Abstract

Description
发明的技术领域
本发明涉及人工晶状体(IOL)及其制造和使用方法。具体地讲,本发明涉及可调节性人工晶状体,其用于在病患天然晶状体在例如白内障手术中被手术摘除后实施屈光校正。
发明的背景技术

取得商业成功的IOL由各种生物相容性材料制成,材料的范围是从较坚硬的材料例如聚甲基丙烯酸甲酯(PMMA)至能够被折叠或压缩的较软、柔性更大的材料如硅酮、某些丙烯酸树脂和水凝胶。一些IOL的触感部分与光学部分分开成型,再通过诸如加热、物理桩接和/或化学粘结等处理过程而连接到光学部分上。还有一些触感部分与光学部分成型为一个整体件,通常称作“单件式”IOL。
较软、柔性较大的IOL具有可被压缩、折叠、卷绕或其他变形的能力,因此在最近几年非常流行。这种较软IOL可以在通过眼睛角膜中的切口嵌入之前变形。在将IOL嵌入眼中后,IOL会因软质材料具有记忆特性而返回其初始预变形形状。刚刚描述过的较软、柔性较大的IOL可以通过一个2.8至3.2mm的切口植入眼中,该切口比较坚硬的IOL所需的4.8至6.0mm的切口小得多。较坚硬的IOL需要较大的切口,因为晶状体必须通过角膜中的比不可弯曲的IOL光学部分的直径略大的切口嵌入。由于现已发现较大切口伴随着诸如感应散光等术后并发症的发病率增加,因此较坚硬的IOL在市场中比较不受欢迎。
在IOL移植后,较软和较坚硬的IOL均会受到施加到其外周边缘上的压缩力,这些压缩力来自睫状肌的天然脑感应收缩和放松以及玻璃体压力的增大和减小。这种类型的压缩力在有晶状体眼中是有用的,即可以使眼睛聚焦在不同的距离上。商业上最成功的用在无晶状体眼中的IOL设计结构具有单焦点光学部分,即光学部分是固定的并且只能使眼睛聚焦在一定的固定距离上。这种单焦点IOL需要配戴眼镜以改变眼睛的焦距。少数双焦点IOL已经被引入商业市场中,但它们的缺点是每个双焦点图像只能提供大约百分之四十的可用光线,而剩下的百分之二十的光线会因散射而损失掉,因此视敏度会降低。
由于目前IOL设计结构存在上述缺点,因此需要有一种可调节性人工晶状体,其被设计得能够在无晶状体眼中提供多焦点视觉成像功能,而不必借助于眼镜。
本发明概述

本发明详细描述

通过设置IOL 32的上述尺寸,环形和非环形触感元件40从接触板38直至连接部分44和光学部分34在平面48-48中相对较厚,而且柔性中部62在平面46-46中的厚度尺寸优选小于在平面48-48中的宽度。在压缩力施加到接触板38上时,具有所述结构的环形触感元件40能够防止偏移到紧密邻近于外周边缘36处,从而以使沿着光轴OA-OA的轴向位移最大化。当可调节性IOL 32用作屈光晶状体时,可以提供出稳定、可靠的多焦点屈光校正。
通过在一个或多个触感元件40的宽阔且非常薄的带状形状中加入一个加固元件60,如图8所示,IOL 32的触感元件40理想柔性特性也同样可以获得或加强。加固元件60可以安置在触感元件40中,以使宽阔或加宽平坦表面62定向在一个平行于平面48-48的平面内,从而在一个平行于平面46-46的平面内轴向减薄。加固元件60的结构功能类似于工字梁,以便在压缩力施加到接触板38上时,使沿着光轴OA-OA的轴向位移最大化。
加固件60由柔性低于IOL 32的材料制成。加固元件60的适宜材料包括但不局限于聚酰亚胺、聚烯烃、高密度聚乙烯、聚酯、尼龙、金属或任何具有适宜加固特性的生物相容性材料。加固元件60可以由一层或多层网织品、筛网、带材和/或片材制成,以提供出这里描述的理想柔性特征。在希望采用更薄触感元件并同时获得理想稳定性和柔性特征的情况下,加固元件60可以与上述触感元件40结合使用。
制作IOL 32的适宜材料包包括但不局限于可折叠或可压缩材料,例如硅酮高聚物、碳氢化合物与碳氟化合物的聚合物、水凝胶、软质丙烯酸酯聚合物、聚酯、聚酰胺、聚氨酯、带亲水性单体单元的硅酮高聚物、含氟的聚硅氧烷弹性体以及它们的组合物。优选制作本发明的OIL 32的材料是由2-甲基丙烯酸羟乙酯(HEMA)和6-甲基丙烯酸羟己酯(HOHEXMA)制成的水凝胶,即HEMA与HOHEXMA的共聚物。HEMA与HOHEXMA的共聚物是优选制作IOL 32的材料,因为它的平衡水含量为重量的大约18%,而且具有大约1.474的高折射率,这高于眼睛的水样液的折射率,即1.33。高折射率是制造IOL时的理想特征,这样可以利用最小的光学厚度获得高光学能力。通过使用具有高折射率的材料,可以借助于较薄的IOL校正视敏度的不足。HEMA与HOHEXMA的共聚物还因为其机械强度而是优选制作IOL 32的材料,即它适于承受相当大的物理操作。HEMA与HOHEXMA的共聚物还具有适于IOL应用的理想记忆性能。由具有良好记忆性能的材料如HEMA与HOHEXMA的共聚物材料制成的IOL可以在眼睛中以更加受控的方式而非突发的方式展开到预定形状。由具有良好记忆性能的材料制成的带有触感元件40的所述IOL 32的独特结构还能够在触感元件插入眼睛10中时提高触感元件的控制性。IOL的突发式展开是不希望的,因为这可能会损坏眼睛内的脆弱组织。HEMA与HOHEXMA的共聚物还具有在眼睛中的稳定性,这是所希望的。
尽管本发明的原理优选应用在由可折叠或可压缩材料制成的软质或可折叠IOL中,但也可以用于由相对刚性的材料如聚甲基丙烯酸甲酯(PMMA)制成的较坚硬且柔性较小的晶状体中,晶状体上带有由相同或不同材料制成的柔性触感元件。
IOL 32的光学元件34可以是0至大约+40屈光度的正放大率透镜,或者是0至大约-30屈光度的负放大率透镜。光学部分34可以是双凸式、平凸式、平凹式、双凹式或凹凸式(弯月式)透镜,这取决于为达到高效操作时的适宜中央和周边厚度所需的放大率。
IOL 32的光学元件34可以选择性地毗邻外周边缘36形成有一个眩光降低区56,它的宽度为大约0.25至2.00mm,优选为大约0.3至0.6mm,最优选为0.5mm,用于在高强度光线进入眼睛10中或在瞳孔58放大而导致光线照射到IOL 32的外周边缘36时降低眩光。眩光降低区56通常由与光学部分34相同的材料制成,但也可以是不透明的、彩色的或者以传统方式形成图案,以阻塞或扩散光轴OA-OA所在平面上的光线。
所述IOL 32可以模塑成型出来,或者优选这样制造,即首先利用选定材料制作圆盘,如美国专利No.5,217,491和5,326,506中所述,上述专利整体结合在此作为参考。在圆盘制出后,IOL 32通过传统方法由材料圆盘机加工出来。在机加工或模塑成型后,IOL 32就可以通过本领域的普通技术人员所公知的传统方法抛光、清洗、消毒和包装。
所述IOL 32是这样移植到眼睛10中的,即在角膜12和晶状体囊24上产生一个切口,摘除天然晶状体16,将IOL 32嵌入晶状体囊24中,再封闭切口。
本发明的IOL 32提供了适于用在眼睛10的晶状体囊24中的可调节性晶状体。IOL 32具有带柔性特性的触感元件40,以使沿着眼睛10的光轴OA-OA的轴向位移最大化,从而不借助于眼镜就能够实现多焦点成像。
虽然这里显示和描述了本发明的一些特定实施例,但对于本领域的普通技术人员而言,显然在不超出基本发明思想的范围和精神的前提下,可以做出各种修改,而且本发明的范围和精神并不局限于这里显示和描述的特定形式,它们只由附属 确定。
Claims (18)
1.一种可调节性人工晶状体,其用于大致垂直于眼睛光轴植入眼睛中,包括:
一个确定出光学部分的外周边缘以及两个、三个或四个永久性连接着外周边缘的触感元件,
由此,一个足以使上述晶状体在直径上压缩1.0mm的压缩力将导致上述光学部分沿着眼睛光轴轴向移动大约1.0mm以上,从而使得眼睛实现多焦点视觉成像。
2.一种可调节性人工晶状体,其用于大致垂直于眼睛光轴植入眼睛中,包括:
一个确定出光学部分的外周边缘以及两个、三个或四个永久性连接着外周边缘的触感元件,

5.如权利要求1、2或3所述的人工晶状体,其特征在于,上述晶状体由选自下面一组中的材料制成:硅酮高聚物、碳氢化合物与碳氟化合物的聚合物、水凝胶、软质丙烯酸酯聚合物、聚酯、聚酰胺、聚氨酯、带亲水性单体单元的硅酮高聚物、含氟的聚硅氧烷弹性体以及它们的组合物。
6.如权利要求1、2或3所述的人工晶状体,其特征在于,上述晶状体由一种水凝胶材料制成。
7.如权利要求1、2或3所述的人工晶状体,其特征在于,上述晶状体由一种水凝胶材料制成,该材料含有18%重量的水。
8.如权利要求1、2或3所述的人工晶状体,其特征在于,上述晶状体由HEMA与HOHEXMA的共聚物制成。
9.如权利要求1、2或3所述的人工晶状体,其特征在于,上述晶状体由折射率高于1.33的材料制成。
10.如权利要求1、2或3所述的人工晶状体,其特征在于,上述晶状体由一种丙烯酸树脂材料制成。
11.如权利要求1、2或3所述的人工晶状体,其特征在于,上述晶状体由一种硅酮材料制成。
12.如权利要求1、2或3所述的人工晶状体,其特征在于,上述触感元件被成形得在大致垂直于眼睛光轴的平面上的尺寸大于或等于在大致平行于眼睛光轴的平面上的尺寸。
13.如权利要求1、2或3所述的人工晶状体,其特征在于,一个眩光降低区毗邻光学部分的外周边缘形成。
14.如权利要求1、2或3所述的人工晶状体,其特征在于,一个或多个上述触感元件中包含一个加固元件,该加固元件在大致平行于眼睛光轴的平面上的弯曲阻力低于在大致垂直于眼睛光轴的平面上的弯曲阻力。
15.如权利要求1、2或3所述的人工晶状体,其特征在于,触感元件中中包含一个加固元件,该加固元件由选自下面一组中的材料制成:聚酰亚胺、聚烯烃、高密度聚酯、尼龙和金属。
16.一种制造如权利要求1、2或3所述的人工晶状体的方法,包括:
成型出由适宜材料构成的圆盘,以及
由上述圆盘机加工出上述晶状体。
17.一种制造如权利要求1、2或3所述的人工晶状体的方法,包括:
模塑成型出由适宜材料构成的上述晶状体。
18.一种使用如权利要求1、2或3所述的人工晶状体的方法,包括:
在眼睛的角膜和晶状体囊上产生一个切口,
摘除上述眼睛的天然晶状体,以及
将上述人工晶状体嵌入上述眼睛的上述晶状体囊中。
