然而,如果肿瘤细胞已经从肿瘤的原发部位转移,则某些肿瘤细胞极有可能污染收集得到的造血细胞群。在这种情形中,收集得到的造血干细胞包括了污染的肿瘤细胞。因此,重要的是要找到一种机制,在把干细胞重新导入患者体内之前杀死所有已转移的肿瘤细胞。如果把任何有活性的致肿瘤细胞重新导入患者体内,则它们会导致复发。
在传统的骨髓收集程序中已经报道了从造血细胞中除去肿瘤细胞的问题(Campana,D.等,急性白血病中最少量残余疾病的检测:方法论进展和临床重要性,Blood,1995年3月15日,85(6):1416-34)。当其它人试图用更新的方法将取出的外周血细胞的白细胞输回患者(leukopheresis)同时清除肿瘤细胞时,也发现了类似的问题(Brugger,W.等,肿瘤细胞和造血祖先细胞转移到有实体肿瘤的患者的外周血液中,Blood,83(3):636-40,1994)。
在这些程序的每一个中,污染性肿瘤细胞的数量范围为每4百万个收集的单核细胞有大约10-5000个肿瘤细胞,这取决于用于治疗转移的化疗药物方案。对全部造血细胞收集物进行不连续密度梯度离心,获得单核细胞。从患者获得的单核细胞总数通常大约有100亿个细胞。因此,收集物中的肿瘤细胞总负荷从大约25,000个细胞的下限到大约12,000,000个细胞的上限。
通过遗传标记已经显示,这些污染的肿瘤细胞会导致肿瘤复发(Rill,ER等,关于自身骨髓移植治疗实体肿瘤会返回大量致肿瘤细胞的直接证明,Blood,84(2):380-383,1994)。非常需要有效的方法来除去造血细胞移植物中的所有肿瘤细胞(Gulati,SC等,净化自身于细胞移植物的原理,Joumal of Hematotherapy,2(4):467-71,1993)。一种迅速可靠地除去所有污染肿瘤细胞的方法会改善人数不断增多患者的造血干细胞移植的效率。
其它人已经尝试从造血干细胞收集物中除去污染性肿瘤细胞,但是仅获得有限的成功。现已提出从收获的干细胞中除去肿瘤细胞群的几种方法并进行了测试(A.Gee,编辑 骨髓加工和净化,第5部分,CRC Press,Boca Raton,Florida,1991)。因此,构成所有这些净化方法的基本思路是分离或破坏恶性肿瘤细胞,同时保留移植患者中造血重建所需的造血干细胞。
一些公司和医生已经尝试用免疫亲和珠为基础的选择来从非肿瘤细胞群中消除恶性肿瘤细胞。在该过程中,使整个细胞群与免疫亲和珠接触。例如,为了从造血细胞中分离出肿瘤细胞,第一步(阳性)CD34选择将肿瘤细胞与造血细胞分离。对造血细胞有特异性的抗CD34抗体结合于免疫亲和珠的结合使得医生能从非造血细胞群中特异性地取出这些细胞。在一些例子中,也可通过肿瘤特异性抗体与玻珠的偶联来对肿瘤或上皮细胞标记进行免疫亲和珠为基础的阴性选择。
试图从非肿瘤细胞群中除去肿瘤细胞的另一种方法是,使一种毒性剂与只对肿瘤细胞有特异性的抗体免疫偶联。在该系统中,使抗体与化学毒性剂、毒素或放射性核素结合,然后再与收获的细胞群接触。不幸的是,这种处理并不能杀死所有的肿瘤。
从非肿瘤细胞群中分离肿瘤细胞的其它系统采用了造血细胞的非特异性结合特点作为分离的基础。例如,Dooley等利用其粘附特性用厚层过滤(deep bedfiltration)来分离造血细胞(Dooley DC等,一种从移动的外周血于细胞产物中清除乳腺癌细胞的廉价新技术,Blood,88(10)suppl 1:252a,1916)。然而,发现一些肿瘤细胞与造血细胞一起被分离出,这给患者可能的复发提供了机会。
另外,曾使用细胞毒性剂(例如4-羟基-过氧-环磷酰胺(4HC))来选择性杀死肿瘤细胞而不破坏造血干细胞。不幸的是,该系统也会导致造血细胞收获降低,因为细胞毒性剂削弱或破坏了一些非肿瘤细胞。
在其它一些方法中,用光敏剂(如步花菁)与细胞群混合,然后对细胞群进行光辐射,以特异性杀死肿瘤细胞(Lydaki等,步花菁540介导的白血病细胞光辐射)Journal of Photochemistry and Photobiology 32(1-2):27-32,1996)。另外,Gazitt等人采用荧光激活细胞分拣法(FACS)来从肿瘤细胞中分拣出造血干细胞(Gazitt等人,纯化的CD34+Lin-Thy+干细胞不含克隆的骨髓瘤细胞Blood,86(1):381-389,1995)。如众所周知的,流式细胞计每次可分拣出一类细胞,并用物理方法将一组标记的细胞与另一组细胞分开。然而,已经表明个别神经元在加入用于流式细胞计量术的吸附染料后会被杀死(Miller,JP和Selverston A1.,单神经元被注入细胞间的染料的辐射迅速杀死Science,206:702-704,1979)。因此,用FACS分离细胞群是不利的,因为细胞产量会非常低。
在另一个方法中,Clarke等人公开了用腺病毒介导的自杀基因转移来选择性地杀死肿瘤细胞(Clarke等人,重组的bcl-xs腺病毒选择性地诱导癌细胞凋亡但不诱导正常骨髓细胞凋亡Proc.Nat.Acad.Sci.92(24):11024-8,1995)。
然而,上述大多数方法是根据对整群肿瘤细胞实行分离或杀死的方法。不幸的是,上述的整群肿瘤净化方法不能将收获的干细胞群中所有污染的肿瘤细胞杀死或清除。在最佳的情况下,最初收获物中每100,000细胞的残余肿瘤细胞负荷会保留1至10个肿瘤细胞(Lazarus等人,体外骨髓净化会改善自身骨髓移植后的结果吗?Joumal of Hematotherapy,2(4):457-66,1993)。
因此,即使采用最佳的可行技术,在自身干细胞移植中重新导入患者体内的残余肿瘤细胞数也大约有10-2000个细胞。如果肿瘤细胞成指数型迅速生长,那么移植中的这些残余的肿瘤细胞会很快导致患者复发肿瘤。
美国专利No.4,395,397(授予Shapiro)中描述了采用激光技术的另一种方法。在Shapiro的方法中,将标记的细胞置于有荧光检测器的流式细胞仪中以鉴定标记的肿瘤细胞。当标记的细胞通过检测器时用激光束杀死该标记细胞。该方法有许多缺点。首先,一旦不想要的细胞通过检测器/激光区,就没有办法检查该细胞的破坏是否已经完全成功。如果肿瘤细胞躲过了破坏,则它必定会被重新导入患者。第二,激光束的焦点直径必须大于穿过切面的液体流,这样,在不想要的细胞区域中的许多细胞(包括健康的细胞)就会被激光束破坏。
美国专利No.5,035,693(授予Kratzer)描述了采用激光技术的另一种方法。在该方法中,将标记的细胞群置于移动带上。然后用检测器鉴定标记的细胞并用激光加以破坏。然而,该系统具有与Shapiro方法相同的许多缺点。例如由于细胞在带上以一个方向移动通过检测器,因此该方法不可逆。因此,如果有一个肿瘤细胞逃脱检测,它就会被重新导入患者体内。
因此,尽管有许多深入的研究努力和许多革新方法,但仍非常需要从收获的细胞群中有效根除几乎每一个肿瘤细胞的方法和系统。本文所述的系统和方法满足了这一需求。
发明概述
本发明提供一种单独识别和消灭细胞群中污染的肿瘤细胞的靶向系统和方法。使用本发明的系统,就能有效识别每个肿瘤细胞并予以各个摧毁。于是,就能进行自身造血细胞移植和类似的医疗技术,而不会重新引入任何污染性肿瘤细胞。
可用本发明所揭示的数种方法来鉴别肿瘤细胞。一个实例包括采用一种非破坏性标记方法,从而在显微镜下能把所有活的肿瘤细胞与非肿瘤细胞区别开来。在此实例中,可用对肿瘤有特异性的、偶联荧光染料的抗体来特异性标记每个肿瘤细胞,而不标记任何造血细胞。然后用显微镜鉴别非肿瘤细胞群中的被标记的肿瘤细胞。此后把窄谱高功率激光束聚焦在每个被识别的肿瘤细胞上,并且输送短的致死性光脉冲。然后鉴别并杀死下一个肿瘤细胞,依此类推,直至每个被标记的肿瘤细胞被摧毁。
在另一个实例中,用选择性结合造血细胞但不结合肿瘤细胞的抗体来鉴别造血干细胞。识别所有的肿瘤细胞(例如:没有吸收标记的细胞),然后用窄的高功率激光束将它们杀死。
再一个实例是一种从包含非肿瘤细胞的细胞群中清除肿瘤细胞的方法,该方法包括下述步骤:a)对细胞群进行标记,从而能将肿瘤细胞与非肿瘤细胞区别开来;b)参照标记确定一个肿瘤细胞的位置;以及c)通过将来自受控能源的脉冲施加至已定位的肿瘤细胞上将该肿瘤细胞杀死。
又一个实例是一种在造血细胞群中富集干细胞数目的方法,该方法包括:a)用干细胞特异性的标记来标记造血细胞群;以及b)把高能激光光脉冲施加至细胞群中至少一个未被标记的细胞上。
还有一个实例是一种制备用于重新导入患者体内的分离的造血细胞的方法,该方法包括:a)用肿瘤细胞特异性标记来对分离的造血细胞中的肿瘤细胞作标记;以及b)把高能激光光脉冲施加至细胞群中至少一个被标记的细胞上。
本发明的另一个实例是一种确保将第一群细胞从第二群细胞中清除的体外方法。该方法包括:标记第一群细胞,从而能够把第一群细胞与第二群细胞区别开来;将第一群细胞和第二群细胞置于表面上的静止不动位置;通过参照标记确定第一群细胞中一个细胞的位置;向这个细胞施加受控能源的第一脉冲;确定该细胞是否被第一脉冲杀死;以及如果该细胞未被杀死,则施加受控能源的第二脉冲。
本发明的还有一个实例是一种从包括非肿瘤细胞的细胞群中清除肿瘤细胞的方法。该方法包括下述步骤:标记非肿瘤细胞,以使非肿瘤细胞可以与肿瘤细胞区别开来;鉴别细胞群中未标记的细胞而确定每一个肿瘤细胞的位置;以及当肿瘤细胞在表面上处于基本静止不动的位置时,施加受控能源的脉冲至已被定位的肿瘤细胞将该细胞杀死。

详细描述
本发明提供了一种靶向方法用于从非肿瘤细胞群中去除污染性肿瘤细胞。如上所述,这种方法涉及作为治疗方案一部分的医疗程序,此程序是取出细胞群然后重新导入患者体内。通常,靶向方法首先使用一种标记作为标志,以便用显微镜对污染的肿瘤细胞进行鉴别和定位。
所选细胞标记可以是识别和鉴别非肿瘤细胞群中污染的肿瘤细胞的任何标记。例如与荧光染料结合的抗肿瘤抗体可用作特异性标记(参见A.Gee编辑,骨髓处理和净化第5部分,第10章,CRC Press,Boca Raton,Florida,1991)。对于各种各样的致肿瘤细胞已经发现了多种对肿瘤有特异性的标记。例如,在收集到的造血细胞群中污染性乳腺癌细胞上已经发现了对于上皮细胞有特异性的许多表面标记。这些抗体可用来识别造血细胞群中的乳腺癌细胞,并将致死性能量脉冲输送引向乳腺癌细胞。与此类似,其它肿瘤脱瘾性的交联荧光色素的抗体可用于识别其它类型的污染的肿瘤细胞。
还应注意,如果对于肿瘤细胞群不能获得特异性标记,则该方法可以“相反”的方式来进行。可用非肿瘤细胞群的特异性标记来识别那些非致肿瘤细胞。例如,在造血细胞群中,可用CD34细胞标记来单单染色造血细胞,但不对不造血的肿瘤细胞染色。然后通过递送聚焦的能量脉冲将没有标记的细胞杀死。其它残存的活细胞群将只含有具有细胞标记(如CD34)的细胞。
在识别肿瘤细胞后,用一受控能源(例如激光、非激光的平行或聚焦光、RF能量、加速颗粒、聚焦的超声能量、电子束或其它辐射束)来输送靶向致死性能量脉冲,以逐一杀死每个肿瘤细胞。由于非肿瘤细胞群中的肿瘤细胞可被特异性地、独特地识别,因此该方法可用来完全只除去污染的肿瘤细胞。
在靶向方法的一个实例中,通过同时荧光和激光照射来杀死肿瘤细胞。在第一步中,用对肿瘤细胞有特异性的荧光染料对细胞群进行染色。例如,在该实例中可采用对肿瘤细胞有特异性的抗体,并将抗体与荧光分子偶联。然后用合适的光(例如紫外光)照射细胞群,从而可以识别出非肿瘤细胞群中的产生荧光的肿瘤细胞。
然后将标记的细胞置于Petri碟中并置于系统内。如果系统以自动模式运转,则荧光灯会照射,CCD相机开始拍取碟中细胞的视频图象。相连的计算机上运行的计算机程序开始分析图象并识别每一个有荧光的细胞。激光束靶向每个细胞并将其破坏。计算机通过记录激光激活前后的荧光变化来确认每个细胞已被杀死。
另一个实例是从癌症患者的活体检查标本中去除污染的细胞。由于标本中存在的污染性原代细胞体外生长特性优于肿瘤细胞,使得从多种肿瘤体外建立原代人肿瘤细胞系的过程变得复杂。例如,在建立许多癌细胞系时,污染的成纤维细胞是主要的挑战。本文公开的系统可用来特定地标记和破坏这些污染细胞,同时保留活检标本肿瘤细胞的完整。因此,更具侵袭性的原代细胞不会超过并破坏癌细胞系。
在一种相关的应用中,本系统可用来除去用于组织基因工程改造和细胞治疗的接种物中的污染性细胞。在建立用于组织基因工程改造和进行细胞治疗的原代细胞培养物时,存在细胞污染问题。特别是,用于软骨缺陷的软骨细胞治疗受到软骨活检获得的细胞群中的杂质的妨碍。因此,本发明的系统可用来特异性地除去接种物中这些类型的细胞。例如,可获取软骨活检物并使样品在常规条件下生长。然后用针对污染细胞的特异性标记对生长的培养物进行染色。然后将全部细胞群置于下述系统中,并破坏有标记的污染性细胞。
在另一个实例中,可以战胜异体人干细胞(HSC)移植中移植物抗宿主病(GVHD)的显著危害和严重性。利用本文所述的系统就可以确定和控制患者的T细胞含量。而已有技术要进行此类控制则特别困难,因为T细胞的过度缺少导致丧失在异体移植中所见的有益的移植物抗白血病效应。因此,为了攻击(例如)白血病细胞,一定水平的T细胞对于患者是有益处的。然而,T细胞水平过高却会对受者组织的全身免疫系统进行攻击。本系统和方法能精确地控制任何异体移植中T细胞的数量。
图1中描述了自动化靶向系统的一个实例。计算机系统12通过电缆14与相机16连接。计算机系统可以是任何商业上可获得的、能接收和分析视频数据的计算机。这种计算机系统的一个例子是运行Microsoft Windows95软件的IntelPentium Pro计算机。
尽管图1公开的实例中描述了一种电视相机,但是相机16可以是本领域技术人员已知的任何类型的图象收集设备。相机16可安装在支架20上,它与地面通常呈垂直方向。
相机16的镜头22被设计成通过目镜26与显微镜24匹配。因此,传送到显微镜24中的任何图象通过目镜26被传递到相机16的镜头22。
相机16通过电缆14将其捕获的图象传输给计算机12。通过这种方式,计算机12捕获显微镜显示的任何图象并进行分析。例如显微镜24可以聚焦在碟30中的细胞上。细胞图象将从显微镜24传递至相机16,最终被送至计算机12。然后,计算机12可以运行软件来分析捕获的图象。
计算机12还通过第二根电缆32与激光器34连接。激光器安装在电子控制的转体36上,这样通过接收来自计算机12的信号,其相对于支架20的位置可以改变。如图所示,在靶细胞基本静止不动时,可调节激光器34的光束通过显微镜24镜片来瞄准细胞。另外,可旋转激光器34以便将光束直接发射至碟30。
在本发明的内容中,术语“碟”有广泛的含义。它包括能保留薄层液体(如细胞培养液)的任何表面。碟可放入冷却装置中或与冷却装置相连,以保持碟中液体在进行本文所述方法时低于室温。可能还希望(但不是必需的)用惰性气氛(如氮或氩)来遮盖液体表面。
尽管图1所述实例包括一种电子控制转体36来瞄准激光,但是也期望用其它方法。例如,在激光器及其目标之间放置电子控制的镜子,从而利用镜子的转动来瞄准激光器。瞄准激光的其它机制(例如本领域技术人员已知的分束器、镜或棱镜)均包括在本发明的范围内。
宽带光源40也通过接口42与显微镜24连接。接口42使得宽带光源发射出的光照射置于碟30上的细胞。宽带光源是本领域技术人员熟知的,在自动寻靶系统10中可以安置许多不同的类型以照射置于碟30上的标记细胞。
碟30下方支承碟的是受计算机控制的可移动溜槽48。计算机12可沿通信电缆49传递信号,从而改变溜槽48相对于激光器34和显微镜24的位置。因此,为了特异性靶向细胞,计算机12可经电缆49传递信号给溜槽48,使碟30滚动到特定位置。通过计算正确的位置,激光器可以通过将碟30移动到所选位置来瞄准特定的细胞。所选位置的坐标可根据计算机12从分析相机16收集的细胞图象来确定。
计算机12上运行的软件捕获通过电缆14自相机16传递来的图象。如果碟30中放置了一组标记的细胞,则相机16会记录下标记细胞的图象。如上所述,可用宽带光源40照射细胞。通过在计算机收集的图象上重叠一层两维网格,软件就可通过寻找直径为一个细胞宽度的黑点来确定标记细胞的X/Y坐标。然后,软件计算标记细胞的笛卡尔坐标并使激光器34特别瞄准那些坐标。通过发射短的高能脉冲,激光器能选择性地杀死每个标记的细胞并且不破坏任何没有标记的细胞。
计算机系统12还可装有检测来自染色肿瘤细胞的荧光信号的传感器,然后计算要摧毁的第一肿瘤细胞的坐标位置。然后,计算机系统将聚焦能源(如激光或电子束)瞄准第一肿瘤细胞的计算位置。因此,非肿瘤细胞群中被靶向的肿瘤细胞被特异性地消灭。这是由于靶细胞在此过程中基本上静止不动而实现的。
在另一个典型系统中,细胞在受计算机控制的X-Y桌面(table)上、在CCD数码相机连接的显微镜下移动。激光器安装在显微镜上,这样它瞄准视野中的一个具体的固定位置。然后,计算机沿X方向连续扫描第一列细胞直至显示出一个染色的肿瘤细胞。
然后,计算机移动X-Y桌面直至肿瘤细胞进入激光器的目标区域内;使激光器产生脉冲破坏细胞;然后使X-Y桌面返回到其原先的位置,再沿X方向移动直至扫描完第一列细胞并破坏了该列中的所有肿瘤细胞。
然后沿Y方向移动X-Y桌面直至显示出第二列,该列有少数细胞与第一列重叠。然后沿X方向移动桌面,如上所述那样消灭肿瘤细胞,重复该程序直至扫描完整个细胞群。较佳的,计算机软件可自动识别并靶向细胞。然而,也可考虑用由操作者控制的指点装置(例如轨迹球、操纵杆或鼠标)来定位和标记显示屏上的肿瘤细胞和/或控制X-Y桌面的移动。当然,计算机控制系统可有多种变化形式,包括扫描细胞和相对于显微镜移动激光器的其它方法(例如,仅仅在Y方向或区域中的任何处移动)。还可设想,碟/桌面可保持基本不动,而显微镜/激光器组件可移动以便靶向标记的细胞。
例如,可以移动显微镜头和激光寻靶系统,而使表面上盛放着细胞的瓶子或Petri碟静止不动。另外,激光器系统可自细胞群或细胞群上发射。由于激光寻靶系统通过显微镜来聚焦,因此激光束可指向不同的焦平面。因此,通过将激光束靶向不同焦平面就可将位于不同垂直高度的细胞特异性地杀死。例如,一旦计算机通过检测器识别出在特定X和Y坐标处的一个细胞,就可将激光发射到那些坐标处的几个焦平面。这就保证了无论该细胞是否在垂直平面内,它都会被激光杀死。
对细胞群中的每个肿瘤细胞重复该靶向程序,就可自动破坏全部的肿瘤细胞群。标记、识别和杀死每个肿瘤细胞的程序也可手工进行,操作者通过显微镜识别每个标记的肿瘤细胞,然后将激光束聚焦到受照射的肿瘤细胞上。允许手工靶向细胞的组合激光器/显微镜系统可从Photonic Instruments(Arlington Heights,IL)购得。
例如,可用能观察连续毗邻区域的Nikon7 Diaphot 300显微镜在适当时间内观察整个细胞群。典型细胞群在铺满时有1.5×108个细胞,将占据300cm2的面积。假定在4X放大下一个视野为2×1.5mm,则显微镜总共需要扫描100×100个矩阵即10,000个视野。手工操作每小时可能扫描500-1000个区域,因此手工程序将花费大约10-20小时。自动化程序将会快得多。
一旦用识别剂(如荧光标记抗体)处理全部细胞群然后用能量脉冲杀死后,就可根据患者个体临床需要洗涤细胞群。然后,经处理的细胞群准备移植到所选患者体内。如上所述,患者通常就是细胞群原来提供者同一人。
寻靶方法的较佳的聚焦能源是发射光波长在375-900纳米范围内的激光。一种这样的激光器由Photonic Instruments(Arlington Heights,IL)生产。如众所周知的,激光器提供了具有高度特定方向性的单色光源,使得其能以直径仅为0.5微米的显微镜聚焦的能量束来瞄准。
用预定波长可以实现传递光能的高精度(在空间和时间上的分辨率)。杀死特定肿瘤细胞的最佳波长可通过实验来确定,并采用最佳的结果。另外,可用约为10-100纳秒的脉冲长度来特异性杀死细胞群中单独的一个细胞。
另一种方法是,可用自动光圈将宽带光源(例如从Xenon Lamp购得)较窄地聚焦,以提供受控的能源。大功率宽带光源可聚焦于小到一平方微米的区域上,因此可选择性地用来杀死非肿瘤细胞群中的单个肿瘤细胞。
光源功率、光脉冲的时间可以调节,以达到所需的特异性杀死特定肿瘤细胞的效果。激光脉冲长度可短达2至6纳秒,光能可以高达50微焦或更高的能量发射。然而,宜选择提供给指定靶细胞的光能总量,使其不会引起靶细胞物质或周围的基质显著沸腾。
将细胞培养基局部加热至沸点会使视野变得模糊,从而搅乱了自动寻靶系统。另外,位于靶向细胞附近的非肿瘤细胞也有可能被破坏。在一个实例中,传递的总光功率会最小程度地破坏细胞膜从而通过细胞质组分损失而最终导致细胞死亡的影响程度最小。或者,液可以选择总光功率使其不可逆地破坏细胞组分,从而导致细胞死亡而不破坏细胞膜。
为了用显微镜观察肿瘤细胞,最好将总细胞群置于名义上的平面上,这样使大量细胞出现在单个焦平面中。理论上,该表面上的细胞密度可以是任何值。然而,细胞密度应尽可能得高,以最大程度地减少程序所需的总表面积。如果可明显识别污染性细胞,则总细胞群可铺满置于表面上(每平方厘米约500,000个细胞)。
如果收获的细胞群是作为部分骨髓移植物取出的造血细胞,则只对所需干细胞群进行富集使得能在更小的总面积中观察细胞群。例如,为了观察35亿个造血细胞(70公斤患者每公斤体重有5百万)的CD34富集细胞群,需要大约700平方厘米的总表面积。然而,如果造血干细胞富集程序包括筛选具有多种干细胞特异性标记的程序,则靶细胞的总数会减少,因而靶向程序所需的总面积更少。如大家所知晓的,可以根据表达的干细胞标记Thy-1,CD38,CD15,CD11b,Gly-A,CD3或CD19来富集造血细胞群以获得干细胞。另外,也可用熟知的厚层过滤或逆流淘析技术来富集细胞。
如果在细胞铺满时肿瘤细胞的检测不受妨碍,则可能理想的是将靶细胞群维持在亚铺满(subconfluency)状况下,从而最大程度地减少对邻近细胞的附带损伤。然而,由于细胞总数与肿瘤细胞数相比较大,因此对于周围细胞的一些附带破坏可以忍受。用共焦点显微镜检测和定位三维空间内的标记的肿瘤细胞就不需要平的表面,识别并会聚致死性光源于放置细胞的体积内。
在肿瘤细胞被特异性破坏后,可低温洗涤细胞除去细胞碎片。根据程序预计的时间长短,可将靶细胞群冷却至4℃,这样进行除去污染物程序时非肿瘤细胞群生理性能只收到最少程度的降低。当非肿瘤细胞是造血细胞时,这是特别重要的,因为造血细胞会在室温下退化。在该过程中可用电冷却装置来冷却细胞群。
如众所周知的,将细胞群冷却到4℃即消除了所有细胞的移动。放置细胞的表面还可用聚阳离子化合物(如poly-I-赖氨酸)涂覆,这样就可牢固地粘住细胞。
下列实施例描述了从造血细胞群中除去肿瘤细胞的靶向方法的一个实例。
实施例1
造血干细胞移植
由医生确定一位患有转移性肿瘤并需要自身骨髓移植的患者。处理的第一步是,对患者进行骨髓收集程序。在该程序中,在手术室内使患者处于全身麻醉状态。然后由外科医生在患者骼骨后嵴多次穿刺并抽出骨髓。
收集程序获得大约1×109个造血细胞。首先上免疫亲和柱,选择具有CD34造血细胞特异性表面抗原的细胞,从收获的细胞中富集造血细胞。为了进一步富集收获细胞群中的干细胞,进行第二次选择,将收获细胞加入结合有抗干细胞抗体(Thy-1)的免疫亲和柱走柱。
从柱上常规洗脱后,使富集的造血细胞群与偶联了荧光染料的CD34抗体接触。此标记抗体与造血细胞特异性地结合,但不与肿瘤细胞结合。然后将细胞群置于名义上的平面上铺满。细胞密度为每平方厘米约500,000个细胞的单层。打开紫外灯使偶联了荧光染料的CD34抗体发亮以识别造血细胞。
然后,操作者将氮气激光器的激光对准靶。用波长为375纳米、脉冲长度为5纳秒的激光束手工靶向并杀死每个不发荧光而被识别的肿瘤细胞。
一旦消灭了没有荧光的肿瘤细胞,就将其余细胞群低温保藏。在重新导人细胞前,给予患者化疗处理,以破坏患者体内已转移的肿瘤细胞。在该处理后,通过37℃迅速融化制得分离的细胞用于重新导入。然后将不含肿瘤细胞的造血干细胞移植到患者体内。随后,患者恢复而原来的癌症没有恢复。
虽然骨髓收集和重新导入程序通常由医生进行,但是本发明方法的操作者却无需医疗训练。
实施例2
进行一个实验以确定是否可用碘化丙锭(PI)来确认细胞群中的靶细胞已被激光处理所杀死。如本领域中所知的,碘化丙锭对双链核酸有特异性。由于PI能扩散到死亡细胞的细胞核内但不能穿透存活细胞的细胞膜,因此我们测试是否能通过细胞核内产生的PI颜色来确认靶细胞的存活与否。
另外,PI在493nm处有吸收峰,这与标记物藻红蛋白的480nm吸收峰很接近。因此,PI和PE可用同一光波长来激发。另外,PI的发射峰为617nm,这远得足以与PE的578nm区分。因此,即使当PE标记的抗体与细胞结合时仍能区分出阳性PE读数。
细胞系
人造血细胞系KGla从美国典型培养物保藏中心(Rochville,MD)获得。收到的细胞为低温保藏的,然后迅速将其融化并悬浮在含有4mM L-谷氨酰胺、3.04克/升碳酸氢钠和20%胎牛血清的IMDM(Life Technologies,Grand Island,NV)中。该细胞系每3至4天传代,并保持在37℃、95%湿度和5%CO2(ATCC指南建议)的恒温箱内。
细胞染色
如下方式对KGla细胞染色:根据生产商指导所建议的(Becton DickinsonImmunocytometry Systems,San Jose,CA),用藻红蛋白(PE)偶联的抗人CD34膜抗原的单克隆抗体对KGla细胞表面抗原直接进行免疫荧光染色。使标记的KGla细胞(靶)与未染色的KGla细胞(对照群体)以1∶100的比例混合。
加入碘化丙锭(Sigma Chemical Company,St.Louis,MO),至1×106个细胞/毫升中的最终浓度为5μg/ml。在384孔板(Nalge Nunc International,Naperville,IL)的孔中加入1.0×106细胞/毫升的混合细胞悬液各20μl等份,细胞沉降30分钟后,捕获图象并进行激光切除。
细胞图象捕获
图2描述了激光切除系统75的一个实例的示意图。该系统包括与个人电脑80相连的Charged Couple Device(CCD)相机77。个人电脑80是SGI O2工作站(Silicon Graphics,Mountain View,CA)。数字化的细胞图象被安装在带20倍物镜84的倒置Diaphot300荧光显微镜(Nikon Inc.,Melville,NY)中的冷却CCD相机77(Photometrics Inc.,Tucson,AZ)捕获。
用选择性荧光滤片86(B2A型;Chroma Technology Corp.,Brattleboro,VT)获得亮的PE发射图象。用IseeTM软件(Inovision Corp.,Durham,NC)在个人电脑80上运行显示和处理捕获的图象。
软件为系统提供了完全的自动化,包括阶段性工具(stage tool)操作、Z-方向聚焦、调整感兴趣的区域(例如靶)以及控制激光器。384孔板的每个孔中有63个视野图象区域,用Isee中的常规的阶段性工具软件正确控制视野间的分步移动。在给定参数(如亮度、大小和形状)下,识别视野内的每个靶细胞90,它们的位置通过IseeTM软件中的特征提取(feature extract)功能来调整和标记。在该程序中,靶细胞在盛放瓶的表面92上保持不动。
激光切除
用认证类IIIb氮气激光器94VSL-337型(The Laser Science Inc.,Newton,MA)来杀死个别细胞,激光器上安装了分束器96和Coumarin 440染色细胞98单元(Photonic Instruments,Arlington Heights,IL)。分束器96使来自激光器94的光通过,但折射汞弧灯99发射的蓝光。因此,汞弧灯99和激光器94的光沿同一途径被送到靶细胞90。
汞弧灯发射波长范围为450-490纳米的光。然而,如图2所示,来自汞弧灯99的蓝光通过D470/40激发滤片100(Chroma Technology Corp.,Brattleboro,VT),以便激活与KGla细胞的CD34表面抗原连接的PE偶联物。因此,同一途径载有激发光和激光。
然后,激光和激发性汞弧灯光联合通过第二个分束器102。分束器102将激光94和汞弧灯99的光折射通过显微镜物镜84到达靶细胞90上。分束器102还将标记的靶细胞发出的光通过滤片104传送到CCD相机77上。
应注意在正常的程序中,激光只在瞬时内发送。相反,汞弧灯光通常是一直发送的,从而使每一个标记的细胞可将光反射通过分束器102、滤片104并进入CCD相机77。
脉冲产生器105与激光器94相连,以提供杀死靶细胞90所需的最短的激光瞬时脉冲。脉冲越短越理想,因为周围细胞由于细胞基质的局部加热而被破坏的机会就越少。
用Isee软件系统捕获染色的靶细胞区域的数字图象,并调整和标记它们的位置。由于弧光灯99产生的宽带光使得碟上每个标记的细胞都产生荧光,因此计算机系统能一次性收集和处理视图的整个区域。
在细胞被来自激光器的短的光脉冲射击时,一种在垂直方向上可移动的z-马达110提供了将激光特异性地聚焦到碟中不同焦平面上的机制。z-马达110是一种与物镜84相连的电子控制的马达。z-马达的连接使得物镜84可以在垂直方向上下移动以便精确地将激光聚焦到不同焦平面上。另外,z-马达可由计算机系统80来控制以便将激光脉冲聚焦于从靶细胞90底部到靶细胞顶部(移动)。用z-马达将聚焦激光脉冲补偿了由于并非完全平的组织培养板表面而引起的细胞位置在垂直方向上的变化。
细胞成功消灭的确定
在每个细胞遭受激光脉冲后,用计算机分析PI发射峰617nm处的光谱。如果细胞被激光杀死,则将在细胞的原始位置发现617nm发射峰。如果没有发现PI发射峰,则激光器会向该细胞位置发射另一光能脉冲。通过这种方式,可以确认细胞群中的每个肿瘤细胞已经被杀死。
结论
本发明能使医生有效治疗需要造血细胞移植的患者,而无重新导入癌细胞引起复发的危险。本文所示的寻靶方法提供了一种从造血干细胞或其它细胞群中完全或几乎完全除去污染的肿瘤细胞的方法。
在不脱离本文的精神或实质性特征下,本发明可能还包括其它具体的形式。本文描述的实例均应被认为仅仅是描述性的,而不是限制性。因此,本发明的范围由所附权利要求而不是前文描述来表明。在与权利要求等价的含义以及范围内的所有变化均包括在该范围内。
Claims (30)
1.一种在体外从包含非肿瘤细胞的细胞群中清除肿瘤细胞的方法,该方法包括下列步骤:
a)对所述细胞群进行标记,以使所述肿瘤细胞可以与所述非肿瘤细胞区分开来;
b)参照所述标记确定一个所述肿瘤细胞的位置;和
c)当所述肿瘤细胞在表面上处于基本静止不动的位置时,通过施加受控能源的脉冲至所述已被定位的肿瘤细胞将所述肿瘤细胞杀死。
2.根据权利要求1所述的方法,其中对于所述非肿瘤细胞群中的每个肿瘤细胞重复步骤b)和c)。
3.根据权利要求1或2所述的方法,其中所述非肿瘤细胞群是造血细胞群。
4.根据权利要求1至3任一所述的方法,其中所述造血细胞群包含已富集了造血干细胞的造血细胞群。
5.根据权利要求4所述的方法,其中所述造血干细胞群根据CD34表面蛋白的表达来富集。
6.根据权利要求4所述的方法,其中所述造血干细胞群根据选自Thy-1、CD38、CD15、CD11b、Gly-A、CD3和CD19的细胞标记的表达来富集。
7.根据权利要求4所述的方法,其中所述富集的细胞群用厚层过滤或逆流淘析来产生。
8.根据权利要求1至7任一所述的方法,其中步骤b)和c)在计算机控制下进行。
9.根据权利要求1至7任一所述的方法,其中所述受控能源是激光器。
10.根据权利要求9所述的方法,其中所述激光器发射的光波长在375纳米至900纳米之间。
11.根据权利要求1至10任一所述的方法,其中所述标记对所述肿瘤细胞有特异性。
12.根据权利要求1至11任一所述的方法,其中所述标记对所述非肿瘤细胞有特异性。
13.一种体外富集造血细胞群中干细胞数量的方法,该方法包括下列步骤:
a)用干细胞特异性标记对所述造血细胞群作标记;和
b)当所述干细胞在表面上处于基本静止不动的位置上时,向所述群体中的至少一个未标记的细胞施加高能激光光脉冲。
14.根据权利要求13所述的方法,其中所述施加步骤包括对所述群体中几乎所有未标记的细胞施加所述高能激光光脉冲。
15.根据权利要求13或14所述的方法,其中所述干细胞特异性标记是CD34。
16.根据权利要求13至15任一所述的方法,其中所述激光器发射的光波长在375纳米至900纳米之间。
17.根据权利要求13至16任一所述的方法,其中所述施加步骤在计算机控制下进行。
18.一种体外制备用于重新导入患者体内的分离的造血细胞的方法,该方法包括:
a)用肿瘤细胞特异性标记对所述分离的造血细胞中的肿瘤细胞作标记;和
b)当所述标记的细胞在表面上处于基本静止的位置时,向所述群体中的至少一个受标记的细胞施加高能激光光脉冲。
19.根据权利要求18所述的方法,其中所述施加步骤包含向所述群体中所有标记的细胞施加所述高能激光光脉冲。
20.根据权利要求18或19所述的方法,其中所述激光器发射的光波长在375纳米至900纳米之间。
21.根据权利要求18至20任一所述的方法,其中所述施加步骤借助计算机来自动实施。
22.一种在体外从第二群细胞中清除第一群细胞的方法,该方法包括:
a)对所述第一群细胞进行标记,以使所述第一群细胞可与所述第二群细胞区分开来;
b)将所述第一群细胞和所述第二群细胞置于表面上静止位置处;
c)参照所述标记确定所述第一群细胞中的一个细胞的位置;
d)向所述该细胞施加来自受控能源的第一脉冲;
e)确定所述该细胞是否被所述第一脉冲杀死;和
f)如果所述该细胞未被杀死,则施加来自受控能源的第二脉冲。
23.根据权利要求22所述的方法,其中标记所述第一群细胞的方法包括标记肿瘤细胞。
24.根据权利要求22或23所述的方法,其中从第二群细胞中清除第一群细胞的方法包括从第二群非肿瘤细胞中清除第一群的肿瘤细胞。
25.根据权利要求22至24任一所述的方法,其中施加第一脉冲的方法包括施加激光脉冲。
26.根据权利要求22至25任一所述的方法,其中施加第二脉冲的方法包括施加激光脉冲。
27.一种在包括非肿瘤细胞的细胞群中清除肿瘤细胞的方法,该方法包括下列步骤:
a)对所述非肿瘤细胞作标记,以使非肿瘤细胞与所述肿瘤细胞区分开来;
b)通过识别所述细胞群中的未标记细胞来确定所述肿瘤细胞中的一个细胞的位置;和
c)当所述肿瘤细胞处在表面上基本静止不动的位置时,向所述位置的肿瘤细胞施加受控能源的脉冲将所述肿瘤细胞杀死。
28.根据权利要求27所述的方法,其中杀死所述肿瘤细胞的步骤包括施加激光脉冲。
29.根据权利要求27或28所述的方法,其中杀死所述肿瘤细胞的步骤包括当所述肿瘤细胞在名义上的平面上处于静止不动位置时向所述位置的肿瘤细胞施加受控能源。
30.根据权利要求27至29任一所述的方法,其中杀死所述肿瘤细胞的步骤包括当所述细胞群铺满时向已定位的肿瘤细胞施加受控能源。
