Abstract

Description
本发明涉及一种应用靶向磁滞疗法治疗患者的方法。具体地说,本发明涉及一种应用定位磁滞热损失技术治疗患者的方法。
例如恶性肿瘤等的人体疾病通常由手术切除、化疗、放疗或这些方法联合治疗。每一种方法都有其局限性从而影响着临床应用。手术切除在病变呈大范围扩散或在不能手术的位置时是不适合的。化疗药物通常是非特异性的,因而导致正常细胞与病变细胞的同时死亡。与化疗一样,放疗也是非特异性的从而导致正常组织暴露于电离辐射而死亡。更进一步,一些疾病诸如肿瘤等对电离辐射相对地会有抵抗能力。这在肿瘤肿块核心尤其是个问题。
高温疗法已经被提出作为一种癌症治疗方法。有大量的公开证据证实高温疗法对于治疗类似癌样生长的病变有效。高温疗法的治疗学优点通过两个主要机制起作用:(1)由升温超过42℃产生的对于组织的直接肿瘤杀伤作用而导致癌细胞的不可逆损伤;和(2)高温疗法被认为可增敏放射治疗和某些化疗药物对癌细胞的作用。与放疗和化疗相比,高温疗法的无蓄积毒性更说明寻求发展完善的高温治疗体系的必要性。
哺乳动物细胞以一种时间/温度和细胞周期依赖的方式承受高温损伤。这种对热的细胞反应依次被多种细胞内和细胞外的环境因素所调节。影响高温细胞损伤的细胞内因素包括不同种、器官甚至细胞系间的内在差异。细胞外因素包括细胞的氧和营养状况、细胞外介质的pH值、绝对的温度升高和高温的持续时间。
尽管有一些证据表明肿瘤细胞比相应的正常组织细胞对高温的作用更敏感,但这种现象并不普遍,而且最近的一些研究表明组织对于高温损伤的敏感性与细胞的肿瘤-正常状态并没有很大联系。
若干研究证实高温疗法和放疗有协同作用。即使是很小的温度变化也能显著地影响细胞受辐射损伤后的生存前景。
影响高温疗法和放疗协同作用的因素包括高温疗法持续程度、高温疗法和放疗的次序、分次的和总的辐射剂量、细胞外介质的pH值、细胞的氧状态和营养状况以及细胞的组织学类型和恶变状况。
所有肿瘤的无血管部分中心的细胞都是处于酸中毒、缺氧且营养缺乏的状态。所有这些因素看起来都独立地加强高温疗法的作用。由于同样的原因,严重缺氧细胞抗电离辐射的能力约是有氧细胞的三倍。最重要的事实是,虽然这些缺氧细胞可以在受辐射后生存,但高温却能部分地消除这种抗辐射能力,并能增强放疗对酸中毒和缺氧细胞的杀伤力。
目前对患者引入临床高温疗法的可用方法尚有许多问题。正常人体组织和器官是热敏感的,在超过42℃的温度下很多组织将发生不可逆损伤。目前可用的产生临床高温治疗的方法是非特异性的,会将正常组织和肿瘤细胞一样地加热。几乎所有的加热技术都在一个广泛的靶向区域产生热而对病变组织很少有特异性,尽管目前用于超声和电磁生热的聚焦装置正在发展以改善热生成在更精确靶区域的集中。
目前已有一些用于引发临床高温的技术,它们既能局部地选择特定器官的部分区域也能在全身起作用。这些技术的一些将在下面讨论。
全身性高温可被内源性或外源性的热源诱导,但通常在无麻醉时不能耐受42℃以上的高温。局部高温技术包括:器官灌流、各种形式的电磁辐射或超声波。
普通波的电磁或超声加热因很差的组织穿透性和随深度增加的能量迅速衰减而受到限制。
频率从0.3到3MHz的超声波因诸如空气、骨骼等组织界面产生的干扰而受到限制。然而,目前正在发展的改良聚焦装置可能使之成为一种更可接受的深层组织加热方式。
频率在434至2450MHz的微波加热已被应用,尽管通常其组织穿透性很差。相阵列(phase array)装置能使微波能量在深层组织聚焦,但加热效果的变化不定仍是个问题。
频率直到434MHz的射频波已有一些成功的应用。这些加热技术同时包括介电和电感的方式,并甚至能导致相对均匀组织的加热。但是应用感生电流对深层器官加热的聚焦仍是个问题。
这类治疗方法要有效有两个基本要求。首先,需要定位治疗特定靶点位置。其次需要维持高温治疗在对患者的安全操作范围内最大化对病变组织的加热程度。
虽然应用高温疗法治疗体表肿瘤已取得很大成功,却仍然需要一种选择性定向和治疗患者病变组织的方法。穿透深度不足和外部应用的微波和超声波束较差的聚焦能力这两个主要局限严重限制了医生有效地将足够的热负荷输送到深层成熟病灶而不给周围健康组织带来同样不可耐受的损伤。本发明寻求在应用高温疗法时至少改善在热的穿透深度和定位不足方面的问题。本发明的概括
本发明提供了一种改进了的位点特异性治疗患者病变组织的方法,它包括以下步骤:
(ⅰ)至少选择一种磁性材料,当磁场条件等于或小于7.5×107A/s时,其磁加热效率至少为4.5×10-8J.m/A.g;
(ⅱ)将磁性材料输送到患者病变组织;
(ⅲ)将患者体内的磁性材料置于一个线性变化的磁场中,其频率大于10KHz,其场强要使场强、频率以及暴露区域半径的乘积小于7.5×107A/s,从而在病变组织中产生磁滞热。
本方法中的步骤(ⅰ)到(ⅲ)优选反复进行直到病变组织被破坏或使病情得到足够改善。
当磁场条件等于或小于7.5×107A/s时,本发明方法中使用的磁性材料必须具有大于约4.5×10-8J.m/A.g的磁加热效率(MHE)。当磁场条件等于或小于7.5×107A/s时,优选选择MHE大于约7×10-8J.m/A.g的磁性材料。磁场条件等于或小于7.5×107A/s时,最优选选择MHE大于约1×10-7J.m/A.g的磁性材料。
使用具有很大MHE磁性材料的优点包括:
1)通过更快地达到更高肿瘤温度的优点改善治疗效果(当温度超过42℃时高温疗法的效果显著改善);
2)减少毒付作用,因为:
ⅰ.肿瘤中用于达到治疗性加热的微囊更少(若微囊有任何内在毒性则更具优越性),
ⅱ.能使用更低的磁场强度H,
ⅲ.能更快地加热肿瘤意味着更少地牵连肿瘤周围健康组织(加热肿瘤所需的时间越长则越多的周围组织被热传导所加热);
3)增加成功治疗的可能性,尤其对于那些否则只能期望获得边缘性益处的肿瘤;
4)此技术对于不同类型癌的治疗有更广泛的应用价值;
5)使用减小的磁场强度降低机器设计上的工程难度;
6)使用减小的磁场强度意味着减少机器运转时的电力消耗和冷却要求。
适用于本发明的磁性材料的选择基于材料的MHE。MHE可由以下公式算出: 公式中Physt是由磁滞损失效应产生的加热功率(单位W/g),H是所用磁场的振幅(单位A/m)而f是所用磁场的频率。用于治疗病变组织的磁滞所致的热产生的主要限制来自于随时间变化的磁场对活组织的影响。一般来说,f和H的乘积增加使这些影响增大。因此,使Physt最大化且使f和H乘积最小化是至关重要的。
更进一步,Physt可由以下公式算出:
Physt=f.W (W/g) (2)式中W是磁性材料在所用磁场每个周期中产生的磁滞热能(单位J/g),而f和前面一样是频率。
合并(1)和(2)式消去f项,则可看出只要已知H和W就可计算MHE。W必须按每个H值实验测得。这可由这里叙述的方法实现。于是从式(1)和(2)计算出MHE。
W能用下述几种不同的方法确定:
1)对于磁滞测量使用一个振荡样品磁强计(Vibrating SampleMagneto meter,VSM)测定W。将一定已知量的磁粉(典型情况下少于1克)用非磁性的环氧树脂固定于一个非磁性、非金属的VSM样品容器中。样品最初处于去磁化状态而W值在随后的更高场强下测定。
2)一个50Hz的交变场磁强计也被用于测量W。为VSM准备的样品放入一个小线圈中。然后小线圈放在一个可以产生50Hz交变磁场的磁铁的磁极间。小线圈中感生出的电压等于N.dB/dt,这里N是线圈的匝数。此电压信号经结合、校正空气磁通量后,做出磁化强度M对H的图。此即磁滞回路(hysteresis loop),其面积等于W。
3)一个可替换的方法是:取已知量的磁粉,典型情况为125毫克,将其分散于5毫升琼脂凝胶中(3%琼脂溶于热水,当冷却回室温时琼脂固化)。将一个温度探针插入凝胶中并将其全部暴露于所需强度的交变磁场中。从所得的温度对时间的曲线就可以计算出一定H下的W值。
任何在磁场条件等于或小于7.5×107A/s时表现出磁滞现象且其MHE大于4.5×10-8J.m/A.g的磁性材料都可用于本发明。磁性材料优选铁磁材料。铁磁材料可包括诸如铁、锰、砷、锑和铋等元素,但并不仅限于这些元素。可选择的磁性材料种类包括CrO2、γ-氧化铁(钴处理和非处理的)和金属铁、钴或镍。具有一般形式的铁氧体MO.Fe2O3这里M为二价金属,例如Mg、Mn、Fe、Co、Ni、Cu、Zn、Cd或Li,钴处理的铁氧体或具有一般形式MO.6Fe2O3的磁铁铅矿型氧化物(M型),这里M为大的二价离子,例如Ba、Sr或Pb,也都是本申请有用的磁性材料。更进一步,超顺磁、单畴粒子也可用作磁性材料。最优选从已知的γ-氧化铁(γFe2O3)类铁磁物质中选择所需铁磁材料。
可从中选择磁性材料的合适铁磁物质的实例包括钴处理的γ-氧化铁、一些非钴处理的γ-氧化铁、钴处理的铁氧体和二氧化铬。
本发明方法提供了一种将病变组织升温至41℃以上而降低恶性细胞生存能力的手段。恶性细胞生存能力的降低导致细胞死亡或增加细胞对电离辐射或化疗药物的敏感性。
磁性材料在所用磁场每个周期中产生的磁滞热的量由W给出。为了将磁滞热能转化为能够加热组织的热能,磁场必须有高交变频率。在治疗时,患者被置于一台产生具有强度H和频率f的磁场的机器中。频率越高则含有磁性微囊的组织的加热速率越快。然而对高振幅、高频率磁场的生理学反应限制了临床应用中的磁场振幅和频率。这些限制是由神经肌肉激活和特别依赖于组织导电性的涡旋电流生热造成的。此二者均为由磁场在组织内感生出的电场所致。这些有潜在危害的感生电场与H、f和正对磁场方向的暴露区域半径r的乘积的平方成正比。H、f和r的乘积很大程度上决定了磁场条件。H、f和r的乘积值不能超过7.5×107A/s、即H.f.r≤7.5×107A/s。为说明这一点,请考虑全身暴露于沿身体轴向磁场的情况。这种情况下,r一般为0.15m,所以f和H乘积不能超过5×108A/m.s。
本发明中所用的磁性材料可用任何已知工艺中的方法输送到患者的病变组织。合适的给药途径可包括肿瘤内、肿瘤周围和血管内给药(如动脉内、腹膜内、皮下或鞘内注射)。磁性材料优选通过动脉或静脉血流输送到病变组织。
优选将磁性材料混入乳液中或置于微囊中,然后它们能和合适的生物相容性介质混合以输送入患者体内。最优选将磁性材料置于一种基质材料中形成微囊。典型地,大多数磁性粒子本身太小、太稠密以致于不能取得到病变组织部位的最佳输送。所以应将其包裹入微囊中。微囊的重要性质是它的密度和直径。密度影响微囊被血流携带到病变组织血管网络中固定位置的效率,而尺寸大小则决定了固定位置离病变组织的距离。
优选将磁性材料置于一种不对磁性粒子磁滞或涡旋电流生热性质有不利影响的基质材料中。这种非毒性的结合剂或基质材料可包含任何微囊技术中已熟知的适当非毒性材料,适当的材料例如有蛋白质、聚合物树脂,如苯乙烯-二乙烯基苯聚合物等、Biopol、白蛋白、chitoxan等。
本发明的优选形式是将微囊改造成结合、吸收或包含有细胞毒物质,此物质可以在加热微囊时释放出来。例如,微囊可以由一种多孔性、热敏感的材料组成,此材料对于动物组织来说是非毒性的,优选惰性或相容性的,而且在其中充填有适当的磁性材料。微囊材料的孔中则可按需要填充细胞毒化合物。在磁滞加热作用下微粒应能够膨胀从而释放出细胞毒化合物。但这种粒子应能在磁滞加热作用下抗熔化。因此,本发明方法中这种微粒的应用提供了一种能够将化疗和高温疗法联合用于治疗患者病变组织的简单装置。
根据本发明的进一步体现,可以将电离辐射源和磁场共同用于病变组织部位,这里所说的含有微囊的组织包括在前面叙述中。辐射源可以是含有诸如钇-90等的放射活性化合物的微囊或是从一个外部辐射源输送。本发明的详细说明:
图1表示一个代表性的磁滞回路,它说明当所用磁场(x-轴)周期性变化时磁性样品的磁化强度(y-轴)是如何变化的。一个完整的周期产生一个磁滞回路。回路的面积给出一个W值。
图2表示MHE作为所用磁场强度的函数,用于选择磁性材料。
图3表示暴露于高频磁场时所选择磁性材料的加热情况。
图4说明了使用等于临床相应剂量的微囊加热灌注良好的活组织时微囊的加热效率。

在41℃或更高温度范围(高温)内的热生成会引起病变细胞的不可逆损伤。因此,作为适当磁性材料磁滞热损失的结果,可以通过升高病变组织的温度来治疗病变组织(热疗)。磁滞加热产生的热优选在从42℃到60℃的范围。
本发明可以用于治疗任何对热疗、化疗、放疗、或热疗与化疗或放疗联合疗法敏感的病变组织。本发明优选用于治疗癌样生长或含有一个或多个肿瘤的组织。虽然下面将讨论微囊的应用和癌/肿瘤的治疗方法,但应考虑到本发明方法的应用要超出癌/肿瘤的治疗和仅应用微囊的范畴。
当一种磁性物质被置于一个强度周期性变化的磁场中时,由于磁滞损失产生一定的热。每个周期产生的热量依赖于因不同铁磁材料和不同磁场条件而不同的磁滞损失。对于多磁畴铁磁材料,磁滞损失不依赖于粒子尺寸。包埋于肿瘤部位周围并置于振荡磁场的磁性粒子中将被加热到一个依赖于材料磁学性质、磁场强度、振荡频率和肿瘤部位血流冷却能力的温度。
每当一个铁磁样品置于一个交变磁场时,作为其磁滞损失的结果,将产生热的形式的能量。每个磁场周期中由磁性材料产生的磁滞能量的量由W(J/g)给出。因此,由磁滞效应产生的加热功率可由下式给出: 式中f为所用磁场的交变频率,ρ为磁性材料的密度。∮HdB的值等于磁滞回路的面积,此面积在特定磁场条件下表明了磁性样品的特性(一个典型的磁滞回路实例如图1所示)。
为了确定出使磁性微囊成为有效治疗剂所必需产生的最小热量,需要先确定Ptumor(W/cm3),Ptumor由下式给出:

Ptumor在本质上代表了肿瘤组织的加热速率。当涉及本发明时Ptumor应足够大以至于它能在合理的时间内使肿瘤组织温度从体温升高到一个对病变组织致死性的温度。而且,Ptumor应该足够大以克服组织的各种冷却因素,诸如血流和组织的热传导等。Ptumor优选大于60mW/cm3。更优选为大于80mW/cm3,而最优选为大于100mW/cm3。
为了得到优选范围内的Ptumor值,需要为变量f、W、ρ、v和n选择适当的值。
关于人体对振荡磁场反应的可用实验数据很有限。这些数据导致从10kHz到100MHz的最适频率范围的确定。小于这个范围的频率有不自主神经肌肉激动的危险,而高于这个频率则因电磁能在组织中穿透力的降低开始出现局限。因此,频率要维持在10kHz到100MHz的范围内。频率优选维持在10kHz到500kHz的范围。更优选地,频率应维持在10kHz到100kHz之间,最优选的是不超过50kHz。例如频率可以是20kHz。
W(J/g)制成微囊的磁性材料的内在性质。当所使用磁场的最大振幅H增加时,W增大。但是将本发明方法应用于患者时H能够增加的范围是有限制的。这种限制也依赖于所用的频率和暴露于磁场的组织面积。当磁场条件维持在对患者的安全操作范围内时,选择用于微囊的磁性材料的MHE至少应为4.5×10-8J.m/A.g。当磁场条件等于或小于7.5×107A/s时,优选选择MHE大于1×10-8J.m/A.g的磁性材料。当磁场条件等于或小于7.5×107A/s时,最优选选择MHE大于1×10-7J.m/A.g的磁性材料。
对磁场强度和频率的要求也依赖于微囊的性质。这些性质由下面几个参数决定:ρ(g/cm3)、v(cm3)和n/cm3。
本发明方法所用的微囊应该具有合适的尺寸以穿过患者的血管网络,并在病变组织中分散和栓塞(在有或无血管活性药物的协助下)。所用的微囊应能够被器官、肿瘤或组织的前毛细血管和毛细血管网络所捕获而不返回全身静脉循环中。优选地,微囊直径应大于10微米以使其进入肿瘤的血管供应体系,而又应小于500微米以使其在到达肿瘤之前不在血管中栓塞。微囊尺寸大小更优选10至100微米,最优选30到40微米。
小于10微米的较小微囊如果能通过胞吞过程掺入肿瘤组织则也可用于本发明方法。
此外,铁磁材料的密度应该使微囊能被患者体内的血流所携带。微囊优选具有1~5g/cm3的密度。更优选的密度为1.8~3g/cm3。最优选的密度为1.8~2.2g/cm3,如2g/cm3。
采用多样的基质材料和制造工艺,有许多不同的方法可用于制备微囊。在本发明的一个优选形式中,将包含钴处理的γFe2O3粒子的微囊作为铁磁材料,用Biopol基质((R)-3-羟基丁酸和(R)-3-羟基戊酸的共聚物)使之结合。使用这种基质,可获得密度在1.8~2.2g/cm3和尺寸在20~50微米的磁性微囊。
微囊可由生物降解或非生物降解的材料制成。本发明所用的微囊优选采用非生物降解材料制成,它可永久地滞留于肿瘤血管网络中。因此它们能反复地被用以产生局部化的肿瘤加热作用。通过将长有肿瘤的器官置于磁场中,包含在微囊中的铁磁材料将发热产生高度局部化的肿瘤加热作用,与此同时不影响周围的正常实质。
微囊可以用任何适当的已知技术制成(实例见《化工百科全书》Encydopedia of Chemical Technology,KIRKO-OTTHER,Vol.15Wiely-Interscience)。例如,可以将铁磁性粒子加入蛋白质溶液中,如白蛋白溶液。然后所得的蛋白质溶液优选被加入不断搅动的油相中从而形成乳液。随后这些蛋白质材料可用加热或诸如戊二醛的化学试剂进行交联,形成包含铁磁粒子的微囊。
在另一种方法中,可将铁磁粒子加入含有Biopol的二氯甲烷溶液中。然后优选将此混合物滴入一个盛有聚乙烯醇或类似物的烧杯中,同时用均匀搅拌器进行混合。其后,应将此混合物缓慢搅拌一段适当的时间使二氯甲烷挥发掉。于是就形成了微囊,随后可进行洗涤并按大小筛分。
制成微囊后,制品可按大小筛分选择出本发明方法优选使用尺寸的粒子。优选地,制品也可用密度筛分来选择具有优选密度的粒子。
在本发明的一个具体实例中,由一种对动物为非毒性、优选为惰性或相容性的材料组成的,至少结合有一种铁磁性材料的微囊被靶向地(直接或间接)输送至患者的病变组织。此微囊然后被置于一个小于7.5×107A/s的磁场,持续足够的时间对病变组织进行治疗。治疗这样的组织所需的时间依赖于微囊中产生的热量,而这依赖于所用的磁场和所用微粒的性质。
有多种不同的给药途径可用于治疗患者及动物。所选择的具体给药方式当然依赖于具体的治疗条件和达到所要求疗效所需的微囊数目。一般来说,本发明方法可以用任何医学上可接受的方式实施,此方式能够选择性地将微囊输送到病变组织而不引起临床不良反应,并且能够将微囊输送到患者的病变组织以使微囊以一种实质上均匀的方式遍布病变组织。这样的给药方式可包括非胃肠道的途径(如皮下、肌肉内、动脉内和静脉内)。
本发明的一个具体实例中,优选通过将微囊混悬液注射到病变组织的动脉(或门静脉)血流供应体系中来输送微囊。适于方便地非胃肠道给药的组分包括微囊的无菌水溶液制剂,此水溶液优选与受者血液等渗。无菌制剂可以是无毒的非胃肠道可接受稀释剂或溶剂的注射液或混悬液。能用的可接受载体和溶剂包括水、林格氏溶液和等渗氯化钠溶液等。
本方法中每单位体积的微囊数完全依赖于患者待治疗病变组织的数量。每克组织的微囊数优选在5,000~300,000(微囊数/g)的范围。更优选在10,000~100,000的范围,最优选在40,000~70,000的范围。例如,n约为每立方厘米肿瘤组织中60,000个微囊。
例如,如果本发明用于治疗肿瘤或癌组织,微囊应进入并在含肿瘤组织的血管网络中栓塞以使微囊浓集于肿瘤部分而不滞留于周围正常实质中。
正常组织和浸润肿瘤的交界区域处的血管系统主要由含肾上腺能受体的小动脉组成,而肿瘤内的血管则失去这些特性。虽然肿瘤血管床几乎没有血流调节作用,但邻近正常组织的肿瘤的小动脉供血却服从于正常的血管运动调控。肿瘤中这种血流调控作用的丧失正是在增加热量输入的情况下,为何肿瘤不能以正常组织同样速率散热的主要原因。这样就导致了肿瘤组织的优先加热。
进行性肿瘤生长导致肿瘤中心区域变得相对少血管和缺氧。这些区域通常仍有萎陷的血管,它们在某些血管活性药物的作用下能输送血流。含有铁磁性材料的微粒子进入肿瘤组织血管床的能力可通过用血管活性药物操纵肿瘤和周围组织血流的方法来增强。本发明的一个具体实例中,微囊优选在血管活性药物控制下输送至病变组织。更优选地,正常实质用血管收缩药物处理以阻止微囊进入组织。
载有铁磁性材料的微囊和血管活性药物例如血管紧张素Ⅱ、去甲肾上腺素加β阻断剂、加压素、肾上腺素或其它血管活性药物等联合给药可使肿瘤中心部分萎陷的微循环重新疏通并将微囊输送至这些区域。血管活性药物的作用消失后肿瘤中心部分又将回复到少血管和缺氧状态,但将对高温损伤敏感。
这样,肿瘤血管的生理学无反应性现象可以被操纵,从而使微囊选择性靶向地输送至肿瘤组织。将血管收缩药物注入长有肿瘤的器官的动脉循环中会导致给正常组织供血的血管暂时收缩,但并不引起给肿瘤供血血管的收缩。当注入血管活性药物后立即将微囊导入动脉循环,微囊会优先定向至并被捕集于肿瘤血管网络,而不在正常组织中滞留。血管活性药物的这种作用将在几分钟内消失。然而,此时微囊会牢牢地滞留于肿瘤毛细血管网络。相反地,血管舒张药物可用于选择性靶向输送辐射保护或热保护药物至正常非肿瘤组织。
通过血管途径输送铁磁性微囊与直接注射相比有如下优点:
(ⅰ)微囊和血管活性药物给药联合应用动脉输送能使微囊在病变组织内均匀或实质上均匀分布,而不会将微囊输送至正常实质。与此相反,直接将微囊注射入病变组织不能使微囊平均或实质上的平均分布。在这种情况下,当微囊被注射进病变组织时,将在注射部位周围有最高浓度。随着远离病灶注射位点,单位体积病变组织中的微囊密度逐渐降低。
(ⅱ)动脉输送微囊可减少遗漏继发性肿瘤的危险,而这在注射法输送微囊中可能出现。
(ⅲ)动脉输送微囊避免了对所有肿瘤手术介入的需要。
(ⅳ)动脉输送微囊避免了肿瘤细胞扩散的可能性,而这在肿瘤被穿刺时可能会发生。
根据本发明进一步的具体实例,载有铁磁粒子的微囊与一种或多种血管活性药物共同导入肿瘤或含有肿瘤的组织。然后用一个磁场作用于肿瘤位点来诱导由铁磁粒子磁滞加热或涡流生热而导致的加热作用。
任何能给出所需场强和频率的磁体均可用于本发明。合适的磁体包括空心线圈或迭片硅铁磁芯电磁铁以及铁氧体磁芯磁铁。磁体可为便携式的。
一些不同的装置可用于产生符合条件的时间变化磁场。
一种交变磁场的数学描述为:
H(t)=H.Sin(2πft) (5)式中H(t)是在时间t时所用磁场的强度,H是所用磁场的最大振幅,而f是其交变频率。任何能产生这样磁场的装置均可用于本发明。
当使用一个交变磁场时,用于产生磁场的装置优选地使用一种与一个电容器组相联的电感元件,从而形成一个串联或并联的谐振电路。此谐振电路优选由一个适当的具有相匹配变压器的电源所驱动。磁动力优选由一种合适的电感元件,诸如一个或一对线圈产生。在某些情况下,线圈可加上一种不导电的高磁导率磁芯改善其效果。在一个实施例中,线圈由一种低电阻金属如铜制成。线圈优选用一种适当的冷却方式冷却,例如循环水和液氮。更进一步,线圈可由空心管来制造,冷却剂从空心管中流过;或者线圈也可由很多直径很小的绞合线,例如李兹(Litz)线组成,它被浸入冷却剂中冷却。
本方法所用的装置优选能在大到足以容纳一个患者的空间区域内产生所需的磁场条件。而且,此装置优选能够最大化微囊的MHE值。
本发明更进一步的特征将在下面的实施例中更详细地描述。然而需要理解的是,这些详尽的描述只是出于说明本发明的目的,而不应被理解为是对上面概括性叙述的限定。实施例1铁磁材料的选择
本实施例比较了在交变磁场中大量不同铁磁性材料的产热效率。
下述铁磁性材料(见表1)由工业来源获得:

根据前述方法检验这些磁性材料的产热效率。此分析方法涉及在样品振荡磁强计或50Hz交变场磁强计中测量每种材料的磁滞回路。这些分析的结果如图2所示。
少量上述磁性材料样品暴露于高频磁场(53kHz,28kA/m)中时放出的热也进行了直接测定。测定的结果如图3所示。这些结果显示了γFe2O3材料(钴处理的和未处理的)在特定场强下具有明显的更优越产热特性,反映出它们更高的MHE因子。铁磁性微囊的制备
当场强为47.1kA/m时有最大MHE值1.05×10-7J.M/A.g的γFe2O3粒子由Bayer Chemicals获得。将1克的γFe2O3粒子与含15%Biopol(Fluka Chemie,瑞士)的二氯甲烷溶液完全混合。将此混合物滴入一个盛有150毫升0.25%聚乙烯醇溶液(2.5克87~89%水解的PVA,分子量124,000-186,000,溶于1升水中)的烧杯中,并用一个均化搅拌器以3900-4000转/分钟的转速搅拌将此混合物搅拌10分钟后,再缓慢搅拌60分钟以使二氯甲烷全部挥发。
这样形成的微囊相继用63、45和20微米的筛进行洗涤。保留20到45微米间部分。然后,将微囊悬浮于经少量丙酮稀释、比重为2.2的二碘甲烷中。弃去所有沉降的微囊。剩余物经洗涤后悬浮于稀释到比重为1.8的二碘甲烷中。将沉降下来的微囊回收,洗涤后备用。活组织的加热
本实验表明用等于临床相应剂量的按上述方法制备的微囊能用于加热灌注良好的活组织。
经肾动脉将按上述方法制备的磁性微囊注入兔肾脏。三种不同剂量,50mg、25mg、12.5mg(相应于浓度约为125,000,62,000和31,250个微囊/立方厘米组织)的微囊分别用于各分次实验中。固定好温度计探针(fluoroptic探针,Luxtron公司)以测量全肾脏各处的温度。然后将动物置于可产生交变磁场的装置中,磁场强度为28kA/m,频率为53kHz,半径为0.05m(MHE=6.1×10-8,即磁场条件为f.H.r=7.4×107A/s)。开启磁场,监测温度约15分钟。典型数据如图4所示。因为肾脏是身体中灌注程度最高的器官,因此最难于加热,这些数据证明本发明所述方法甚至在最极端的条件下也能加热组织。靶向加热肿瘤
本实验显示,上述方法是如何能够用于加热肝脏肿瘤组织到达治疗温度而保持周围健康组织不受影响的。
将少量VX2癌移植到半垂耳兔(Half lop rabbit)的肝脏表面下。当肿瘤长到大约1cm3时,将按上述方法制备的50mg微囊用5ml盐水通过一根插于流入肝脏的肝动脉附近的胆囊动脉上的0.8mm O.D.导管注入兔子体内。将温度计探针固定就位以测量肿瘤坏死核心、肿瘤生长边缘、靠近肿瘤的正常肝组织以及距离更远的肝脏部位的温度。然后将兔子置于上述的磁场装置中(f.H.r=7.4×107),开启磁场,监测温度。大约1小时后关闭磁场使温度回复到正常值。图5显示了按此程序所得的一次实验数据。肿瘤和正常组织加热效果的差异显而易见。肿瘤被加热到并保持在治疗域温度42℃,而正常肝脏的温度则不超过40℃。疗效的评价
本实验检验了上述方法的疗效。
上述操作过程均在无菌条件下重复。将肿瘤温度维持在等于或高于42℃30分钟。兔子复苏后保留饲养7或14天的时间。在这些时间点上处死兔子,切除其肝脏并记录肿瘤的重量。这些重量的数据与未加治疗的对照组的数据都显示在图6中。结果表明,治疗后14天时的肿瘤重量有巨大和显著的差异。
Claims (32)
1.一种用于位点特异性治疗患者病变组织的方法,它包括以下步骤:
(ⅰ)选择至少一种磁性材料,当磁场条件等于或小于7.5×107A/s时,其磁加热效率至少为4.5×10-8J.m/A.g;
(ⅱ)将磁性材料输送到患者病变组织;
(ⅲ)将患者体内的磁性材料置于一个线性交变的磁场中,其频率大于10kHz,其场强要使场强、频率以及暴露区域半径的乘积小于7.5×107A/s,从而在病变组织中产生磁滞热。
2.根据权利要求1的方法,重复其中的步骤(ⅰ)至(ⅲ)直到病变组织得到充分治疗以至缓解疾病。
3.根据权利要求2的方法,其中病变组织包含至少一种癌样生长或一种或多种肿瘤。
4.根据权利要求1的方法,其中当磁场条件等于或小于7.5×107A/s时,所选择磁性材料的MHE值大于7×10-8J.m/A.g。
5.根据权利要求1的方法,其中当磁场条件等于或小于7.5×107A/s时,所选择的磁性材料的MHE值大于1×10-7J.m/A.g。
6.根据权利要求1的方法,其中磁性材料为铁磁性材料,它至少包含选自铁、锰、砷、锑和铋的一种元素。
7.根据权利要求1的方法,其中磁性材料从以下物质中选择:CrO2,金属铁,钴,镍,γ-氧化铁,钴处理的γ-氧化铁,具有通式MO.Fe2O3的铁氧体,其中M是二价金属,钴处理的铁氧体,或具有一般式MO.6Fe2O3的磁铁铅矿型(M型)氧化物,其中M为大的二价离子。
8.根据权利要求7的方法,其中磁性材料是钴处理的γ-氧化铁化合物中的一种。
9.根据权利要求7的方法,其中磁性材料是未经处理的γ-氧化铁化合物中的一种。
10.根据权利要求7的方法,其中磁性材料是二氧化铬类化合物中的一种。
11.根据权利要求1的方法,其中所用的磁性材料在输送入患者体内之前混合在一种生物相容性乳液中。
12.根据权利要求1的方法,其中使用的磁性材料结合于一种用于形成微囊的基质中。
13.根据权利要求12的方法,其中微囊结合、吸附或包含有一种细胞毒物质,此细胞毒物质在微囊加热时被释放出来。
14.根据权利要求12的方法,其中将电离辐射与磁场共同作用于病变组织部位。
15.根据权利要求14的方法,其中辐射由包含有放射活性化合物的微囊输送到体内。
16.根据权利要求1的方法,其中肿瘤加热速率大于60mW/cm3。
17.根据权利要求15的方法,其中肿瘤加热速率大于80mW/cm3。
18.根据权利要求15的方法,其中肿瘤加热速率大于100mW/cm3。
19.根据权利要求1的方法,其中线性交变磁场具有10kHz到100MHz的操作频率。
20.根据权利要求1的方法,其中线性交变磁场具有10kHz到500kHz的操作频率。
21.根据权利要求1的方法,其中线性交变磁场具有10kHz到100kHz的操作频率。
22.根据权利要求1的方法,其中磁场具有20kHz的操作频率。
23.根据权利要求12的方法,其中微囊具有适当的尺寸以至能通过患者的血管网络并在病变组织中分散和栓塞。
24.根据权利要求12的方法,其中微囊密度在1到5g/cm3之间。
25.根据权利要求12的方法,其中微囊密度在1.8到3g/cm3之间。
26.根据权利要求12的方法,其中微囊密度在1.8到2.2g/cm3之间。
27.根据权利要求12的方法,其中微囊密度为约2g/cm3。
28.根据权利要求1的方法,其中磁性材料用(R)-3-羟基丁酸和(R)-3-羟基戊酸的共聚物结合在一起。
29.根据权利要求1的方法,其中磁性材料用(R)-3-羟基丁酸和(R)-3-羟基戊酸的共聚物结合在一起,其密度在1.8~2.2g/cm3的范围,其尺寸在20~50微米的范围。
30.根据权利要求1的方法,其中从肿瘤内、瘤周围或血管内给药等方法中选出任何一种给药方法将磁性材料输送到病变组织。
31.根据权利要求1的方法,其中通过动脉或静脉血液供应将磁性材料输送至病变组织。
32.根据权利要求1的方法,其中将磁性材料与至少一种血管活性药物联合输送到病变组织。
