。
将支撑安装限制器/限深器的追踪轨道与支撑活组织检查装置的重量的活组织检查轨道分离可有利地减少各组件之间的相互干扰,允许实现其中可以有选择地安装和除去某些构件且不会干扰其它构件的操作步骤。
在使用中,通过将电缆管理卷筒30放置在从控制模块12的一侧突出的电缆管理连接鞍座32上而制备MRI相容活组织检查系统10以便使用。缠在电缆管理卷筒30上的成对的电气电缆34和机械电缆36用于分别传递控制信号和刀具旋转/前进动作。具体地,电气和机械电缆34,36各具有连接在位于控制模块12中的相应电气和机械端口40,42上的一端和连接到安放MRI活组织检查装置14的机架44上的另一端。可以在不使用时保持机架44的MRI对接杯座46通过对接工位安装托架48被钩到控制模块12上。
安装到墙上的界面锁定盒50将系绳52设在控制模块12上的锁定端口54上。使系绳52有利地、独特地终止,并不够长以避免疏忽地将控制模块12过份靠近MRI机地设置。直插式外罩56可以有利地将系绳52,电气电缆34和机械电缆36对准在控制模块12上的它们各自的端口52,42,44。遥控键盘58可以在远侧被连接到电气电缆34上,以提高医生对MRI活组织检查装置14的控制,特别是当在将HRI活组织检查装置插入到定位夹具16中后不容易进行对MRI活组织检查装置14自身的控制时。
通过第一真空管线60提供真空辅助,该第一真空管线连接在控制模块12和捕捉液体及固体碎片的真空罐64的出口62之间。管道组件66完成控制模块12和MRI活组织检查装置14之间的气体连通。特别是,将第二真空管线68连接到真空罐64的入口70上。第二真空管线68分成被连接到MRI活组织检查装置14上的两个真空管线72,74。利用安装在机架44中的MRI活组织检查装置14,控制模块12进行功能检查。将盐水手工注入活组织检查装置14中以用作润滑剂并有助于实现真空密封。控制模块12致动在MRI活组织检查装置14中的刀具机构(未示出),监控全部行程。
也将MRI相容活组织检查系统10的靠近MRI机使用的部分组装起来。将通常已知的乳房线圈18连同其它的身体支撑垫(未示出)一起放置在MRI机的台架上。被连接在位于乳房线圈18的每一横向侧上的凹陷部内以接近在其中悬垂地暴露的病人乳房的定位夹具16包括水平的中间板80,可重复使用的基底组件82,侧面组件84和定位基座86。定位夹具16还装配有一次性的中间栅90和侧窗(或多孔板)92。
基底组件82放置在乳房线圈18的选择的侧面凹陷部内。中间栅90被连接到中间板80的中间边缘上,在接收病人乳房的选择的乳房孔94的内边缘下大致沿乳房线圈18的纵向轴线垂直地对准。在病人如此定位并且将乳房的外部区域消毒的情况下,使侧窗92向下滑入侧面组件84的三边框架式导向器96中,该导向器96又放置在中间板80上。基底组件82和侧面组件84沿Z轴彼此相对地移动,以将病人的乳房压在中间栅90和侧窗92之间。在侧面组件84,基底组件82和中间板80之间形成的机构保持了这种压缩。
可以将造影剂注射到病人体内以增强成像。使台架前进到MRI机孔中以成像定位夹具16和乳腺组织。将侧窗92上的基准标记定位并指定为X-Y-Z坐标系的原点。然后,将可疑病变定位在图像内,并且选择其上的一个点以确定其相对于原点的位置。应当理解,通过具有用可成像材料形成的侧窗92,可以便于定向初始扫描的X-Y-Z轴,由此提供除基准标记的原点之外的X-Y平面。在确定了目标位置的情况下,从MRI机器孔收回台架。
定位基座86沿侧面组件84的X轴可滑动地被接合,并限定了用于在选择的Y轴坐标上定位单个靶标轨(“轨道”)98的垂直导向器。轨道98又沿着用于在所需的Z轴坐标上定位限深器26和机架44的Z轴设置深度导向器。限深器被锁紧在轨道98上。其后,可以使标记仪(未示出)穿过限深器26,以在乳房上标记插入点。此后,将限深器26移开。在浅注射麻醉剂之后,在标记的位置处划开一切口,并随后将麻醉剂更深地注入划开的切口中。然后,将限深器26在轨道98上重新定位至所需的Z轴坐标参考。
将闭塞器24插入套管22并可以定位成封闭套管22的任何孔(侧面和/或末端),从而给乳腺组织提供封闭的表面。也可以如此地成形或构成闭塞器以提高孔位置的可见性。闭塞器24和套管22之一会提供一锋利的尖端(未示出)以穿刺乳腺组织。例如,如果使用具有开口端的套管22,则闭塞器可以设置锋利的尖端。
将闭塞器24插入到套管22中,并且由轨道98将组合体引导到适当的取向,直到到达由限深器26设定的准确的深度。一旦完全插入,限深器26就防止过度插入。套管22有利地锁住轨道98和/或限深器26以防止无意中造成的收回,例如当收回闭塞器24并且从乳腺组织接收压力时,或更后面的当将MRI活组织检查装置14的探头100从套管22收回时。
将台架移入MRI机孔中并且对病人重新成像以确定套管22相对于可疑病变的放置。有利地是,套管22和/或闭塞器24的可成像材料,或许包含或包括标记材料,提高了在为随后的活组织检查样本定位时确定套管22和其套管侧孔102的位置的能力。
通过缩回台架而将病人从MRI机中移出,并且将有机架的MRI活组织检查装置14带到定位夹具16。从MRI活组织检查装置14的探头100上去除保护帽(未示出),并且从套管22取出闭塞器24。图2和3中示出了将机架44安装到轨道98上,其中机架44和MRI活组织检查装置14结合在一起地在轨道98上滑动,而该轨道98已经相对于基座86和侧面组件84被定位在一确定位置。套管22和探头100的构件可以有利于从视觉上且以机械方式相对套管侧孔102定向探头100的探头侧孔104,以及形成气体密封。有利地是,机架44和/或探头100可以锁定在轨道98和套管22上,以确定完全插入并避免过度插入及无意中发生缩回。机架44允许希望手握使用的MRI活组织检查装置14在它与定位夹具16的连接中有足够的支撑,以准确地保持它的位置并且避免或最小化由探头100带来的负载。
其后,MRI相容活组织检查系统10可以通过在真空辅助下致动刀具机构、收回该刀具并取出组织样本来采集组织样本,取出组织样本的步骤或许也可以在真空辅助下进行。探头100/套管22的组合体能够为了另外的样本手动、或许自动地转动相对于它们的纵向轴线的所需角度,或者可以在目前的取向上通过进一步采取真空辅助来采集另外的样本。然后,使刀具前进以关闭探头侧孔104,并且将机架44从定位夹具16退回,从而将探头100从套管22取出。
在这点上可以进行额外的步骤或步骤的组合,例如使用探头100、专门的闭塞器24(例如,细探针)或仅仅是套管22来将各种药剂引导到活组织检查的手术部位。这些例子包括排液,引入麻醉剂,引入止血剂,随气压吹入和引入用于随后定位活组织检查部位的标记,或其它诊断或治疗过程。
然后,为了重新成像而通常将病人拉回到MRI机孔中,以确定至少一部分可疑病变的去除和标记的可能位置。在这个重新成像期间,用闭塞器或细探针24密封套管22。其后,移开定位夹具16,将病人用绷带包扎且从台架上移开,并且将MRI相容活组织检查系统10的一次性使用部分作为医疗废物进行处理。
特别参考附图2-3,单个靶标轨道98便于分离组件的顺序安装。首先,安装限深器26,然后是套管22(如图1所示),接着使活组织检查工具14滑上单个靶标导轨98。作为替换方式,如图2-3所示,单个靶标导轨98可以接收限深器26,然后使用MRI活组织检查装置14而不用分离的套管22。穿刺到病人乳房中的最大深度通过限深器26在单个靶标导轨98上的位置而预先设定。在机架44和单个靶标导轨98之间的接合机构(未示出)和/或由被表现为向上伸出的销110且在限深器26的上部轨道夹紧臂112上的掣子和卡扣在向上伸出的销110上的由弹簧向下偏压的摇杆插口(rocker latch)114形成的接合机构防止MRI活组织检查装置14的无意缩回。机架44可以通过在摇杆插口114的近侧致动臂116上的向下压力而脱离。
单个靶标导轨98可以将纵向尺寸设定成在近侧有效地延伸,从而在探头100接触病人的皮肤之前,MRI活组织检查装置14接合单个靶标导轨98。还将单个靶标导轨98的尺寸设定成不会在近侧延伸到其不能用于闭孔MRI机(未示出)的程度。相信该MRI相容活组织检查系统10将程序运转时间最小化到小于上述的45分钟。然而,尽管迅速完成运转,放射科医生可以将探头100准确定位到病变中心的2mm(最大5mm)内。另外,在对病人最小限度地重新定位的过程(桌子的两侧)中,放射科医生可以最大限度地接近两个乳房(左或右)。此外,穿刺组织需要最小量的力,例如小于4lbs。虽然限深器26用于防止超射,但在进一步插入探头100之前用于将限深器26重新定位的构件允许医生在瞄准另一位置方面具有灵活性。
在图4中,用于MRI相容活组织检查系统10的一种可替换的导向组件200包括有连接到靶标导轨204上的支架202,并且设有用于支撑MRI活组织检查装置的活组织检查导轨206,两个导轨204,206对准Z轴。将靶标导轨204连接到定位支柱86(图4中未示出)上并垂直地调整到所需的Y位置。圆形安装点208可以形成与定位基座86的旋转接合以允许倾斜的瞄准导向。
靶标导轨204的侧面210包括上凸缘212和下凸缘214,它们各具有用于滑动地接收套管固定件216的L形横截面。在每个凸缘212,214上的多个竖排的侧向突出的凸脊218用作用于套管固定件216的锁定表面。在凸缘212,214之间,使侧通道220在其中凹陷。套管固定件216通过使它的套管毂224在近侧被接收在套管固定件216的毂接收器225中来引导套管222并在远侧由限深器226定位和限制。
限深器226包括接合侧通道220的滑动件228。限深器壳体230与该构件连接,终止于标线片232。锁定杆234被垂直地钉在远侧开口的凹陷部(未示出)内,在限深器226中形成由弹簧从那里偏压离开的侧面部分236,从而在远侧突出的支脚238相对凸脊218枢转并接合该凸脊218,特别是相对近侧移动地枢转。将侧面部分236靠着限深器壳体230的远侧开口的凹陷部向近侧压下的操作释放了在远侧突出的支脚238,以允许在远侧重新定位限深器226。
使活组织检查装置10的穿刺轴线与通过靶标导轨204和活组织检查导轨206限定的轴线对准,上述两个导轨分别从那里沿横向和纵向垂直地偏移。从靶标导轨204延伸的水平平面和从活组织检查导轨206延伸的垂直平面在为穿刺轴线的共同中心线处相交。具有垂直地对准并平行于穿刺轴线的活组织检查导轨206有利地用否则会产生插入的远端的偏移的最小扭转负载为活组织检查装置14的重量提供支撑。从而,即使对于相对较重和细长的装置,也可以在所需插入点的5mm、甚至2mm内实现其远端的定位和保持。由此,可以进行“免手持”过程,可用在穿刺轴线上方垂直位移的方案替换在所显示的变型中避免穿刺的不方便或不实用性的方案。特别是,具有可以与靶标导轨204的任一侧接合的支架可以进一步提供垂直对称并允许操作者充分利用通过乳房线圈18提供的空间。
虽然“免手持”能力对于单独插入式/多样本式活组织检查装置是有利的,但应当理解,带有在此所描述的预设的限深器的这种穿刺导向已经应用于甚至轻型活组织检查装置,该装置在每单个样本单独插入时使用针芯活组织检查。特别是,不需要在连续成像时修正放置。当免手操控的时候,避免了插入时的过度穿刺和无意的移动。
在靶标导轨204中的底部楔形通道240接收在支架202上的顶部楔形延长部242,而且该延长部在其中滑动。应当理解,当从近侧看时在这里示出了关于定位基座86的左侧的安装,但是导向组件200有利地包括允许在定位基座86的两侧上的安装和使用的对称部分,以增加在定位探头100方面的灵活性。因而,支架202的水平基底244将活组织检查导轨206形成为由第一和第二对单片眼镜插孔248,250位于侧面的活组织检查导向通道246,从而可以将单片眼镜254上的一对锁定钩252插入每对单片眼镜插孔248,250中,这取决于哪一个更靠近病人。可以将支架202直接连接到定位基座86(未示出)上,而不如图所示地将支架202安装到靶标导轨204上。支架202是机械上坚固的并能够支撑MRI活组织检查装置14的总重量。由于MRI活组织检查装置14不共用支架202,当需要接近浅的或深的病变时,可以最优化支架202以支撑MRI活组织检查装置14。
作为在定位和准备插入点时的初始步骤,插入单片眼镜标线片258中的导向衬圈256引导标记仪和/或画线手术刀(未示出)。其后可以将单片眼镜254去除或留在适当位置上,以便在除去限深器226的标线片232之外还引导套管222,限深器也可以保持用于引导套管222的导向衬圈260。将导向衬圈256,260去除允许单片眼镜254和限深器226的标线片258,232引导更大的组件,例如用于将可疑病变相对导向组件200定位的基准262。
以首先通过套管固定件216的接收套管毂224的毂接收器225的方式保持套管222的对准。在图中显示的变型中,套管222具有用于接收导入闭塞器268的端部开口的轴266,该导入闭塞器包括在固体闭塞器轴272的远端的穿刺尖端(例如扁平刀片)270。进入固体闭塞器轴272的斜切的凹陷部276对准套管222的套管侧孔278,并且由此终止于探头100(图1-3)。可以选择闭塞器268的材料以帮助定位套管222的套管侧孔276,否则该侧孔可能更加难以在MRI扫描部分中看到和定位。
套管毂224具有被连接到导向指旋轮282上的近侧圆柱形边缘280,该导向指旋轮282在近侧从套管固定件216的毂接收器225延伸出,用于旋转套管222,以将它的套管侧孔278相对于指旋轮282上与之相对应的且作为锁定槽284示出的可见标记定位。指旋轮282包括由夹在指旋轮282和套管毂224的近侧圆柱形边缘280之间的弹性密封环288和鸭嘴密封件290密封的中心通孔286。因而,包括进入锁定槽284的锁定条292的闭塞器268的插入封闭了中心通孔286并相对弹性密封环288形成动态密封。
在去除了闭塞器268后,可以将细探针298插入套管222,从而可以将在细探针298的近侧呈现的软管尖端300用于对手术位置吹气或用作其它目的,例如排放体液或将治疗或诊断剂通过细探针298的探针杆302插入到对准套管222的侧孔278的探针侧孔304。细探针298还包括锁定小突起306。
套管固定件216包括由弹簧向下偏压的卡扣接合在限深器226的倾斜捕捉器310上的摇杆插口308,防止套管222在无意中被缩回。套管固定件216可以通过在摇杆插口308的近侧致动臂312上的向下压力被脱开。被连接到套管固定器216的底部上的由弹簧向上偏压的摇杆插口314同样地接合限深器226。因而,在将限深器226在靶标导轨204上设置到所需的插入深度之后,可以使套管固定器216在远侧前进,且不会超射,随后当从其移去器具,例如闭塞器268,细探针298和MRI活组织检查装置14时,可以将该套管固定器保持就位。
在图5中,用于MRI相容活组织检查系统10的另外一种可替换的导向组件400包括接合第一靶标导轨404的底部通道403的支架402。为了给图1-3所示的MRI活组织检查装置14提供额外的导向,第二靶标导轨406包括沿第一靶标导轨404的纵向导向翼片410受引导的侧面通道408。当完全接合在其上时,在棘爪弹簧414的推进下围绕侧窗418中的垂直棘爪销416枢转的棘爪412落入在近侧定位在第一靶标导轨404上的近侧制动器420中,所述侧窗418在近侧定位在第二靶标导轨406中。
套管422包括在近侧连接到圆柱形毂424上并具有接近开口的远端428的侧孔426的中空轴(或插管)423。圆柱形毂424具有用于转动侧孔426的在外部设置的指旋轮430。圆柱形毂424具有一内部凹陷部432,该凹陷部432包含鸭嘴密封件434,弹性密封环436和密封保持器438,以在轴423是空的时候提供液体密封,并密封到插入的导入闭塞器440上。

当在第一位置将第二靶标导轨406安装在第一靶标导轨404上之前,最好将套管固定件460可调整地定位在第二靶标导轨406上,以设定所需的穿刺深度。特别是,深度导向器490由具有侧面通道496的新月形深度指示器492形成,该侧面通道496被做成接合顶部和底部导向凸缘466,468的形状。定位在侧面通道496的顶部和底部上的前倾表面498,以接合在第二靶标导轨406上的脊状棘齿表面470,允许通过从第二靶标导轨406的远端插入深度指示器492来进行组装。其后,摩擦接合阻碍进一步的近侧运动并强烈地抵抗特别是来自深度导向器490的深度导杆500的任何远侧运动,当将深度致动器杆505用于在其中转动和纵向定位深度导杆500时,该导杆的远端502在深度指示器492中的外侧孔504内旋转并且其近侧端向侧面偏转。将深度导杆500的中部容放在纵向通孔506中,该纵向通孔在套管固定件460中形成,外侧为侧面通道408。为了粗调深度,使深度导杆500上的外部导向螺纹507有选择地接合套管固定件460,直到顶部和底部粗调按钮508,510被向内压入套管固定件460中,压缩相应的顶部和底部粗调压缩弹簧512,514。各粗调按钮508,510包括一相应的垂直长孔516,518,它们的内表面具有涡轮部分520,522,以在通过释放的粗调压缩弹簧512,514而被促进接合时接合在深度导杆500上的外部导向螺杆507。
Hibner等人申请了两个美国专利申请,其中一个申请的名称为“一种带有可拆卸探头的MRI相容活组织检查装置”,该申请于2002年4月23日申请,系列号为10/170,535,并于2003年10月23日公开,其公开号为US2003/0199753;另一申请的名称为“MRI活组织检查装置”,于2005年3月10日申请,系列号为11/076,612,上述两篇专利的公开内容在此作为参考全部引用。在这两个美国专利申请中描述了一种可拆卸的探头(或套管),其具有许多优点,例如在重新成像的过程中允许MRI过程在保持探头插入的情况下进行。在图1-5中,分开的套管和闭塞器的性能还提供了附加的临床灵活性。应当理解,可以根据特殊的应用或喜好来选择构件的各种组合。与活组织检查装置中的样本采集侧孔相对应地在套管中具有侧孔通常是理想的。例如,根据需要插入可疑病变中的端部开口的探头或活组织检查针可以产生充有剩余造影剂的血肿,使得难以在那个部位进行进一步的成像研究。另一例子是,穿刺可疑病变可造成轨迹转移灶的危险。另外,首要的一点是,这种针或探头的尖端可能难以相对于可疑病变成像,以准确地定位后者。
相比较而言,在图6中,套管602的侧孔600可以被定位在可疑病变的旁边,从而穿刺尖端604不需要通过可疑病变。在MRI扫描部分中定位该侧孔600将似乎更容易,这是由于侧孔602限定了一条线,这条线更容易允许将成像部分沿它的长度相对几何基准定向,该几何基准可以容易显示为了进行活组织检查而从哪个方向将组织拉入侧孔600中。然而,那些不理想地定向或穿过可以形成套管602的MRI相容材料的部分仍然可以使侧孔600的准确而便利的识别复杂化。为了有助于这种识别,当导入套管602时,闭塞器606通过基本上或完全地阻塞侧孔600来提供帮助,从而组织不会下垂到侧孔600中,并不会阻碍插入和/或引起组织病变。
在一些应用中,进一步希望在套管602中具有远侧开口608。因而,闭塞器606有利地包括在套管602中在远侧延伸出远侧开口608的穿刺尖端604。闭塞器606进一步具有当将闭塞器606完全插入其中时对准套管602中的侧孔600的侧面凹陷部(例如,凹口,斜削面,独木舟形凹槽)610。由于径向不对称,这种侧面凹陷部610提供了用于定位侧孔600的快速获得和表示的基准。
在图7中,侧孔620在套管622中形成,该套管具有形成或支撑穿刺尖端624的闭合远端628。闭塞器626在导入时用于使组织脱垂部成为进入侧孔600的形状,并包括当将闭塞器626完全插入其中时对准套管622中的侧孔620的侧面凹陷部(例如,凹口,斜削面,独木舟形凹槽)630。
在图8中,闭塞器646包括延伸出图6所示套管602中的远侧开口608的穿刺尖端644。通过具有上部纵向部分649和下部纵向部分651,填充器646形成一个特殊的横截面。使上部纵向部分649的形成成为可控制组织在侧孔600中的脱垂并提供用于MRI扫描部分的容易定位的基准。
在图9中,带有被做成或支撑穿刺尖端624的闭合远端628的图7所示套管622被表示出在由闭塞器656导入的过程中使它的侧孔620关闭,所述闭塞器656具有表示出上部纵向部分649和下部纵向部分651的特殊的横截面。上部纵向部分649具有这样的横截面形状,即,其被设计成使组织下垂进侧孔600中并提供用于MRI扫描部分的容易定位的基准的形状。
在图10中,套管702具有侧孔700和被做成包围闭塞器706的不对称的穿刺尖端704的闭合远端708。闭塞器706具有由形成特殊的横截面(例如如图11-12中示出的X形)的上部纵向部分709和下部纵向部分711形成的连续的轮廓形状。可替换的是,闭塞器706可以具有特别的横截面,例如被连接到下部纵向半圆柱体717上的向上的纵向脊部715。应当理解的是,组织在侧孔处的脱垂提供了MRI图像回扫(image return),而在闭塞器的其它部分中,位于套管702和闭塞器706之间的空气空间同样显得较暗。
在图15中,闭塞器806包括最接近穿刺尖端804的侧面凹口810。MRI可视嵌入物807(例如水凝胶)可以有利地具有足够的硬度以保持在位并防止组织下垂进入套管的侧孔(图15中未示出)中,而非依靠在重力、触摸和真空辅助下的组织脱垂。在图16中,代替从侧面插入的作法,闭塞器826可以包括在近侧接近的标记腔827,标记嵌入物829可以通过该标记腔以最接近远侧的穿刺尖端824的方式被插入。
作为对加入的MRI可视材料的替换,在图17中,闭塞器846包括真空腔848,该真空腔与侧面凹口850连通,以抽入足够的体液851,从而提供套管(未示出)的侧孔的MRI可视图像。在图18中,闭塞器846利用通过真空腔848提供的真空辅助,以使组织853下垂到侧面凹口850中,从而提供MRI可视图像。在图19中,闭塞器846进一步包括以高过侧面凹口850的方式滑动、从而捕捉MRI可视材料(例如含水流体,钆溶液等)857的薄护套855。
在图20中,闭塞器876包括基本上由圆柱形护套877包围的固体的细探针嵌入物879,除了在固体的细探针嵌入物879中形成的侧面凹口881。圆柱形护套877在远侧被连接到陶瓷的不对称穿刺尖端874上。
在图21中,闭塞器899具有一个开口的远端894和一个侧孔890,后者可在近侧从在其中形成的标记腔893进行真空辅助下的空气排空,从而允许由体液填充标记腔893以提供MRI可视材料

在图26-37中,示出了套管和闭塞器形状的其他变型,其有利地提高了在穿刺过程中提供闭合形状来采集活组织检查样本之前定位可疑病变和确定其侧孔的正确位置的能力,该闭合形状可以改变为对应于在该处采集样本的分离区域的形状,该形状表现为可视的固体,以便即使当从成像部分的各个角度看时都可以容易地辨认。
这个特征解决了依靠用于成像的探头的缺点。在成像区域中具有金属物质会导致会使所感兴趣的组织模糊的假像(局部图像浮散),例如试图使用活组织检查探头自身封闭套管。当通过中空套管22的成像部分会引起难以识别侧孔的困难时,成像时移去探头并仅依靠套管造成了另一成像问题。通常,所选择的MRI相容材料不给出MRI图像回扫,正如跨过由此不存在回扫的侧孔提供填充空气的空隙。
在图26-27中,MRI相容活组织检查系统1210包括具有凹口1202的套管1222,该凹口与MRI活组织检查装置的探头的探头侧孔(图26中未示出)的位置和尺寸相对应。另外,在一些情况中,凹口1202的深度可以比探头侧孔深,以突出在用于成像目的的套管1222上的该位置。
在图26中用虚线示出了在基本阻塞套管1222的凹口1202的其“闭合位置”的闭塞器1224,该闭塞器可以如上所述地有利地由热塑性塑料制成,并且具有延伸通过套管1222的开口远端1221的在远侧的陶瓷刀片部分1220。陶瓷材料在MRI环境中操作很好并保持锋利边缘。在凹口1202由同轴插入的闭塞器1224关闭的条件下,可将套管1222插入乳腺组织中。
在图28-29中,示出的闭塞器1224有利地包括如图28-29所示的纵向分开的设计,其中下部1223包括沿其长度向下的楔形通道1225,该通道可滑动地接合从闭塞器1224的上部1229向下延伸的楔形条1227。陶瓷刀片部分1220仅连接到下部1223上。如图28所示,上部1229可以在近侧随在远侧完全插入到套管1222中的下部1223运动,由此打开套管1222的凹口1202。由于闭塞器1224是固体,当由MRI机采集3-4mm的图像片断时,闭塞器1224的下部1223填充在凹口1202内,从而可以容易地确定它的位置。这种两件式闭塞器1224有利地容纳了具有复杂横截面形状的套管腔,例如所示的椭圆形套管1222(图27)。
在图30-31中,套管1322包括在远侧连接到它的轴1319上的整体的尖端1320,该轴1319限定了圆形切割腔1321和下面的真空腔1323。在图32-33中,圆形闭塞器1324通常是杆形,以插入切割腔1321,但是填充器1324带有对应于套管1322的凹口1302的凹形缺口1325。只要圆形闭塞器1324在切割腔1323内可旋转,就可有选择地提供凹形缺口1325以打开套管1322中的凹口1302,或旋转大约半圈以关闭凹口1302。
图34-35中示出了在MRI图像扫描中产生的结果,其中用闭塞器1324选择性地关闭套管1322中的凹口1302提供了固态像,但没有对凹口1302的定向的指示。在图36-37中,利用可被旋转以打开凹口1302的闭塞器1324,容易显现凹口1302的定向。
如以上针对图4-5所描述的,作为在闭塞器1424和套管1422之间的锁闩锁定特征的替换,在图38中,闭塞器1424可以有选择地在近侧被连接到套管1422上,以防止无意的缩回并利用卡口型连接辅助保持气/液密封,该卡口型连接由从闭塞器1424的圆柱形基底1434沿径向突出的楔子1432形成,而所述闭塞器1424被容放在在套管1422的套管腔1440的近侧孔1438内对准的L形凹陷部1436内。在图39中,通过围绕闭塞器1424a的圆柱形基底1454的外螺纹1452形成螺纹连接1450,该闭塞器由围绕套管1422a的套管腔1460的近侧孔1458的内螺纹以螺纹接合方式接收。在图40中,通过锁定插座1472形成按钮释放连接1470,该插座由通过套管1422b的套管腔1480的近侧孔1478压下的按钮1476在纵向位置围绕闭塞器1424a的圆柱形基底1474径向露出。
通过采用一个单独分开但是与MRI活组织检查装置一起使用的套管提供附加的治疗和诊断能力。这种应用通过在套管内包含流体和气体密封而得以加强,从而可选择地排出体液,可将麻醉剂或其它药物成分引入活组织检查的手术位置,可进行手术部位的吹气和排气,和/或可以保持MRI相容活组织检查系统的真空辅助。
在图41-42中,套管1522越过对套管腔1526的近侧孔开口1524配置大直径密封件或隔板1523。因而,在没有闭塞器、MRI活组织检查装置的细探针或探头同轴插入套管1522中的条件下,可以在乳腺组织中保持一定程度的气压(例如,吹气法)和/或防止流体漏出。套管1522在其基底432中有利地配置有口1530,该基底为了例如排出流体或加入麻醉剂而与套管腔1526连通。
在图43-44中,类似的套管1522a装有代替大直径密封件的O型动态密封环1550,它与闭塞器1524一起形成密封,该闭塞器可以沿其长度有利地包括凹槽或通道1552,以允许口1530和侧孔1502之间的流体连通。
在图45-46中,类似的套管1522b结合使用一个更远侧的O型动态密封环1560和口1562及近侧的O型动态密封环1550和近侧口1530。闭塞器1524b可以有利地具有两个凹槽或通道1570、1572,它们在各自的口1530、1562和各自的侧孔1502和开放的远端1574之间连通。
参考图47,闭塞器1600包括锋利的穿刺尖端1602,该尖端在没有用于穿刺组织时通过圆柱形护套1604有利地得到保护。由此,减少了因疏忽受伤和损坏设备的危险。在图48中,当闭塞器通过套管1606插入时,圆柱形护套1604穿过密封件座圈1608并由保持在该密封件座圈1608和套管毂1612之间的鸭嘴形密封件1610捕获。穿刺尖端1602穿过套管毂1612,并继续穿过套管1606的插管1614。应当理解,当闭塞器1600缩回时,穿刺尖端1606穿入护套1604并且拉动护套1604脱离套管毂1612。

套管可以有利地由或是同质或是合成的聚合材料制造,其尽管壁薄但仍然坚固,由此整个外径不需要明显比已知的活组织检查探头大,从而实现最小限度地侵入。强度和小横截面区域使通过皮肤的开口尺寸最小化,从而一般避免缝合闭合伤口的需要,减少了插入探头需要的力,并使穿刺可疑病变过程对乳腺组织的伤害最小。强度和硬度有利地维持用于通过那里的后续活组织检查和其它过程的开口腔。另外,套管有利地由可对病人和MRI相容的生物相容材料制造。通常,这样的材料不会产生明显的使接近套管22的组织图像模糊的假象。
尽管没有列表全部包括,但可以使用的聚合材料的例子包括:聚酰亚胺,聚醚酰亚胺(例如由GE PLASTICS生产的ULTEM_树脂),热塑性塑料液晶聚合物(LCP)(例如CELANESE AG生产的VECTRA_),聚乙基醚酮(例如,VITREX生产的PEEKTM),聚酰胺,聚碳酸酯(例如,BAYER POLYMERS生产的MAKROLON),聚砜,聚醚砜,聚苯砜(例如,ROWLAND TECHNOLOGIES生产的RADEL_),以及尼龙和尼龙共聚物。

在图71中,可以采用聚酰亚胺处理方法形成这种材料,其中由溶液形成薄膜。标准的做法是通过使来自线轴的缆线经过熔炉而通过聚酰亚胺涂敷溶液、到达卷取线轴来涂敷连续的缆线。缆线通常通过多次处理以形成涂层厚度。串联的炉子或加热元件馏出溶剂并导致聚酰亚胺部分交联。当已经获得了所需的涂敷厚度时,通常发生全交联。全交联发生在比涂敷过程中使用的温度高得多的温度下。
为了形成自站立的聚酰亚胺管,随后去除缆线。例如,在从两端拖拽或牵引缆线以将其拉直的情况下可切割多段涂敷的缆线,由此减小它的外直径,直到与涂层分离,然后抽回缆线。可替换地,可以将容易化学侵蚀的材料用于该缆线。例如,可以通过过二硫酸钠盐配位溶液溶解铜线,留下自站立的聚酰亚胺管。
另外,为了形成更复杂的横截面形状(图72A-72D)(例如,椭圆形等),在最终的交联加热步骤之前,可以按一个形状放置聚酰亚胺管。另外,可以将芯轴穿过聚酰亚胺管插入以进一步限定形状,特别是与压缩的外部形状共同作用,例如在用于椭圆形的如图73A-73C的过程中所示出的,和用于腰部变细的椭圆形的如图74A-74E的过程中所示出的。
在图75、76A-76B中,随后可进行后续的处理过程以在其远端形成侧孔和/或在其近侧端形成安装孔。例如,通过eximer或TAG激光器的激光切割可以形成所需的穿孔。另外,eximer激光器不仅可用于形成通孔,也可以形成可以充分产生与重叠注塑件的足够的机械干涉(mechanical interference)的放气缝,以在正常的加载和拉伸的过程中确保装配完整性。可以采用完全穿孔以允许重叠注塑件,例如近端的固定机构,在变硬前从孔中流出。
为了得到在图72所示管中的孔,可以使用几种方法。例如,eximer或YAG激光器可以形成穿孔(图75A)。也可以对eximer激光器编程以不仅形成通孔,而且形成放气缝。放气缝可以作为替代物,以与重叠注塑件产生充分的机械干涉,从而在正常的加载和拉伸过程中确保装配完整性(图75B)。作为另一例子,可以将冲孔或模切处理与成型模具相结合。在以相同形状热处理后,元件可以被切割,修整和冲孔。
作为有利方式,成型模具应当坚硬并且具有较高的导热率。钢虽然很硬,但是具有低的导热率。相反地,铜或黄铜具有高的导热率,但是比较软。硬质铝合金可以是用于带有冲孔用钢嵌件的成型模具的适当材料。
也可以由编织的合成物形成护套。可以将编织物的各纤维缠绕在聚酰亚胺的初始层上,然后密封在一层聚酰亚胺层中。编织物可以是MRI相容陶瓷纤维,例如由3M生产的NEXTEL。
套管和/或闭塞器的部分可以由为它们的可成像能力(暗或亮)而选择的材料形成。在为了可能的癌症而用于成像乳腺组织的多数标准的MRI扫描程序中,已知的工程塑料显得暗或具有低对比度,这可能在为了定位和诊断目的识别该构件时产生问题。因此,除了考虑当形成具有足够强度和MRI相容性的套管时的上述考虑之外,可有利地替换或增加对MRI机是明亮的但不会产生明显的假像的材料。另外或作为一种替换的方式,可在结构较“暗”的材料上形成对MRI机显得明亮的重叠注塑或涂敷或嵌入材料。另外或作为一种替换的方式,为了强度或其它原因而选择的“暗”材料可以与吸收增强对比度或亮的流体的材料一起进行重叠注塑、涂覆或镶嵌模塑。另外或作为另外一种替换的方式,可使复合材料或多层材料具有一些为诸如强度的特性而选择的层和另一些为了其对于MRI机是可视的的特性而选择的的层。
例如,可以将标记条带的特殊图案放置成低于套管的侧孔,或放置在邻近或远离套管周围的侧孔的隔离环中。例如,可以将Dy2O3或Fe2O3与墨水混和,然后印制到套管22的部分上,或用于填充闭塞器24或细探针上的凹陷的凹槽。这种图案也可以通过将Dy2O3或Fe2O3作为填充物分散到热塑性塑料中产生,从而它可以通过回流(reflow)或热粘接被施加到套管24和/或闭塞器上。另外一种方案是例如通过装填模制的/挤出的合成塑料(例如,PEEK,ULTEM)而在装置中重叠注塑Dy2O3或Fe2O3并固定(例如,重叠注塑PEEK中30%为Dy2O3的环)。
作为另外的替换方式,材料的区域能够由水基溶液和/或造影剂(例如,钆螯合物)浸渍或进行水合作用,从而当成像时显得明亮。作为另外的替换方式,材料的区域,例如浸泡或水合,可以在即将使用前发生,或该材料可以被预先水合并无菌地包装。
特别是,对MRI机显得明亮的某些聚合物可以选择包括具有高的醇或羧酸官能度的水溶性的合成聚合物。例如,纤维素衍生物包括:羧甲基纤维素,乙基纤维素,羟乙基纤维素,羧甲基羟乙基纤维素,乙基羟乙基纤维素和甲基纤维素。另外的例子如,丙烯酸类,包括:聚丙烯酸盐和聚丙烯酰胺。再例如,其它人造材料,包括聚乙烯醇(PVA),聚乙烯基甲基醚,聚乙烯吡咯烷酮(PVP)和聚环氧乙烷(PEO)。另外例如,天然物和衍生物包括:玉米淀粉,凝胶,糊精,海藻酸盐,casien,胶原质(例如,牛的等)和天然胶树胶(例如,呫吨胶,黄芪胶,刺梧桐树胶)。另外例如,生物高聚物包括聚乳酸,2-丙交酯-共聚-乙交酯(PLG)(即,例如,丙交酯异构体(D,L,DL)(MP=225-300℃)),聚己酸内酯(MP=60℃),乳酸盐和葡糖酸盐,聚二噁烷酮,聚乳糖(即,缝合材料)。
对MRI机显得明亮的其它聚合物包括基于硅树脂的材料,例如用羟基(-OH)和羧基官能化的硅氧烷,和例如硅树脂(即,流体和树胶橡胶)。
在解释性描述中,当使用聚合材料成像而不会在MRI中出现过多的假像时,氧化镝(Dy2O3)或氧化铁(hermatite)(Fe2O3)作为填充物散布在能够加热成型(例如挤出,模制等)的热塑性塑料载体中。从而当结合到例如套管22或闭塞器24的装置中时,形成标记,改进MRI下的装置可视性(例如,梯度响应EPI,闪光,实时真FISP)。特别是,将Dy2O3(35%)散布在ATOFINA Chemicals,Inc.的Rilsan_聚酰胺(75%)中。将这种化合物挤入薄壁的(即0.002英寸)管中,该管使用闪光而完全可见。另外,闪光的出现产生了敏感装置(包括Dy2O3和Fe2O3)的最好的可视性,EPI较少可见,而且看不到实时真FISP。
对MRI机显得明亮的其它聚合物包括亲水性聚合物和聚合物泡沫,例如将亲水性嵌段共聚物链段接枝到聚氨酯主链上或采用如等离子体氧化或化学氧化的表面官能化方法使其能快速吸收湿气的聚氨酯泡沫。类似地,可以泡沫化其它聚合物,例如带有线型低密度聚乙烯的淀粉,发泡的聚四氟乙烯(PTFE),或其它材料(例如,聚酰亚胺,聚烯烃,聚苯乙烯,聚酯,尼龙,丙烯酸树脂,丙烯酸酯,聚碳酸酯,蜜胺树脂,聚氯乙烯,聚乙酸乙烯酯)。
作为将基于水的溶液浸入或与水结合进入这种材料的实施方式,该溶液为了在溶液中增加T1的钆基的化合物,包括二亚乙基三胺五乙酸(DTPA),gadoteridol(GD-HP-D03A)(非离子性的),二酰胺钆(gadodiamide)(GD-DTPA-BMA)(非离子性的),和GdDOTA(离子性的)。该溶液也可以包括用于T2提高的铁基溶液,例如Feridex(超顺磁剂)。
已经在前面讨论了在选择可选择地关闭侧孔102的闭塞器的材料或成型时套管22的侧孔102的突出。还提及了可以突出上述套管22的特殊区域。关于后者,使用在MRI成像下明亮的材料标记侧孔102可以利用可商业购买的造影剂来实现,以操纵现有的重要设备和供应通道。这种造影剂的例子如钆(Gd+3)(例如,BERLEX生产的MAGNEVIST_(gadopentetate dimeglumine));铁(Fe+3)(e.g.,FERIDEXIV_(ferumoxides可注射溶液);及锰(Mn+2)MnDPDP(e.g.,AMERSHAM HEALTH生产的TESLASCANTM Mangafodipir)。在存在水的条件下,聚合物的母体(matrix)可以膨胀以产生用于造影剂的限制性的水合的环境。这些聚合物可以渗透水,但是对造影剂分子具有有限的渗透性。造影剂可以用化学方法限制到基体中,以减少或限制它们移动脱离基体。适合这种母体的聚合物的例子包括水凝胶,带亲水嵌段的氨酯丙烯酸酯,乳胶涂料/乳液,填充亲水微粒、例如二氧化硅粒子和微粒聚集体的涂料。
闭塞器内的空间可以包括通风孔和隔膜。随着空气通过通风孔可以将造影剂注入通过隔膜。空间内的吸水性泡沫或纤维素材料(例如棉花)可以用于更好的保留造影剂。可替换地,对比螯合物可以预填充到吸水性泡沫或纤维素材料上,并在进程中结合水。
作为替换或另外的方式,在MRI下明亮的材料,可以结合激活标记条带技术,以照亮套管22中的侧边或远端孔。用于不同用途的激活照明的例子在美国专利Nos.5,211,165;5,307,808;5,318,025;5,437,277;5,443,066;5,445,150;5,715,822;5,882,305以及6,289,233中有说明。邻近采样孔(例如具有沿套管22的长度形成的电连接的侧孔102)地形成线圈。由于在聚酰亚胺上形成薄膜电路的技术和方法的重大发展,聚酰亚胺是用于形成这种线圈和电连接的基底的特别好的材料选择。电绝缘可以由聚酰亚胺的另一层涂层获得。围绕与这些标记条带电连接的套管22的基底的电感线圈将允许RF连接到这些线圈上,并对病人提供RF隔离。可替换地,可以将专用的集成电路和能源(例如电池,电容器)集成到套管22中,以消除外部激励的需要。可以并联或串联或单独地激励这些标记条带线圈。作为另一替换方式,可以串联设置两个线圈但带有一个中间抽头。
在一些应用中,需要结合热敏电阻或热电耦,该热敏电阻或热电耦可以监测不能接受的温度升高(例如,4℃)而自动关闭。作为另一替换方式,可以将光学变换器结合在套管中,从而明亮纤维可以传递信号进出。
类似的考虑适用于套管或闭塞器的穿刺部分;然而,穿刺组织的需要可以导致其它选择。作为引入器械,用于MRI安全医疗装置的金属组件必须是生物适合的,并不会与MRI成像过程中使用的强磁场相互作用。标准的300和400系列不锈钢广泛用于医疗装置的设计中。这些材料结合了耐蚀性、生物适应性、坚硬和抗张性的特性。这些材料主要含铁。300系列材料与磁场的相互作用比400系列材料小,但是硬度性能较低,这限制了将其用于切割和/或穿刺组织的锋利边缘。所有的300和400系列材料都具有相当含量的铁,这限制了将它们用于MRI成像用途。
铁合金:存在至少一种含铁的奥氏体合金,超耐热不锈钢(SuperAlloy Nitronic),该合金即使在剧烈成型操作后也没有磁性。其它相关的材料包括:Carpenter 22Cr-13Ni-5Mn,HPA 50,XM-19。合金316也相对无磁性,但是当其变硬时会变得更加有磁性。合金成分如下

理想的范围是铁基合金需要下落到最小磁性的范围。
钴合金:钴合金是优秀的替换。这些合金很硬并且不会与磁场产生强烈影响。该合金的例子包括L-605和MP-35。钴合金优化了耐磨性、高温使用和/或耐蚀性。对于乳房活组织检查工具,耐磨性和耐蚀性是人们最关心的。提供这些性质的主要合金元素是加入铬(US873,745)。钼和钨是优秀的补强剂。碳的加入有效改善了耐磨性。加入2.4%的碳导致碳化物结构。这种合金的一个例子是商品名为Stellite的合金。用于改善耐磨性的替换方式是加入钼和硅的组合物。这种合金的一个例子是商品名为Tribaloy的合金。这种合金已经成功沉积为薄膜。
已经发现加入镍以改善高温性能。这种合金的例子是带有约2.5%的镍的Stellite21。以下的合金,例如X-40和L-605已经将镍含量增加到大约10%。通常,带有下面的成分范围的合金对钴基、高抗张性、高硬度材料最佳。

镍基合金:镍-铬-钼合金是获得坚硬、无磁性金属合金的另一途径。这类合金的一些成员具有多于5%的铁(Inconel 600)并且镍基合金即使在没有铁的条件下也能够具有显著的磁性。合金的成分和处理是它们的磁性和物理性能的关键。一些合金,例如Inconel 625具有超过95Rb的洛氏硬度(Rockwell)。

合成方法:软金属,例如钛或完全退火的316 SS具有适当的磁性,但是硬度低,从而切割能力差。这些金属能够通过下述处理在切割或穿刺表面局部硬化:(1)将坚硬材料的边缘钎焊、焊接或连接到软金属上;(2)对坚硬材料例如氮化钛的气相淀积(化学、真空等);(3)离子束注入;(4)通过激光或摩擦加热局部加热/处理以硬化;(5)或者上述方法的组合。
非金属材料选择:用于产生锋利的切割表面的其它非金属包括下面的非晶体/陶瓷材料:(1)氧化铝;(2)氧化锆(包括稳定的氧化钇);(3)二氧化硅。单晶体材料包括:(1)硅;(2)锗;(3)金刚石形式的碳;(4)蓝宝石形式的铝;(5)红宝石。使用材料的单晶体性质可以产生单晶体切割边缘。择优浸蚀(例如在1,0,0硅晶片上的醇-KOH)能够用于形成准确的角度,并由此形成锋利边缘。
穿刺件几何形状:刀片的几何形状对于优化穿刺组织的力很重要。早期的套管针上的锥形设计已经在最近由扁平刀片设计代替。点和切割角度的理论追溯到18世纪的Augur刀刃。近年来,为了磨损问题等而研究、发展和精确了切割理论。因为对于特定直径的刀片扭矩和推力(医生推进套管针的量)是固定的,决定这种最优化的关键因素是尖端的几何形状。当尖端分离组织的层时,大多数(几乎90%)的穿刺力由刀尖控制。使用较低穿刺力的好处是这会产生较少的疼痛。当插入刀片时在两个方向存在120度扭转动作。没有测量刀片推入的推力。可以假设套管针以约5lbs推入。切割刀刃是是尖端的主要元件,它分开(切割)组织。在目前的设计(图77)中,存在带有垂直表面的扁平刀片的30-35度尖锐锥角。如图78A-78B所示,本发明研究并优化了以具有切割角和在中心的第二平点角度的偏移切割边缘优化尖端设计。
定义:动态切割角(αdyn):在通过位于切割边缘上的点并垂直于水平线的平面上测量在前倾面和包括切割边缘和切割速度向量两者的平面的法线之间的角,其中上述水平线经过位于切割边缘上的点并与钻孔中心轴相交。切割速度向量是旋转切割速度向量和进刀速度向量的向量和。这是可以用于分离组织层的切割角度,并且在图79中示出了对于正角的几何形状,而在图80中示出了对于负角的几何形状。
如上所述,在切割刀片中任何给定的点存在两个速度向量。在目前的设计中,当刀片垂直于该切割边缘时,α=0。假设将刀片的切割边缘分为几个小部分(N)。假设使各部分经历垂直切割。可以基于几何因素得到在任何瞬间和位于切割边缘上的空间位置计算动态前倾角的方法。各瞬间的扭矩可以由下面的公式确定:

在公式中,αd(动态切割角)和γ(i)(钻轴的各部分的半径)对于在切割边缘上的各元件是不同的。
当前设计和建议设计之间的不同之处在于,切割边缘的宽度(WOC)改变,切割角可以更陡(在40-60度的范围内)。当给定1 in-lb扭矩和X lbs推力时,这是切割的相反问题。什么是在尖端得到较小的穿刺力的最好的几何形状以?这可以通过分析得到并在增压舱中进行测试。

这在偏移切割边缘并制造40-60度的更有侵入性的切割角的情况下是可能的。
切割边缘也能够具有多个刀片(例如4个)以增加WOC。切割边缘不应该是锋利的,以避免损害。它可以具有5千弧度以最优化穿刺力。如图81A-81B所示,可以如下地进一步优化扁平刀片。
在图82-86中,闭塞器3000在设置多功能腔3006的中空轴3004上使用扁平刀片3002。在图83中,扁平刀片3002被固定在于两个远侧端倾斜的三角支撑3010、3012之间形成的垂直狭缝3008内。中空轴3004的近侧端3014设置有压力配件3016,以便采用腔3006向最接近扁平刀片3002的可成像侧凹口3018进行气体或流体传送。在图82、84中,近侧端3014上的外部接合件包括邻近周向环形槽3022的周向凸起环3020。在图84中,通过相对的横向侧到达可成像侧凹口3018的通气孔3024允许在套管内实现压力平衡或在套管中使用真空腔(图82-86中未示出)。在图85、86中,顶部导向槽3026使中空轴3004的近侧部分3014沿纵向向下经过,从而可用键锁定与套管的接合,以使可成像侧凹口3018对准套管中的侧孔。在图82、84中,可成像侧凹口3018的倒圆的前缘和尾缘3028、3030最小化组织损害。可替换地,顶部导向槽3018可以允许视觉指引,从而可以在穿刺时确定可成像侧凹口3018旋转脱离与侧孔的对准,以防止组织进入成像侧凹口3018。其后,为了成像确认和/或使用多功能腔3006,可将可成像例凹口3018通过旋转而对准。
优选的是,具有一次性的基准仪,其可由最终用户有利地填充,并且甚至可以丢弃掉。由此,通过允许空的基准仪具有延长的保存期、简化的消毒过程、简化的保存(例如,广泛的温度范围)和减少的包装要求来提高临床适应性。另外,最终用户可以选择造影剂或其它可成像材料。

Claims (10)

1.一种在微创医疗过程中用于人体乳腺组织的器械,该器械包括:
由磁共振成像(MRI)相容材料形成的插管,其包括开口的近侧端和较远的开口;
包括由MRI相容材料形成的中空轴的闭塞器,将其尺寸设定成可插入所述插管中,并且包括一种当如此插入以控制其中的组织的下垂时最接近插管的较远的开口的结构;
邻近突出其识别的插管的较远开口的闭塞器的MRI可成像部分;以及
在该闭塞器中形成的腔,其连通在人体乳腺组织外部的近侧部分和插管的较远开口。
2.根据权利要求1所述的器械,还包括由MRI相容材料形成的穿刺尖端,其被固定到从插管和闭塞器中选择的一个的远端上。
3.根据权利要求2所述的器械,其特征在于,从插管,闭塞器和穿刺尖端中选择的一个包括液晶聚合物。
4.根据权利要求2所述的器械,其特征在于,从插管,闭塞器和穿刺尖端中选择的一个包括PEEK材料。
5.根据权利要求2所述的器械,其特征在于,穿刺尖端包括重叠注塑的陶瓷尖端。
6.根据权利要求2所述的器械,其特征在于,穿刺尖端包括一个近侧的锥体,该锥体具有一个渐变为由侧切削刃终止的钝角的远侧部分。
7.根据权利要求2所述的器械,其特征在于,穿刺尖端包括一锥体,将该锥体一分为二的平面为一扁平的通常为三角形的切削面。
