受控的释放系统可以以预定的速度在一定的时间期限内进行某种药物的体内递送。释放速度一般由该系统的设计而决定并且很大程度上不依赖于环境条件,例如pH。已知一些受控释放系统可以在数年期间递送药物。反之,持续释放系统在典型的情况下在24小时或者更短时间内递送药物,并且一些环境因素可影响该释放速度。由此,药物从一种植入的受控释放系统(一种“植入物”)中的释放速度为该载体植入材料以及药物本身的生理化学特征的函数。在典型的情况下,该植入物由一种惰性材料制成,该材料引发很小的宿主反应或者不引发宿主反应。

一种膜或者储存器植入物依赖于某种生物活性物穿过聚合物膜的扩散。基质植入物由一种聚合物基质组成,生物活性物在该基质中均一地分布。膨胀控制下的释放系统通常基于亲水性的玻璃状聚合物,该聚合物在生物液体或者特定的环境刺激物存在下进行了膨胀。

植入物材料可以是生物可降解的或者生物可侵蚀的。生物可侵蚀的植入物的优势在于该植入物无需从患者体内除去。生物可侵蚀植入物可基于生物活性物质从膜或者基质中的释放。由PLA-PGA制备的生物可降解的微球体已知可用于皮下或者肌内给药。

蛋白质植入物

肉毒杆菌毒素

肉毒杆菌毒素(它们是细胞内肽酶)的生物活性一般同酶一样依赖于,至少部分依赖于它们的三维构象。因此,A型通过加热、多种化学物的表面拉伸作用(surface stretching)以及表面干燥作用而失去毒性。另外,将从已知的培养、发酵和纯化中获得毒素进行稀释至用于药学组合物制剂的非常低的浓度会导致该毒素迅速失去毒性,除非存在一种适当的稳定剂。将毫克量的毒素稀释至含有纳克每毫升的溶液中的过程存在着很大的难度,这是由于特异毒性在这种高稀释度下会迅速失去。另外,该毒素可在含它的药学组合物制剂化后数月或者数年使用。很明显,已知该毒素可在制造和配制期间以及贮藏期间通过稳定剂的使用来进行稳定化,稳定剂的例子如白蛋白和凝胶。
含有肉毒杆菌毒素的商业化药学组合物以商标BOTOX(可购自Allergan,Inc.,of Irvine,California)进行销售。BOTOX由一种纯化的A型肉毒杆菌毒素复合物、白蛋白以及氯化钠组成,它们以消毒的真空干燥形式包装。A型肉毒杆菌毒素从肉毒梭状芽孢杆菌的Hall菌株培养物中制备,该菌在含有N-Z氨基酸和酵母抽提物的培养基中生长。A型肉毒杆菌毒素通过一系列的酸沉淀而从培养物溶液中纯化为一种结晶复合物,该复合物由活性高分子量毒素蛋白以及一种相关的红血球凝集素蛋白组成。将该结晶复合物重新溶解于含有盐和白蛋白的溶液中并且在真空干燥之前进行无菌过滤(0.2微米)。经真空干燥的产品在5℃或低于5℃的冷柜中保存。BOTOX可在肌内注射前以无菌的无防腐剂的盐水进行重构。每药瓶BOTOX含有无菌的真空干燥形式的约100单位(U)的A型肉毒梭状芽孢杆菌毒素纯化神经复合物、0.5毫克的人类血清白蛋白以及0.9毫克的氯化钠,不含防腐剂。
为重构真空干燥的BOTOX,通过将适量的稀释液吸入适当大小的注射器而使用不含防腐剂的无菌普通盐水(0.9%氯化钠注射液)。由于BOTOX可能会通过导入气泡或者类似的剧烈搅拌而变性,因此将该稀释液轻轻地注射入药瓶。出于无菌的目的,BOTOX在优选的情况下在药瓶从冷柜取出并进行重构后4小时内进行给药。在这四小时期间,重构好的BOTOX可保存于约2℃到8℃的冰箱中。重构的冷藏BOTOX在至少2周内保持活性。Neurology,48:249-53:1997。
已有报道A型肉毒杆菌毒素已被用于临床,如下:
(1)每次肌内注射(多个肌肉)约75-125单位的BOTOX以治疗子宫肌张力异常;
(2)每次(向降眉间肌进行5单位的肌内注射并且向各皱眉肌肉进行10单位的肌内注射)肌内注射5-10单位的BOTOX以治疗眉间皱纹(眉沟);
(3)通过以30-80单位BOTOX对耻骨直肠肌进行括约肌内注射以治疗便秘。
(4)以每块肌肉约1-5单位来向上眼睑的侧睑板前眼轮匝肌以及下眼睑的侧睑板前眼轮匝肌进行肌内注射BOTOX以治疗睑痉挛;
(5)为治疗斜视,以1-5单位的BOTOX向眼外肌肉进行肌内注射,所注射的量根据被注射的肌肉的大小以及所需的肌肉麻痹的程度(即,所需的屈光度校正量)而变化;
(6)通过向5个不同的上肢屈肌进行BOTOX的肌内注射以治疗上肢的卒中后痉挛,如下:
(a)flexor digitorum profundus:7.5U至30U
(b)flexor digitorum sublimus:7.5U至30U
(c)flexor carpi ulnaris:10U至40U
(d)flexor carpi radialis:15U至60U

A型肉毒杆菌毒素在治疗多种临床症状中的成功已经引起了对其它肉毒杆菌毒素血清型的兴趣。已经开展了对两种商业化的A型肉毒杆菌毒素制剂(BOTOX和Dysport)以及B和F型肉毒杆菌毒素制剂(均获自Wako Chemicals,Japan)的研究以测定局部肌肉弱化效果、安全性和抗原性潜能。将肉毒杆菌毒素制剂注射入右侧腓肠肌头部(0.5-200.0单位/kg)并且使用小鼠趾外展评测分析(DAS)来测定肌肉弱化。从剂量反应曲线计算ED50值。对另外的小鼠进行肌内注射以测定LD50剂量。根据LD50/ED50来计算治效指数。分组小鼠接受BOTOX(5.0-10.0单位/kg)或者B型肉毒杆菌毒素(5.0-400.0单位/kg)的后肢注射并且进行肌肉弱化以及水消耗增加的测试,后者为口干的一种推测模型。抗原性潜能通过在兔体内的每月肌内注射而测定(对于B型肉毒杆菌毒素为1.5或者6.5ng/kg,对于A型肉毒杆菌毒素为0.15ng/kg)。对于所有的血清型,峰值肌肉弱化和持续时间与剂量相关。DAS ED50值(单位/kg)如下:BOTOX:6.7,Dysport:24.7,B型肉毒杆菌毒素:27.0-244.0,F型肉毒杆菌毒素:4.3。BOTOX同B型肉毒杆菌毒素和F型肉毒杆菌毒素相比具有更长的活性持续时间。治疗指数值如下:BOTOX:10.5,Dysport:6.3,B型肉毒杆菌毒素:3.2。经B型肉毒杆菌毒素注射的小鼠体内的水消耗高于BOTOX,尽管B型肉毒杆菌毒素在弱化肌肉上较为低效。注射后4个月,4只兔子中的2只(经1.5ng/kg的处理)以及全部4只兔子(经6.5ng/kg的处理)产生了抗B型肉毒杆菌毒素的抗体。在一个独立的研究中,9只经BOTOX处理的兔子中无一显示出抗A型肉毒杆菌毒素的抗体。IAS结果显示出A型肉毒杆菌毒素的峰值毒力与F型相当,而F型肉毒杆菌毒素的峰值毒力高于B型肉毒杆菌毒素。至于效力持续时间,A型肉毒杆菌毒素高于B型肉毒杆菌毒素,并且B型肉毒杆菌毒素的效力持续时间高于F型肉毒杆菌毒素。根据治疗指数值的显示,两种商业化的A型肉毒杆菌毒素制剂(BOTOX和Dysport)有所不同。在后肢注射B型肉毒杆菌毒素后所观察到的水消耗行为的提高显示临床上相当可观数量的该血清型进入了该小鼠的体循环。该结果还显示,为获得可与A型肉毒杆菌毒素相当的效果,必须提高其它被测血清型的剂量。提高的剂量可具有安全性。进一步,在兔体内,B型比BOTOX更具有抗原性,这可能是由于为达到有效剂量的B型肉毒杆菌毒素而注射的较高蛋白量。Eur J Neurol 1999Nov;6(suppl 4):S3-S10。
除在外周部位具有药学活性外,肉毒杆菌毒素还可在中枢神经系统中显示去神经支配效果。Wiegand et al.,Naunyn-Schmiedeberg′sArch.Pharmacol.1976;292,161-165以及Habermann,Naunyn-Schmiedeberg′s Arch.Pharmacol.1974;281,47-56中报道肉毒杆菌毒素能够通过逆向运输上升至脊髓区域。同样,在外周部位,例如通过肌内进行注射的肉毒杆菌毒素,有可能被逆向运输至脊髓。
美国专利No.5,989,545披露,一种经修饰的肉毒梭状芽孢杆菌神经毒素或其片段,在优选的情况下为一种肉毒杆菌毒素,同一种特定的靶定部分进行化学偶联或者重组融合后可通过以该试剂对脊髓进行给药而用于治疗疼痛。
乙酰胆碱
在典型的情况下,哺乳动物神经系统的各种类型的神经元仅仅释放一种类型的小分子神经递质。神经递质乙酰胆碱被诸多脑部区域的神经元所分泌,但具体地为运动皮层的大的锥形细胞、基底节的不同神经元、支配骨骼肌的运动神经元、自主神经系统(交感神经或者副交感神经)的节前神经元、副交感神经系统的节后神经元以及交感神经系统的某些节后神经元所分泌。基本上,因为大部分交感神经系统的节后神经元分泌神经递质去甲肾上腺素,因此只有汗腺、立毛肌以及少量的血管的节后交感神经纤维是胆碱能的。在多数情况下乙酰胆碱具有激动效果。然而,已知乙酰胆碱在某些外周副交感神经末梢具有抑制效果,例如通过迷走神经对心律的抑制。
自主神经系统的输出信号通过交感神经系统或者副交感神经系统被传递至躯体。交感神经系统的节前神经元由位于脊髓的中间侧角的节前交感神经元细胞体延伸而来。延伸自细胞体的节前交感神经纤维同位于脊椎旁的交感神经节或者位于脊椎前的神经节中的节后神经元形成突触。由于交感和副交感神经系统的节前神经元均为胆碱能,对这些神经节使用乙酰胆碱将激动交感和副交感节后神经元。
乙酰胆碱激活两种受体,毒蕈碱受体和烟碱受体。毒蕈碱受体见于所有受副交感神经系统的节后神经元以及交感神经系统的节后胆碱能神经元刺激的效应器细胞。烟碱受体见于肾上腺髓质以及自主神经节,该受体位于交感和副交感神经系统的节前和节后神经元之间的突触中的节后神经元细胞膜表面上。烟碱受体亦可见于多种非自主神经末梢上,例如位于骨骼肌纤维的神经肌肉接头的细胞膜上。

最后,用于形成一种与金属阳离子复合的神经毒素的方法包括以下步骤:(a)形成一种含有一种神经毒素的溶液;(b)在适于将多价金属阳离子同该神经毒素复合的pH下,将一种多价金属阳离子成分分散于该神经毒素溶液中,由此而形成了一种与金属阳离子复合的神经毒素悬浮液,其中的金属阳离子成分同神经毒素之间的摩尔比为约4∶1至约100∶1;(c)使所述的悬浮液干燥以形成与金属阳离子复合的神经毒素。

定义

“生物相容性”意指使用该植入物时在该植入位点只有不明显的炎症反应。

详述
本发明基于一种发现,即,含有一种生物相容性的非生物可降解或者生物可降解性的聚合物的植入物可表现出治疗有效量的神经毒素在较长时间内的体内释放。
本发明的一种植入物可经手术而通过切开插入目的效应位点(即,用于减少肌肉痉挛的位点),或者,该植入物可使用一种中空的针头移植枪,例如通过美国专利No.4475,572所披露的类型进行皮下或者肌内给药。该针头的直径可针对所使用的植入物的大小而进行调整。进一步,可以以本发明范围内的植入物进行颅内植入从而向某种目标脑组织提供一种治疗量的神经毒素的长期递送。当本发明范围内的非生物降解植入物被用尽时,该植入物的除去并非必要,这是因为该植入物由一种生物相容性的无免疫原性材料组成。
为使神经毒素稳定于能够同适当的可形成植入物基质的聚合物进行混合的形式(即,经冻干或者冷冻干燥的粉末状神经毒素),或者为使神经毒素在存在于或者整合入选定聚合物的基质时稳定,可以使用各种不同的药用赋形剂。适当的赋形剂可包括淀粉、纤维素、滑石、葡萄糖、乳糖、蔗糖、明胶、麦芽、稻米、面粉、碳酸钙、硅胶、硬脂酸镁、硬脂酸钠、甘油单硬脂酸酯、氯化钠以及脱脂奶粉。
该植入物的厚度可被用于控制本发明的组合物对水的吸收以及由此而控制神经毒素从其中的释放速度,较厚的植入物释放多肽的速度慢于较薄的植入物。
在最初阶段即爆发期(burst period),该植入物可能迅速地释放次最佳量的神经毒素。一般情况下这一爆发期持续不到24小时并且通常在植入后持续约一小时。此后,该植入物所释放的神经毒素的量迅速降低并且稳定于显著降低并且相当恒定(即,零级反应动力学)的神经毒素释放水平。这一较长的神经毒素释放的第二阶段可延续约一年到约5或6年的时间。该第二阶段的初始部分可被称为成形期(makeup period)。
在爆发期和成形期所释放的加和量的神经毒素在优选的情况下等于治疗特定的失调或疾患的最佳量的神经毒素。成形期的时间在一定程度上短于一定的时间范围,在该时间范围之后该神经毒素的最佳给药表现出显著降低的效力。例如,为治疗上肢痉孪,肌内A型肉毒杆菌毒素的最佳量可为注射入肱二头肌的90个单位。在典型情况下,快速浓注后1-7天内所诱导出的松弛性麻痹在约3个月后基本耗尽。本发明范围内的神经毒素皮下植入物可经设置而在植入后基本上立即(即,在爆发期间)释放约60个单位的肉毒杆菌毒素。这一次最佳量的神经毒素提供了迅速的基本病痛解除。在第二阶段该植入物连续地释放约0.4单位/天的神经毒素,例如A型肉毒杆菌毒素,由此而使90个单位的最佳量在约75天后通过该植入物完全释放入靶组织。
尚未鉴定出肉毒杆菌毒素能表现出高度特异亲和性的突触前神经元受体。同样没有已被接受的机制来说明肉毒杆菌毒素的神经元内的长半衰期。虽然如此,已知一种动力学过程发生并且造成了由肉毒杆菌毒素的给药所产生的麻痹效果的渐进式消损,该动力学可能为该肉毒杆菌毒素受体去阻断、复现、再合成和/或重激活,或者为新神经萌芽的出现,抑或二者兼备。因此,当在上例中,最佳量(总计90个单位)的肉毒杆菌毒素由该植入物在75天释放时,由于肉毒杆菌毒素的效果的衰减的动力学特征,由植入物进行的毒素的后续释放(即超过75天)不导致不需要的或多余的麻痹区域。约为需要去神经支配的组织的最佳直径的2/3。此后神经毒素在形成期及其后的释放提供了该组织在最佳或者所需程度上的去神经支配,并且提供了用于在该靶组织中的新近复神经化位点进行重新去神经支配的神经毒素的量。
已知睑痉挛可通过以5单位(每隔2-4个月重复一次)的A型肉毒杆菌毒素向侧面前眼睑软骨轮匝肌进行肌内注射来进行治疗。很明显,我们的发明范围之内的一种单个植入物可被用于睑痉挛的在诸如一年这样的期间的治疗。对于这种病痛以及通过神经毒素的植入物释放的治疗所选定的1年时间以及所使用聚合物的15%爆发特性,装载入该植入物的总神经毒素可为20个单位。在爆发期间,约3单位的毒素被释放(植入后24个小时之内),继之以每天约0.0467单位连续释放(即,每天释放约2.3皮克的BOTOX)。因此,到第42天,总量约5单位的神经毒素被释放。在该例中释放速度(15%爆发释放,剩余部分在364天内释放)为0.234%/天。在该例中,在第一天该患者接受了一个20单位的植入物,并且在一年之后将该耗尽的植入物从该患者中取出并且插入另一个20单位的植入物。这样,将第二植入物在第365天的效果包括在内,在365天期间以25个单位进行了给药。
由于一摩尔(M)的A型肉毒杆菌毒素复合物含有约9×105克,因此一皮克(pg)的A型肉毒杆菌毒素复合物为约1.1×10-18M。因此,0.234%/天的总掺入神经毒素的所需释放相当于约2.53×10-18M/天的释放。对于一年的治疗期,20%爆发释放以及随后的364天80%的释放的结果是约0.22%/天或者0.044单位/天或者2.2皮克/天或者2.42×1018M/天的受控释放。从20单位的植入物的20%爆发释放在植入后最初的约24小时内提供了4单位的神经毒素。一般情况下,该植入物的表面积相当于每y cm2的植入物表面积为x单位的毒素释放/天。
用每次注射约5单位至约100单位的肉毒杆菌毒素注射治疗不同的状况。对于在一年时间内需要的最佳药剂量为25单位A型肉毒杆菌毒素的状况,对其进行治疗的典型植入物可装载有100单位的A型肉毒杆菌毒素复合物。爆发释放可为20%,随后在364天释放80%,这相当于0.22%/天或者0.22单位/天或者11皮克/天或者约1.21×10-17M/天。
对于20次以25单位进行注射的5年治疗期,第一次注射在零时间进行,第20次注射在第57个月进行,总系列注射量为500单位。与之对比,使用我们的发明,用于治疗反应于25单位肉毒杆菌毒素,例如反应于25单位的A型肉毒杆菌毒素的状况的一种5年植入物可以由装载有500个毒素单位的植入物来制成,该植入物的特征在于20单位的爆发释放(4%爆发释放),继之以在1736天内进行的480单位的释放,这相当于该植入物释放0.267单位/天或者5.56×10-4%/天或者13.35皮克/天。
可通过将一种选定的聚合物溶解于一种适当的溶剂中来制造基质植入物。将所需量的冻干或者冷冻干燥的粉末状神经毒素(即,用于在该治疗期间进行释放的全部所需神经毒素,例如未经重构的BOTOX)混合入这种制备溶液。这种方法可被用于制造包被的植入物微粒,其改动在于在本发明的一个实施方案中所使用的包衣为一种生物可侵蚀聚合物,该聚合物对于神经毒素是可透过的。因此,该神经毒素在该包衣降解之前并不从该基质扩散入周边组织。
由于在pH高于7时稳定用的非毒素蛋白从肉毒杆菌毒素解离并由此造成毒性的逐步消失,用于进行肉毒杆菌毒素的混合的制备溶液或者其它溶液的pH被维持于4.2-6.8。在优选的情况下,该pH为约5-6。进一步,该混合物/溶液的温度不应高于约35摄氏度,这是由于该毒素在高于40摄氏度的溶液/混合物中易于失去毒性。
本发明的范围之内的,用于一种神经毒素的体内受控释放的适当的植入物可经制备以使该植入物以连续或者脉冲方式释放该神经毒素。“连续释放”意指毒素在初始的爆发期之后以一种基本上是单相的方式进行释放。连续释放可具有一个拐点,但不具有平台期。连续释放并不需要每单位时间从该植入物中释放相同量的神经毒素。脉冲释放植入物可以双相或者多相方式释放神经毒素。这样,脉冲释放植入物可具有相对短的初始诱导(爆发)期,继之以释放少量或者不释放神经毒素的时期。
具有生物活性的神经毒素的受控释放是一种可产生治疗效果的释放,该释放在一定时期内产生血清水平可忽略的生物活性神经毒素,该时期长于由水相神经毒素的直接给药所获得的作时期。在优选的情况下,受控释放为神经毒素在约6个月或者更长时间内的释放,并且在更优选的情况下为在约一年或者更长时间内的释放。
本发明的范围之内的,用于诸如肉毒杆菌毒素这样的神经毒素的体内受控释放的适当的植入物可表现出该神经毒素的连续释放或者脉冲释放。另外,该植入物可含有一种非生物可降解的或者生物可降解的聚合物材料。很明显,我们的发明包含了:(1)连续释放的非生物可降解的神经毒素植入物;(2)连续释放的生物可降解的神经毒素植入物;(3)脉冲释放的非生物可降解的神经毒素植入物;和(4)脉冲释放的生物可降解的神经毒素植入物,而且这四种类型的植入物包含内容均可经制剂化成为各种构型,这些构型适用于皮下注射或者植入,例如颗粒、片、微球体、薄膜、杆和管构型,这些构型均具有,例如,包被于贮存器上的一种或者多种包衣。
我们的发明的范围之内的植入物可被制剂化而成为一种注射用的悬液。这样的悬液可通过药学领域中所熟知的工艺进行制造,例如将聚丙交酯/多肽混合物在一种超速离心研磨器中进行研磨并将经研磨和过滤的颗粒悬浮于一种注射用溶剂来,该超速离心研磨器配以一种适宜的网筛,例如120孔的,该注射用溶剂的例子如丙二醇、水、油或者其它已知的适宜注射用液体载体,在水中可根据情况加入一种传统的粘度提高剂或者悬浮剂。
被包被的神经毒素在体内37摄氏度下较长时间后的变性可通过稳定化该神经毒素而降低,该神经毒素的稳定化通过将其同白蛋白一起冻干、从一种酸性溶液中进行冻干、从一种低含水量的溶液中冻干(对于A型肉毒杆菌毒素,这三个条件可通过使用非重构的Botox来达到)以及使用一种特定的聚合物基质组合物来进行。
在优选的情况下,具有生物活性的神经毒素的体内释放并不在该神经毒素的释放期间导致明显的免疫系统反应。
基质稳定化神经毒素
我们已经披露,稳定化的神经毒素可含有具有生物活性的非聚集的神经毒素,该神经毒素复合有至少一种类型的多价金属阳离子,该离子具有二价或者更高的价态。
适当的多价金属盐离子包括生物相容性金属阳离子成分中所包含的金属离子。如果金属阳离子成分在其用量下对于接受者无毒并且不存在对接受者身体的明显的毒副作用或者不良效果,例如注射位点的免疫反应,则该金属阳离子成分为生物相容性的。
在优选的情况下,对于以金属阳离子成分进行稳定化的神经毒素,金属阳离子成分同神经毒素的摩尔比为约4∶1至100∶1,在更为典型的情况为约4∶1至10∶1。
用于稳定化神经毒素的一种优选的金属阳离子为Zn++。二价锌离子之所以较为优选是因为肉毒杆菌毒素已知为一种二价锌的肽链内切酶。在一个更为优选的实施方案中,包括Zn++阳离子在内的金属阳离子成分同神经毒素的摩尔比为6∶1。
用于对神经毒素进行稳定化的金属离子的适用性可由本领域的普通技术人员通过实施多种稳定性显示工艺来测定,该稳定性显示工艺的例子如聚丙烯酰胺凝胶电泳、等电聚焦、反相色谱、HPLC以及效力检验,该效力检验针对含有金属阳离子的神经毒素冻干颗粒进行,以此测定该神经毒素在冻干之后以及从微颗粒中释放期间的效力。在稳定化的神经毒素中,通过该神经毒素同至少一种类型的金属阳离子在该神经毒素同聚合物溶液接触前的复合,降低了神经毒素在体内水合期间在微颗粒中发生聚集和/或由于形成受控释放组合物的过程或者由于受控释放组合物的化学性质而失去生物活性或者效力的倾向性。
通过我们的发明,稳定化的神经毒素通过稳定化而防止了其在受控释放期间显著的体内聚集。显著的聚集被定义为一定量的聚集,该量的聚集导致了15%或更多的包被于聚合物中或者掺入聚合物基质中的神经毒素的聚集。在更优选的情况下,聚集被维持低于存在于聚合物中的神经毒素的约2%。
神经毒素受控释放组合物中的神经毒素还可以同其它赋形剂进行混合,例如增量剂或者额外的稳定化试剂,如用于在冻干期间对该神经毒素进行稳定化的缓冲液。
增量剂一般含有惰性材料。适当的增量剂为本领域的技术人员所知。
适用于本发明的受控释放组合物的聚合物或者聚合物基质必须具有生物相容性。如果一种聚合物以及该聚合物的任何一种降解产物对于接受者没有毒性并且对接受者身体并不产生明显的伤害或者不良作用,例如在注射位点的免疫反应,该聚合物为生物相容性的。
神经毒素受控释放组合物的聚合物可由生物可降解材料制成。根据本文的定义,生物可降解指的是该组合物将在体内经过降解或者侵蚀而形成化学物质。降解可通过酶、化学和物理过程所导致。
适当的生物相容性的生物可降解聚合物包括,例如,聚丙交酯、聚乙交酯、聚丙交酯共聚乙交酯、聚乳酸、聚乙醇酸、聚乳酸共聚羟基乙酸、聚己内酰胺、聚碳酸酯、聚酯酰胺、聚酐、聚氨基酸、聚原酸酯、聚氰基丙烯酸酯、聚二噁烷酮、聚草酸烷基酯、生物可降解的聚氨基甲酸乙酯,以及它们的混合物和共聚物。
进一步,该聚合物的末端功能可进行修饰。例如,聚酯可为一种封端、非封端的聚合物或者二者的混合物。根据本领域的经典定义,封端的聚合物特异性地具有封端的羧基末端基团。一般来说,封端基团来自于聚合化的起始物并且在典型的情况下为一种烷基。未封端的聚合物一般具有游离的羧基末端基团。
对于用于本发明的生物可降解的聚合物来说可接受的分子量可通过本领域的普通技术人员考虑一定的因素来确定,这些因素的例子如,所需的聚合物降解速度、诸如机械强度这样的物理性质、以及聚合物在溶剂中的溶解速度。在典型的情况下,可接受的分子量范围为约2,000道尔顿至约2,000,000道尔顿。在一个优选的实施方案中,该聚合物为一种生物可降解的聚合物或者共聚物。在一个更为优选的实施方案中,该聚合物为一种聚丙交酯共聚乙交酯(后文称为“PLGA”),其丙交酯∶乙交酯之比约为1∶1,并且分子量约为5,000道尔顿至70,000道尔顿。在一个更为优选的实施方案中,用于本发明的PLGA的分子量约为6,000道尔顿至31,000道尔顿。
包含于一剂受控释放微颗粒或者别的受控释放系统中的神经毒素的量,包括生物活性的稳定化的神经毒素颗粒在内,为一种治疗或者预防处理上有效的量,该量可由本领域中的普通技术人员考虑一些因素进行确定,这样的因素的例子如,体重、待治疗的状况、所使用的聚合物的类型以及从该聚合物中的释放速度。
在一个实施方案中,神经毒素受控释放组合物含有约10-4%(w/w)至1%(w/w)的生物活性的稳定化神经毒素。所使用的神经毒素颗粒的量根据所需的效果、计划的释放水平、应当释放该神经毒素的时间而变。神经毒素颗粒加载量的优选的范围为约10-4%(w/w)至约0.1%(w/w)的神经毒素颗粒。更为优选的神经毒素加载量范围为约10-3%(w/w)至约1%(w/w)的神经毒素,最为优选的生物活性稳定化神经毒素颗粒加载量为约10-2%(w/w)。
在另一个实施方案中,神经毒素受控释放组合物还含有第二种金属阳离子成分,该成分并不包含于稳定化的神经毒素颗粒中,并且该成分分散于该聚合物中。这第二种金属阳离子成分在优选的情况下含有与稳定化的神经毒素所含相同的金属阳离子种类。或者,这第二种金属阳离子成分可含有一种或者多种不同种类的金属阳离子。
这第二种金属阳离子成分作用于调控该神经毒素从聚合物基质中的释放,例如,通过作为金属离子的一种贮存器发挥作用以进一步延长神经毒素受金属阳离子稳定化的时间长度,从而增强该神经毒素在该组合物中的稳定性。
在对释放进行调控中所使用的金属阳离子成分在典型的情况下含有至少一种类型的多价金属阳离子。适于对神经毒素的释放进行调控的第二种金属阳离子成分的例子包括或含有,例如,Mg(OH)2、MgCO3(例如4MgCO3Mg(OH)25H2O)、ZnCO3(例如3Zn(OH)22ZnCO3)、CaCO3、Zn3(C6H5O7)2、Mg(OAc)2、MgSO4、Zn(OAc)2、ZnSO4、ZnCl2、MgCl2以及Mg3(C6H5O7)2。第二种金属阳离子成分同聚合物的适当的比例为重量比约1∶99至约1∶2。这一最佳比例取决于所使用的聚合物和第二种金属阳离子。
本发明的神经毒素受控释放组合物可形成多种形状,例如薄膜、颗粒、柱体、片或者微颗粒。根据本文定义,微颗粒含有一种聚合物成分,该颗粒的直径低于约1毫米并且在其中散布有稳定化的神经毒素颗粒。微颗粒可具有球形、非球形或者不规则形状。在优选的情况下微颗粒为一种微球体。典型情况下,该微颗粒的大小应适于注射。微颗粒的优选的大小为直径约1至约180微米。
本发明的用于形成一种进行生物活性的非聚集神经毒素的受控释放的组合物的方法中,适量的具有生物活性的稳定化的神经毒素被分散于一种聚合物溶液中。
一种适当的聚合物溶液含有约1%(w/w)至约30%(w/w)的适当的生物相容性聚合物,其中该生物相容性聚合物在典型情况下溶解于一种适当的聚合物溶剂中。在优选的情况下,聚合物溶液含有约2%(w/v)至约20%(w/v)的聚合物。含有约5%(w/v)至约10%(w/n)聚合物的聚合物溶液最为优选。
根据本文定义,适当的聚合物溶剂中该聚合物可以溶解,但在其中稳定化的神经毒素颗粒基本不溶并且无反应性。适当的聚合物溶剂的例子包括极性有机液体,例如二氯甲烷、氯仿、乙酸乙酯和丙酮。
为制备具有生物活性的稳定化的神经毒素,将神经毒素在一种适当的水性溶剂中,同至少一种适当的金属阳离子成分一起,在适于形成金属阳离子和神经毒素的复合物的pH条件下进行混合。在典型情况下,复合的神经毒素应为不透明的沉淀物,该沉淀物悬浮于该溶剂中。但是,该复合神经毒素亦可处于溶液之中。在一个甚至更优选的实施方案中,神经毒素同Zn++进行复合。
用于形成一种神经毒素复合物的适当的pH条件在典型的情况下包括约5.0至约6.9之间的pH值。适当的pH条件在典型的情况下通过使用一种含水的缓冲液作为溶剂来获得,例如碳酸氢钠。
适当的溶剂是那些可使神经毒素以及金属阳离子成分在其中至少微量可溶的溶剂,例如水性碳酸氢钠缓冲液。对于水溶剂,在优选的情况下,所使用的水为去离子水或者注射用水(WFI)。
在同金属阳离子成分进行接触之前,该神经毒素可处于固态或者溶解态。另外,在同神经毒素进行接触之前,该金属阳离子可处于固态或者溶解态。在一个优选的实施方案中,神经毒素的一种含水的缓冲溶液同该金属阳离子成分的一种水溶液进行混合。
在典型的情况下,复合的神经毒素应处于浑浊的沉淀物形式下,该沉淀物悬浮于该溶剂之中。但是,该复合的神经毒素也可处于溶液状态下。在一个优选的实施方案中,该神经毒素同Zn++进行复合。
复合于Zn++的神经毒素可通过诸如冻干这样的方法进行干燥,从而形成稳定化的神经毒素微粒。悬浮状态或者溶液之中的复合Zn++的神经毒素可经过大体积的冻干或者被分成较小体积然后冻干。在一个优选的实施方案中,使复合Zn++的神经毒素悬液体积减小,例如通过使用超声喷嘴,并且随后进行冻干从而形成稳定化的神经毒素颗粒。用于对复合Zn++的神经毒素混合物进行冻干的方法包括本领域所已知的方法。
在优选的情况下,稳定化的神经毒素颗粒直径为约1-6微米。该神经毒素颗粒可分开地进行碎片化,或者,该神经毒素颗粒可在被加入至一种聚合物溶液中后进行碎片化,碎片化的方法的例子如通过超声探头或者超声喷嘴的手段。
在另一个实施方案中,第二种金属阳离子成分也被悬浮于该聚合物溶液中,该成分并不包含于该稳定化的神经毒素颗粒之内。
应当理解,第二种金属阳离子成分和稳定化的神经毒素可按顺序、以相反顺序、间歇地、分开地或者通过同时添加而分散入一种聚合物溶液。在替代情况下,一种聚合物、第二种金属阳离子成分和稳定化的神经毒素可按顺序、以相反顺序、间歇地、分开地或者通过同时添加来进行混合。
在这种方法中,该聚合物溶液随后固化以形成一种聚合物基质,该基质含有稳定化的神经毒素颗粒的一种分散物。
用于从一种聚合物溶液形成一种神经毒素受控释放组合物的适当的方法为溶剂蒸发法,该方法在美国专利No.3,737,337;3,523,906;3,691,090以及4,389,330中有所描述。溶剂蒸发可被用作为形成神经毒素受控释放微颗粒的方法。
在溶剂蒸发法中,含有一种稳定化的神经毒素颗粒分散物的聚合物溶液被进行混合或者以连续相进行搅拌以形成一种乳剂,在其中该聚合物溶剂是部分可溶混的。该连续相通常为一种水性溶剂。在该连续相中通常包含有乳化剂以稳定该乳剂。该聚合物溶剂随后进行几个小时或者更长时间的蒸发,由此而使该聚合物固化以形成一种聚合物基质,该基质中含有稳定化的神经毒素颗粒的分散物。
用于由一种聚合物溶液而形成神经毒素受控释放微颗粒的优选的方法在美国专利No.5,019,400中有所描述。同其它方法相比,例如同相分离法相比,这种微球体形成方法额外降低了产生具有特定的神经毒素含量的受控释放组合物所需的神经毒素的量。
在这一方法中,含有稳定化的神经毒素颗粒分散物的该聚合物溶液经加工而产生一些小滴,其中至少相当一部分的小滴含有聚合物溶液和稳定化的神经毒素颗粒。随后通过适于形成微颗粒的方法进行冷冻。用于加工该聚合物溶液分散物以形成小滴的方法的例子包括将该分散物导向通过一种超声喷嘴、压力喷嘴、瑞利喷射器(Rayleighjet)或者通过其它用于由溶液形成小滴的设备。
适用于对小滴进行冷冻以形成微颗粒的设备包括将这些小滴导入或者靠近一种液化气体,例如液氩和液氮,从而形成冷冻的微颗粒,随后将这些微颗粒同该液态气体相分离。冷冻的微液滴随后被暴露于一种液态非溶剂,例如乙醇或者混合以己烷或者戊烷的乙醇。
冷冻的微液滴中的溶剂以液态和/或固态被抽提进入该非溶剂从而形成了含有微颗粒的稳定化的神经毒素。将乙醇与其它非溶剂进行混合,例如与己烷或者戊烷混合,可提高从特定的聚合物中进行溶剂抽提的速度,该速度高于通过单独使用乙醇所获得的速度,该聚合物的例子如聚(丙交酯共乙交酯)聚合物。
通过改变小滴的大小可制造出较大体积范围的神经毒素受控释放微颗粒,改变的方法的例子如,改变超声喷嘴的直径。如果需要很大的微颗粒,该微颗粒可通过用一个注射器直接注入冰冷液体。提高该聚合物溶液的粘度亦可增加微颗粒的大小。通过这一过程可产生一定大小的微颗粒,例如直径约1000至约1微米的微颗粒。
另一种用于由聚合物溶液形成神经毒素受控释放组合物的方法包括薄膜成形,例如在一种模具中进行,从而形成一种薄膜或者一种模型。例如,在将含有稳定化的神经毒素颗粒的分散物的聚合物溶液置入模具后,随后通过本领域的已知方法除去聚合物溶剂,或者降低该聚合物溶液的温度,直至获得具有相符的干重的薄膜或者模型。
对于生物可降解的聚合物植入物,神经毒素的释放是由于该聚合物的降解。降解的速度可通过改变影响聚合物的水合速度的性质来进行控制。这些性质包括,例如,形成该聚合物的不同的单体的比例,例如丙交酯和乙交酯;一种单体的L-异构体代替外消旋混合物的使用;以及该聚合物的分子量。这些性质可影响亲水性和结晶性,这控制了该聚合物水合的速度。诸如盐、碳水化合物和表面活性剂等疏水赋形剂也可被引入以增强水合作用,并且这些可以改变聚合物的侵蚀速度。
通过改变生物可降解聚合物的性质,可以控制扩散和/或聚合物降解对于神经毒素释放所产生的贡献。例如,提高聚(丙交酯共乙交酯)聚合物中乙交酯的含量并且降低该聚合物的分子量可增强该聚合物的水解作用并由此而提供了一种由聚合物侵蚀而产生的较高的神经毒素释放。另外,聚合物水解的速度在非中性pH下会有所提高。因此,可向用于形成微球体的聚合物溶液中加入一种酸性或者碱性赋形剂以改变该聚合物的侵蚀速度。
我们的发明的组合物可通过非全身该药途径,例如通过植入(例如,皮下、肌内、颅内和)对人类或者其它动物进行给药,从而提供所需剂量的神经毒素,该剂量基于以神经毒素对多种医学状况进行治疗的已知参数。
适于给药的具体植入剂量可由本领域的普通技术人员根据上文所讨论的因素很容易地确定。该剂量还可能依赖于待治疗或者去神经支配的组织大小以及该毒素的商业制剂。另外,对人体内的使用剂量的估计量还可由其它组织的有效去神经支配所需的肉毒杆菌毒素的确定量推导得出。因此,用于注射的A型肉毒杆菌毒素的量正比于待处理的组织的质量和活性水平。一般情况下,该植入物在每单位时间(即,每2-4月一次或者一段时间)内可释放每千克患者体重约0.01单位至约35单位肉毒杆菌毒素,从而有效地获得所需的肌肉麻痹。低于约0.01U/kg的肉毒杆菌毒素对于肌肉并不具有明显的治疗效果,而高于约35U/kg的肉毒杆菌毒素接近于神经毒素,例如A型肉毒杆菌毒素的毒性水平。小心地制备并且置放该植入物可防止大量的肉毒杆菌毒素在全身出现。一个更为优选的剂量范围为约0.01U/kg至约25U/kg的肉毒杆菌毒素,如制剂化为BOTOX的内毒杆菌毒素。用于给药的以u/kg表示的肉毒杆菌毒素的实际量取决于待治疗组织的范围(质量)和活性水平以及选择的给药途径。A型肉毒杆菌毒素是用于本发明的方法中的优选肉毒杆菌毒素。
用于实施本发明范围内的方法的神经毒素为肉毒杆菌毒素,如血清型A,B,C,D,E,F或G肉毒杆菌毒素之一。所使用的肉毒杆菌毒素优选为A型肉毒杆菌毒素,这是因为其在人类中的高效力、容易获得以及通过肌内注射对其进行局部给药用于治疗骨骼肌和平滑肌失调时的有效用途。
本发明在其范围内包括了任意一种神经毒素的使用,该毒素在被用于治疗一种运动失调或者由胆碱能的神经支配所影响的病痛时具有一种长期的治疗效果。例如,由产毒素梭菌,如,肉毒梭状芽孢杆菌、丁酸梭菌和巴氏梭菌,可被用于或者改造用于本发明的方法。另外,肉毒杆菌毒素血清型A、B、C、D、E、F或G均可被有利地用于本发明的实施,尽管根据上文的解释A型是最为优选的血清型。本发明的实施可提供从一个月至约5或6年的有效病痛缓解。
本发明在其范围内包括:(a)通过细菌培养、毒素抽提、浓缩、保存、冷冻干燥和/或重构而获得或者加工的神经毒素复合物以及纯净的神经毒素;以及(b)经修饰或者重组的神经毒素,该神经毒素有一个或者多个氨基酸被有目的地删除、通过已知的化学/生化氨基酸修饰程序或者通过使用已知的宿主细胞/重组载体重组技术进行修饰或者替换,还包括通过这样的方法制造的神经毒素的衍生物或者片段,并且包括一些神经毒素,这些神经毒素具有一个或者多个同其相连的靶定部分,该部分针对一种存在于某种细胞上的细胞表面受体。
用于本发明的肉毒杆菌毒素可以冻干或者真空干燥的形式在容器中真空贮藏。在冻干之前,该肉毒杆菌毒素可同药学可接受的赋形剂、稳定剂和/或者载体,例如白蛋白,进行结合。冻干或者真空干燥的材料可以以盐水或者水进行重构。
我们的发明还在其范围内包括了一种植入的受控释放神经毒素复合物的应用,该应用是为了提供一种慢性失调的治疗缓解,该失调的例子如运动失调。因此,该神经毒素可被一种适当的聚合物基质所包埋、吸收或者携带,该基质可被植入或者包被入皮下从而向所需的靶组织提供一年或者更长时间的该神经毒素的受控释放。允许多肽药物的受控释放的可植入的聚合物是已知的,并且可被用于制备一种适于插入或者皮下粘附的肉毒杆菌毒素植入物。例见,Pain 1999;82(1):49-55;Biomaterails 1994;15(5):383-9;Brain Res 1990;515(1-2):309-11以及美国专利号6,022,554;6,011,011;6,007,843;5,667,808和5,980,945。
用于确定给药的适宜路线和剂量的方法一般由参治医师根据病例决定。对于本领域的技术人员这样的判定是常规行为(例见,Harrison′s Principles of Internal Medicine(1998),edited byAnthony Fauci et al.,14thedition,published by McGraw Hill)。
实施例
以下的实施例阐述了本发明包含的具体的组合物以及方法并且并非意图限制我们的发明的范围。
实施例1
Zn++稳定化的神经毒素的形成
一百个单位的某种神经毒素,如未经重构的Botox,被溶解于碳酸氢钠缓冲液(pH6.0)以形成一种神经毒素溶液。由去离子水和二水合乙酸锌形成一种Zn++溶液并且随后将其轻轻搅拌加入神经毒素溶液中以形成一种Zn++神经毒素复合物。随后通过加入1%的乙酸而将该Zn++神经毒素复合物的pH调节至6.5到6.9。由此形成了一种含有不溶的Zn++稳定化的神经毒素的混浊悬浮沉淀物。由此制成了一种神经毒素(例如A型肉毒杆菌毒素)复合物,该毒素经稳定化而在随后掺入聚合物植入物基质时不产生严重的聚集。
实施例2
神经毒素受控释放颗粒
适于掺入一种聚合物或者可聚合溶液的神经毒素可为A型肉毒杆菌毒素(例如Botox),该毒素可以以一种冷冻干燥粉末的形式购得。另外,可将多种聚合物和共聚物进行混合并且以一种干燥状态进行贮藏,该贮藏过程对最终的植入物的效果并无影响。例如,一种使用UV固化引发剂的丙烯酸酯共聚物。该神经毒素可如上文实施例1所描述的那样同Zn++进行复合。Zn++稳定化的神经毒素复合物随后同未固化的丙烯酸酯共聚物、UV引发剂和某种酸(pH介于5.5和6.8)进行混合。将该混合物置入一种玻璃或者透明的塑料颗粒模具,该模具可允许UV光线透过。将该模具置入一种20℃下的温控水浴之中。以紫外光使颗粒固化的50秒,将颗粒包装并进行灭菌。UV固化过程的时间和强度应使低量的神经毒素被破坏或者变性。
该颗粒的大小以及颗粒中的神经毒素的含量由所需的应用所限定。当颗粒被植入时,该颗粒在体内进行水合,该过程稍微延迟了在该神经毒素从植入物中的初始爆发。以浓度为初始爆发所需的一部分神经毒素包被于该颗粒之外可抵消这种延迟。在本实施例中,该颗粒的效果应约为4到6个月。
实施例3
神经毒素受控释放制剂
为提高该颗粒有效递送神经毒素的时间可使用多层的材料。这样,内层材料可由聚乙烯吡咯烷酮/甲基丙烯酸甲酯共聚物制成。这种材料可维持高浓度的神经毒素复合物。将适量的神经毒素可如上文实施例1所描述的那样同Zn++进行复合并且该复合物随后同未凝固的共聚物、低温引发剂和某种酸(pH介于5.5和6.8)进行混合。将该混合物置于一种玻璃或者塑料颗粒模具之中。将该模具置入一种约35℃下的温控水浴之中约6到8小时。这形成了长期受控释放所需的神经毒素的贮存器。
为延长该神经毒素的释放,随后将第二种材料固化于该初始颗粒之周围。该材料经过高分子密度和生物相容性的选择。聚甲基丙烯酸甲酯(PMMA)是具有这样的特征的材料的例子。将该颗粒(上述的)同未固化的PMMA/低温引发剂一起置于一种模具(插入成型)之中。未固化PMMA的二次包被对于确保该颗粒的均一包被可能是必需的。在优选的情况下,该PMMA的厚度为0.5mm。在成型之后,以所需的初始爆发浓度的神经毒素对该颗粒的外部进行包被。该PMMA层应厚得足以产生贮存器中的神经毒素的延迟(高达3个月)。当该神经毒素达到该植入物的表面时可获得神经毒素的第二次较大爆发。这种二次爆发将继以长约3个月的该神经毒素的缓慢减速释放。在该实施例中,该颗粒效力约为7到9个月。
实施例4
多层神经毒素受控释放植入物
通过使用多层——高密度聚合物/低密度聚合物w/神经毒素——可以增加神经毒素受控释放的时间范围,但该植入物的大小也可能提高了。随着该植入物的大小的增加,该神经毒素在体内扩散到更大的区域,这可能会降低该植入物的效力。为避免这种情况,将该植入物被包入一种非透性的材料,例如钛。保持一个小的开口用于该神经毒素从包被的颗粒中的精确定点释放。这在一般情况下可有效地使该植入物具有显著不同的释放特征。这样做基本上还可以允许神经毒素可通透的较厚的聚合物切片,这有效地提高了该神经毒素释放的持续时间。
内层材料可由诸如聚乙烯吡咯烷酮/甲基丙烯酸甲酯共聚物这样的物质制成。这种物质可维持高浓度的神经毒素复合物。该神经毒素被复合以Zn++。随后将该复合物同未固化的共聚物、低温引发剂和某种酸(pH介于5.5和6.8)进行混合。将该混合物置于一种玻璃或者塑料颗粒模具之中。将该模具置入一种35℃下的温控水浴之中约6到8小时。这形成了长期受控释放所需的神经毒素的贮存器。

当完成了最后一层的高密度聚合物,使用一种钛颗粒作为模具。将该颗粒同未固化的PMMA一同置于该钛颗粒之中。封闭该颗粒的盖子并将之置入35℃下的强制通风炉中约6到8小时。该颗粒的盖子上具有一个22号开口用于该神经毒素的释放。在本实施例中,该颗粒的效力约为10个月到24个月。
实施例5
具有分层柱的神经毒素植入物
为在较长的时间范围内维持释放,一种替代性的方法是将一种高密度聚合物/低密度聚合物w/神经毒素的层结构置入上述的钛颗粒之中。对于应用的每一层,固化过程可在35℃下的强制通风炉中进行约6到8小时。该颗粒的直径应是所用的神经毒素的量的关键性决定因素。层的数量可决定该植入物将维持效力的时间。对于各层,PMMA层的厚度可为约0.5mm并且该低密度聚合物w/神经毒素可为约0.3mm。每加一层可获得约3个月的效力提高。具有2年寿命的植入物可通过将该植入物的长度提高至约6.4mm,加上约1mm的钛壳横断面总计约7.4mm。
在此披露的本发明的组合物和方法具有众多优势,包括以下部分:
1.一种单一植入物可被用于提供治疗效果,该治疗效果为一种神经毒素在一年或者更长的时间内的连续或者脉冲式给药。
2.该神经毒素被递送入一种局部组织区域而并无显著量的神经毒素出现在全身。
3.对患者的后期护理的需求降低。
4.对神经毒素的周期性注射以治疗某种状况的需求降低,该症状的例子如神经肌肉失调。
5.由于所需注射数的减少而带来的患者舒适度的提高。
6.患者依从性的提高。
我们的神经毒素受控释放制剂的一个优势包括了在靶组织产生长期的稳定的治疗水平的神经毒素。这些优势还包括通过降低需要的注射数量而提高患者依从性以及接受程度。

尽管本发明已经通过具体的优选方法进行了详细描述,本发明范围内的其它的实施方案、版本以及修正是可能存在的。例如,多种神经毒素可被有效地应用于本发明的方法中。另外,本发明包括了局部(即,肌内、腺体内、皮下和颅内)给药方法,其中,通过植入物,两种或者多种神经毒素,如两种或者多种肉毒杆菌毒素,被同时给药或者先后给药。例如,A型肉毒杆菌毒素可通过植入物进行给药直至失去临床反应或者产生中和抗体,随后通过植入物以B型或者E型肉毒杆菌毒素进行给药,或者,可以以肉毒杆菌毒素血清型A-G中的任意两种或者多种的组合进行局部给药以控制所需治疗结果的发生和持续时间。进一步,可通过植入物在神经毒素的给药之前、同时或者之后以非神经毒素化合物进行给药,从而在该神经毒素,如肉毒杆菌毒素开始产生其治疗效果之前提供一种辅助效果,如去神经支配的增强或者更快的发生。
我们的发明在其范围内还包括了一种诸如肉毒杆菌毒素这样的神经毒素在制备用于治疗一种运动失调和/或由胆碱能神经支配所影响的失调的药物中的应用,该药物的例子如一种受控释放植入物,该治疗通过经该神经毒素植入物进行的局部给药而进行。
因此,以下权利要求的精神和范围应不受限于上文所进行的优选
实施方案的描述。
Claims (20)
1.一种受控释放系统,该系统含有:
(a)一种聚合物基质,以及;
(b)一定量的神经毒素,该毒素位于该聚合物基质内,其中分部量的该神经毒素可在较长的时间范围内从该聚合物基质中释放,同时不产生明显的免疫系统反应。
2.权利要求1的受控释放系统,其中的神经毒素从该聚合物基质中以一种基本上连续或者单相的方式进行释放。
3.权利要求1的受控释放系统,其中神经毒素从聚合物基质中释放的时间范围为约10天至约6年。
4.权利要求1的受控释放系统,其中该聚合物基质由一种基本为非生物可降解的物质组成。
5.权利要求1的受控释放系统,其中该神经毒素包括一种多肽。
6.权利要求1的受控释放系统,其中该神经毒素包括一种突触前神经毒素。
7.权利要求1的受控释放系统,其中该神经毒素为一种梭菌神经毒素。
8.权利要求1的受控释放系统,其中该神经毒素为一种肉毒杆菌毒素。
9.权利要求1的受控释放系统,其中该神经毒素为一种肉毒杆菌毒素,该毒素选自A、B、C、D、E、F和G型肉毒杆菌毒素。
10.权利要求1的受控释放系统,其中该神经毒素为A型肉毒杆菌毒素。
11.权利要求1的受控释放系统,其中组成该聚合物基质的聚合物选自甲基丙烯酸酯、乙烯基吡咯烷酮、乙烯醇、丙烯酸、聚甲基丙烯酸甲酯、硅氧烷、乙酸乙烯酯、乳酸、乙醇酸、胶原和生物陶瓷聚合物及它们的共聚物。
12.权利要求1的受控释放系统,其中一定量的神经毒素为约1个单位至约50,000个单位的一种肉毒杆菌毒素。
13.权利要求1的受控释放系统,其中一定量的神经毒素为约10个单位至约2,000个单位的A型肉毒杆菌毒素。
14.权利要求1的受控释放系统,其中一定量的神经毒素为约100个单位至约30,000个单位的B型肉毒杆菌毒素。
15.权利要求1的受控释放系统,其中该神经毒素为一种肉毒杆菌毒素,该毒素以一定量进行释放,该释放量在植入系统附近有效地导致一块肌肉或者肌肉群的松弛性肌肉麻痹。
16.一种受控释放系统,该系统含有:
a)一种聚合物基质,以及;
b)位于该聚合物基质中的约10个单位至约20,000个单位的肉毒杆菌毒素,其中分部量的该肉毒杆菌毒素可在约2个月至约5年的较长时间范围内从该聚合物基质中释放,同时不产生明显的免疫系统反应。
17.一种用于制造受控释放系统的方法,该方法包括以下步骤:
(a)将一种聚合物物溶于一种溶剂以形成一种聚合物溶液;
(b)将一种神经毒素混合或分散于该聚合物溶液中以形成一种聚合物-神经毒素混合物,以及;
(c)使该聚合物-神经毒素混合物凝固或者固化,由此制成了一种受控释放系统。
18.权利要求17的方法,该方法在混合步骤之后进一步包括了蒸发溶剂的步骤。
19.一种用于使用一种连续释放系统的方法,该方法包括对一种受控释放系统进行注射或者植入,以此对某种运动失调或者由胆碱能神经支配所影响的失调进行治疗,该受控释放系统含有一种聚合物基质和一种神经毒素。
20.一种用于制造与金属阳离子复合的神经毒素的方法,该方法以下包括步骤:
(a)形成一种含有神经毒素的溶液;
(b)将一种多价金属阳离子成分同该神经毒素溶液进行混合从而将该多价金属阳离子同该神经毒素复合,由此而形成了一种与金属阳离子复合的神经毒素悬液,以及;
(c)干燥所述的悬液以形成所述与金属阳离子复合的神经毒素。
