Abstract

Description
本发明涉及一种由磁性标签控制泵流量和泵吸间隔的医用泵。这种标签可直观地改变各种人工控制器上的刻度,并通过泵中的霍尔效应(Hau Effect)感应装置监测的磁性件位置向泵体发出监测值,该泵体对各种与所需医药产品输注有关的因素都加以考虑。
由人工调控装置,微处理机和电气装置控制输注参数的医用泵在现有技术中已经是众所周知的了。如,美国专利3,701,345和4,320,757公开了有电气驱动系统的装置,这种装置用于向病人注射数量得到控制的液体。但是,这些现有技术要求使用者掌握多种变量,如溶剂中的药品浓度,药品输注速度,病人的体重,根据总的或单位的重量或质量向病人注射所需的药品总量,甚至要知道注射器中注塞的截面积以便计算注射器管中的注塞的所需的速度和总位移量。此外,使用者甚至还要掌握所有这些变量的单位以免出现计算错误。任何一个计算差错都可能对病人造成灾难性的后果。
现有技术使用了有对应于控制板的可程序化的部件的注射系统,这类现有技术包括美国专利4,529,401;4,624,661;4,681,563和4,722,734。这些现有技术仍然要求技术水平高的医务人员进行若干操作,有些医务人员也许会发现这种计算机似的装置不好掌握。此外,由于在许多临床情况下,医务人员的疲劳和紧张节奏使得使用这些设备所需的精确控制难以进行。还有,操作这些设备的困难由于其它一些因素变得更加复杂。如注射开始后医务人员意识到要调整药量以适应病人对正在注射的药的代谢功能,耐受性和过敏反应,等等。
美国专利4,741,732使用了一种复杂的程序控制方案使药品的输注率分布统一起来以此维持在病人体内药品的浓度不变。但这种方案仍然有上段中列出的缺陷,这种方案要求人工输入特定药品的浓度、病人体重。注射管中注塞的截面积等所需的全部数据,这对于疲劳和紧张的医务人员来说是很容易出错的。这种方案由于使用了EPROM,即电子编程只读存储器,使上述的问题仅得到部分解决。这种程序包括与所需的随时间变化的输注率分布的一般特点有关的指令和/或机器数据式目标语言形式。即使使用电子编程只读存储器,由于这种装置复杂,在给病人注射给定药量时,它提供的与其说是简单的“使用者之友”式的直观可检验的控制装置,还不如说是复杂的随时间变化的输注率分布。
本发明的目的之一是提供一种装置可以向病人输注范围广泛的麻醉剂,肌肉松弛剂和血管活化药物,而且与输注有关的各种参数由使用者编程输入装置中。
本发明的另一个目的是输注参数的程序编制既简单又容易。
本发明的另一个目的是输注参数的程序编制易于直观检查。
本发明的另一个目的是使用者在初始程序编制之后还能够手动调整输注率,以适应病人对输注药物的代谢功能,耐受性和过敏反应。
本发明的再一个目的是这种设备适用于广泛的输注范围。
本发明的控制装置包括三个与输注参数有关的调节钮。第一个调节钮是根据所需药物输注率而设置的;第二调节钮是根据病人的体重而设置的;第三个调节钮是根据药量,或者病人单位体重所需的总药量而设置的。微处理机通过这些数据计算出所需的输注流速,注射器注塞速度,总药量和药量持续时间。
第四个调节钮是用来调制工作方式的,如“清洗”(Purge)“关”(Off)“停/确认”Stop/confirm)“药量启始”bolus start)“输注”(infuse)。五位数字液晶显示器和若干发光二极管显示工作方式和输注的总药量。
为了使本装置能适应不同浓度的各种药物的输注和输注率的范围,每种药品包装上都有不同的磁性标签。这些标签标明药物的名称和浓度。这些标签可贴在泵控制装置的前面板上和调节钮周围,这样便可以调整所需输注率,药量,可能的话还有病人体重调节钮的直观刻度。新的药品参数和调整的刻度被恰当地予以考虑,磁性装置被安置在标签预定的部位内。这些磁性装置的位置和磁极与上述的参数和刻度有关,并受泵控制装置中的霍尔效应感应器监测,泵控制装置内的微处理机利用调节钮根据霍尔效应感应器发出的信息设置的位置来确定所需要的输注流量,注塞速度,总药量和药量持续时间。

现详细参阅附图,在所有的附图中相同的数字表示相同的部件。图1显示了设备10,设备10的前面板12有4个调节钮,输注率调节钮14;体重调节钮16;药量调节钮18;工作方式选择调节钮20。
工作方式选择调节钮20有“关”(off)、“清洗”(Purge)“停/确认”(stop/confirm)、“输注”(infusion)“药量启始”(Bolus start)位置。
双位选择器21用于选择注射器类型。
所推荐的实施例只能使用标准的20立方公分(CC)或60立方公分(CC)的注射器,虽然熟练技术人员会意识到其他位置也可以,或与注射器注塞截面积成正比(或成反比)的刻度因素可以被编入在标签30内已编制的信息。
此外,设备10的前面板12包括一个“输注”(lnfusing)发光二极管22,一个“注意”(Attention)发光二极管24,一个“药量”(Bolusing)发光二极管26和五位数字液晶显示器28。
图1还显示了磁性标签30。磁性标签30包括黑体印刷的药物型号和浓度的字母数码表32,磁性标签30还包括3个孔34,36,38,这些孔由刻度40,42,44分别环绕,分别表示输注率,体重和药量。用于输注率调节钮14的孔34有刻度40,刻度的单位是按每分钟,每公斤,病人体重(或重量)的药物的毫克计算。这也可以用每分钟,每公斤病人体重药物的毫升或者药物毫克,或者药物毫升来代替。毫升或毫克指在注射器11中的药液所含的有效药物,不是指药液的总量或重量。
用于病人体重(或重量)调节钮16的孔36有刻度42,刻度以公斤为单位。调节钮16只有在输注率和药量单位以病人单位体重为基础时才起作用。这在下面还要详述。
用于药量调节钮18的孔38有刻度44,刻度的单位是按每公斤病人体重输注药物的总毫克量。这也可以用每公斤病人体重药物毫升,药物毫克,或者药物毫升来代替。毫升或毫克指在注射器11中的药液所含有的有效药物,不是指药液的总量或重量。
刻度40,42和44范围的选择要适应药物类型和浓度,这在字母数字表32中有说明。此外,刻度的单位,特别是输注率和药量(单位可以用毫升或毫克,以病人体重的总重或单位重量为基础,是根据与特定的药物和药液有关的单位而选择的。如果输注率和药量的单位是以总体重为基础而不是以病人单位体重计算的,体重刻度42便标有“不起作用(Inactive)。
同样,对于一些“输注”(Infusing)方式,与“药量”方式相反,体重或药量刻度42,44都是无关紧要的,在此情况下药物以一定的速度在不确定的时间中输注,这些刻度调节到低输注刻度,同时输注率刻度40调节到高输注刻度。操作者将其中一个输注率刻度调到零,而将另一个调到所希望的高输注率或低输注率,这在下边还要详述。
前面板12或其它与设备10关系密切结构部件有铁制的或类似部件以便标签30可磁性固定到设备10上。
图2显示了设备10的前部,从这个角度描述了用夹子13将注射器11固定在设备10上。注塞驱动器23与注射器11的注塞连接,通过驱动装置70(见附图4)以机械方式驱动注塞驱动器23将药物排出。泵设备10与注射器11的连接,以及驱动装置70的机械部件一九八七年八月二十四日申请,并在一九八八年九月二日批准为专利的申请号为088,806的申请中已公开。本专业的技术人员会意识到固定注射器11有若干种不同的方法,但都是在本发明范围之内的。
磁性标签30,除整个标签都有磁性外,在其后部还有孔46,48和50,强钐钴(稀土族)磁性部件可插进上述孔中,如图3所示。图3说明磁性件51和52放置在孔46和50中。在孔46,48和50中的磁性件51和52的位置和磁极与药品浓度(如字母数码表32所示)有关,也与对于补偿前面板12上的调节钮14、16、18周围的刻度表和磁性标签30上的刻度40,42和44之间变化所必要的调整有关,与此有关的还有熟练技术人员认为正确计算所需的输注率,注塞速度和总药量及药的持续时间的所有其他参数。

位置和磁板的组合共12种。(三个因素之一是以三个孔中选择两个孔,其余两个因素,各是一个磁极选择)。本专业技术人员可以意识到,在不脱离本发明范围的情况下;多几个位置或磁强度水平还可以取得更多的组合。本专业技术人员还会意识到,磁性件的单一组合可以满足上面提到的参数的一个以上的组合。如:在其他因素都一样的条件下,通过调整磁性标签30上的不同刻度,不用改变孔46,48和50中磁性件51和52的组合,便可补偿药物的不同浓度变化。
对磁性标签30的尺寸和孔34,36和38的直径和间距的选择使磁性标签30可以安置在前面板12上,如图1所示,从而可以改变与调节钮14,16和18有直观联系的刻度。
本专业技术人员还可意识到标签30和设备10之间的连系还有其他方式,其中包括:
1.插头式连接器方式
插头式连接器可以在标签固定的计算机插板上固定和编码。
2.光电方式

图4公开了设备10内部电子线路的框图。如图1所示当磁性标签30与装置10相连时,霍尔效应感应器54,55,56,57,58,59即被置于孔46,48和50附近的前面板的内部。每个孔有两个感应器与其相连;一个测北极,一个测南极。霍尔感应器54,55,56,57,58和59中均有运行电流流过。当感应器被置于-磁场下,且磁场与插进孔46,48和50中的磁性件

有关药物输注率调节钮14,体重调节钮16和药量调节钮18所处位置的数据经旋转式开关68,70,72和并/串行转换器60输至微处理机66。有关操作方式选择调节钮20位置的数据输至微处理机66,操作方式选择调节钮20安装在电源62和微处理机66之间。微处理机66计算出所需的输注率做为如下因素的结果:
1.由药物输注率调节钮14所显示的输注率;
2.由体重调节钮16所显示的病人的体重;
3.把药物输注率调节钮14的位置转换成由磁性标签30的药量刻度40显示的输注率的刻度因素;
4.把患者体重调节钮16的位置转换成由磁性标签30的体重刻度42显示的患者体重的刻度因素;
5.由磁性标签30的字母数码表显示的药品浓度的量反比;
如果使用中输注率是以绝对重量而不是单位重量为基础来计算,那么计算时可以剔除上述第2-4个因子。
可以由输注分流来计算注塞速度,通过:

当在“输注”方式工作时,仅注塞速度需要计算。但是,当在“药量”方式工作时,下面列出的值需要计算。微处理机66计算出所需的泵时做为如下因素的结果。
7.药量调节钮18的位置标明的药量;
8.输注率调节钮14的位置标明输注率的反比。这是因素1的反比;
9.将药量调节钮18的位置变为磁性标签30的刻度44标明的药量的刻度因素;
10.将输注率调节钮14的位置标明的输注率的反比变为磁性标签30的刻度40上标明的输注率的反比的刻度因素。
总药量是由用药量持续时间乘上输注率计算出的。
熟练技术人员会意识到上述计算过程可以有不同顺序,其它一些因素也许对某些应用是必不可少的。

使用者设定患者体重调节钮16,输注率调节钮14,并将操作方式选择钮20置于“输注”状态。微处理机60将选择一注塞速度,但并不将输注限制在一定时间或数量范围内。“输注”发光二极管LED22将闪亮,数字液晶显示器28将以增量方式显示已输注的药物量。当使用者将输注率调节钮14置于“0”时,和将操作方式选择调节钮20置于“停/确认”位置时,或当注射器11空了时,输注便会停止。
当要装药或追加药量时,使用者可在药量调节钮18上设定出所需的药量值,并将操作方式选择调节钮20置于“药量启始”状态。当声频驱动器67发出简短的“嘟嘟声”时,使用者放开调节钮20,便处于快速装药状态。“药量”发光二极管26将不断闪动以显示正在正常装药。液晶显示器28的数字将累计计数(以患者体重每公斤为基础),来显示目前的药量状况。如终止装药。使用者可将操作方式选择调节钮20转至“停/确认”位置。一旦开始装药,重新置患者体重选择钮16或药量调节钮18,将不会对装入的药量产生影响。
2.根据与病人体重无关的高、低速率进行输注。
根据所需的高速率或低速率,将其它选择钮置于所需的值。然后使用者将操作方式选择调节钮20置于“输注”状态。“输注”发光二极管22将闪动,液晶显示器28以累计方式显示输注的药量。如想改变输注速率,使用者可将一个注速率由调节钮置于新的位置。如果想停止输注,使用者可将二个输注调节钮置于“0”或者将操作方式选择调节钮20置于“停/确认”状态。
3.根据依患者体重确定的高,低速率进行输注。
在此方式下,输注率刻度40和药量刻度42分别由高输注率刻度和低输注率刻度所取代。使用者可根据采用高输注率或低输注率,将上述调节钮置于0,并将另外的调节钮置于所需的值。使用者在患者体重调节钮14上调至患者体重值,然后使用者将操作方式选择调节钮20置于“输注”状态。“输注”发光二极管22将闪烁,液晶显示器28开始以累计方式显示已输注的药量,要改变输注率,使用者可将一个输注率调节钮置于新的位置,要中止输注,使用者可将二个输注率调节钮置于0,或将操作方式选择调节钮20置于“停/确认”状态。
设备10有如下报警显示装置:
1.注射终止或封闭。这种状态由声频驱动器67发出的脉冲声响报警和发光二极管24“注意”闪亮表示。液晶显示器28将显示是那一种状态存在。
2.电池不足。这种状态由发光二极管24“注意”闪亮表示,并不启动声频驱动器67。液晶显示器28将显示这种状态存在。
3.内部故障。这种状态由声频驱动器67和全部发光二极管22,24和26同时闪亮表示。
4.旋钮位置不正常。如果某一个旋钮或调节钮的位置停留或被置于实际选择位置之间的一个位置超过 1/2 秒,设备10便由声频驱动器67发出三声短促的“嘟,嘟,嘟”声响报警。但设备10按原先选择的输注率工作。将旋钮调到正常位置时,报警便停止。
这样,上述的若干目的和优点便会非常有效地达到和表示出来,尽管这里只对涉及本发明的一个推荐实施例进行了公开和详细的描述,但应该理解本发明绝不受限制,本发明的范围将由权利要求书限定。
Claims (15)
1、一种装置包括
泵装置,
控制上述泵装置工作的装置,
显示装置,该装置包括用于选择泵工作方式而预先设置的组合信息,其中,当上述显示装置被安置在靠近上述控制装置时,上述控制装置感应上述预先设置的组合信息,上述控制装置根据上述预先设置的组合信息控制上述泵装置。
2、按照权利要求1的装置,其中上述信息由磁性件的放置位置而编成程序码。
3、按照权利要求书2的装置,其中控制上述泵工作的上述装置1还包括手动控制。
4、按照权利要求3的装置,其中上述显示装置包括当上述显示装置被安置在靠近上述控制装置时所用于上述手动控制的直观刻度,上述直观刻度与上述预定组合相一致,上述控制装置根据直观刻度使用上述预先设置的组合来调整计算。
5、按照权利要求4的装置,其中上述显示装置被磁性装置固定在靠近上述控制装置的位置。
6、按照权利要求5的装置,其中上述控制装置还包括感应上述永久磁性件的预先设置的组合的霍尔效应传感器。
7、按照权利要求6的装置,其中上述霍尔效应传感器感应在上述预先设置的组合中上述永久磁性件的磁极。
8、按照权利要求7的装置,其中上述泵装置驱动医药注射器。
9、按照权利要求8的装置,其中上述控制装置包括微处理机。
10、按照权利要求8的装置,其中只有在预定数目的上述永久磁性件在上述预定的组合时,上述控制装置才允许上述泵装置工作。
11、按照权利要求8的装置,其中上述泵装置能以衡定流速运转。
12、按照权利要求8的装置,其中上述泵装置能在设定时间内以衡定流速运转。
13、按照权利要求8的装置,其中该装置还包括显示上述装置工作方式的灯。
14、按照权利要求8的装置,其中上述装置还包括显示从上述泵装置流出的总流量的多功能显示器。
15、按照权利要求12的装置,其中上述手动控制包括病人体重控制钮,上述设定时间由上述病人体重控制钮的调整而决定。
