Abstract

背景技术
在一些个体中,葡萄糖或其他分析物如乳酸盐、氧等的水平的检测对于他们的健康而言是至关重要的。例如,葡萄糖的监测对于患糖尿病的个体来说尤其重要。糖尿病患者可能需要监测葡萄糖水平来确定何时需要胰岛素以降低他们体内的葡萄糖水平或确定何时需要额外的葡萄糖以提高他们体内的葡萄糖水平。
已经开发了用于连续或自动监测血流或间质液的体液中的分析物(如葡萄糖)的装置。其中一些分析物测量装置被配置为使得将这些装置的至少一部分置于用户的皮肤表面之下,例如,置于用户的血管或皮下组织中。
插入传感器例如传感器导引器和/或传感器本身后,对皮肤和/或下方组织所造成的可能创伤有时会导致该传感器所监测信号的不稳定。这会在多种分析物传感器中发生,但并非所有情况。这种不稳定的特征在于传感器信号的降低,并且当发生这种情况时,通常不会向用户报告、记录或输出所监测的分析物水平。
对于准确的传感器性能来说,用参比葡萄糖度量或读数对分析物传感器进行适当校准是很重要的。校准是通过确定转换因子(或灵敏度)并以最简单的形式将其表示为分析物传感器所产生的电流信号与参比血糖值(例如,来自体外血糖计)的比值,所述参比血糖值随时间(例如,相对的时间对应)与分析物传感器的电流信号相关。理想地,当以与用户的分析物(如间质液)流体接触的方式安置时,在该分析物传感器的整个使用寿命中灵敏度是恒定的。然而,在实践中,灵敏度会随时间而改变。已经观察到通常在从插入或安置分析物传感器算起的预定时间段后发生灵敏度降低或衰减,对于一些分析物传感器来说有时会长达24小时。这种信号特征被称作早期灵敏度衰减(ESA)或被称作ESA状况。ESA状况可能是由对分析物传感器引入皮下组织的生理反应所引起的,并且可能存在于任何皮下插入的分析物传感器中。
通常,使用标准的校准灵敏度计算并不能解决信号衰减的问题。典型的标准校准并不检测或控制衰减的信号特征,并且还会使用错误和衰减的传感器信号来修正或改变校准灵敏度。若在传感器经历信号衰减时进行传感器校准,在信号衰减事件终止后传感器灵敏度恢复时,所报告或所得的传感器数据可能是错误的较高数值。由于可能会导致低血糖事件的漏检或导致如胰岛素的药物疗法的剂量过量,因此这样的较高的偏差结果在临床上可能是不安全的。另一方面,若在早期信号衰减事件之前进行传感器校准,则在传感器灵敏度降低期间很可能会产生错误的偏低的传感器数据。根据早期信号衰减事件的幅度,这样的低葡萄糖结果会导致假低血糖报警或漏检高血糖事件。
另一种方法是将传感器校准从例如在患者中初始插入传感器延迟到早期信号衰减阶段之后。然而,虽然这种方法防止了在此期间报告传感器所监测的可能的错误分析物水平,但是,不论是否存在早期信号衰减,由于不希望延迟显示或报告传感器所监测的分析物水平,因此导致了较低的数据得率。
发明内容
本发明公开的实施方式包括检测与分析物传感器有关的灵敏度变化,如降低(或监测信号水平变化)以鉴别或检测早期信号衰减(ESA)的装置和方法。当所检测到的变化与早期信号衰减无关时,在短暂的传感器平衡期后(例如,约1小时或约1小时左右),报告或输出所检测或监测到的分析物水平。
本发明还提供了系统、计算机程序产品和试剂盒。

具体实施方式
在本发明公开的范围内,早期信号衰减(ESA)状况可以归因于在用户皮层以下安置传感器时由皮肤和/或组织创伤所造成的监测的分析物水平的相关不稳定性。可以制造分析物传感器和/或由插入的传感器所造成的外伤可以使传感器在相对短暂的时间段后达到稳定点或平衡水平,例如,从传感器初始插入算起的约1小时内(或更短的时间)。
一方面,可以监测来自分析物传感器的信号以用于ESA状况检测。当未检测到ESA状况和/或传感器在短时间段内达到平衡水平时,则会配置分析物监测系统以向用户请求参比血糖值,例如,使用血糖计进行手指针刺体外测试,从而校准传感器信号,并且随后向用户报告或显示所监测到的分析物水平。一方面,可以以这种方式在从传感器初始插入算起的约1小时后进行分析物传感器的初始基线校准,并且在成功校准后,向用户显示所得的实时分析物水平,或者否则将其存储或记录到分析物监测系统中和/或将其传输到远程设备或终端。
例如,当在例如从传感器插入算起的约1小时的初始平衡阶段后检测到了ESA状况的潜在可能性或实际的ESA状况时,则可以配置分析物监测系统以提醒用户在提供参比血糖值前等待预定时间段以使得传感器稳定,或者作为另外一种选择,可以促使用户提供参比血糖值以确认所监测的可能ESA状况是否是实际发生的ESA状况。
一方面,可以延迟排定的分析物传感器校准以为传感器提供额外的时间段从而使其达到所需的或可接受的稳定性水平。在其它条件中,可以建立边界值用以为传感器提供额外的时间从而使其在校准所接收的分析物传感器信号前达到预定的或可接受的稳定性水平,并且以此提供给用户。在本发明公开的范围内,可以提供其它条件和参数从而在从传感器初始插入算起的预定时间段内建立或检测ESA状况,例如,在传感器插入的第一个24小时内。
一方面,当确定皮下安置的传感器已以这种方式达到所需或预定平衡水平的可接受稳定性水平时,分析物监测系统就会基本上以实时的方式显示或接受、输出、记录或处理从皮下安置的传感器所接收的监测的分析物水平。一方面,在从传感器初始插入算起约1小时的时候分析该可接受的稳定性水平,并且随后,当未检测到ESA状况时,对比参比血糖值(例如,从体外葡萄糖计接收的数据)来校准分析物传感器数据。
在检测到ESA状况或这种信号衰减的可能性的情况下,在一种实施方式中,可以配置分析物监测系统以进行一个或多个例行程序或功能来验证传感器相关信号从而确认ESA状况,尤其是通知用户尽量不要使用血糖计进行手指针刺测试来提供用于校准的参比血糖值。

通常,本发明公开的实施方式涉及检测体液中诸如葡萄糖的至少一种分析物的方法和装置。在一些实施方式中,本发明公开的内容涉及在一段时间内使用包含至少部分置于用户皮肤表面下方的分析物传感器的连续分析物监测系统对一种或多种分析物水平进行连续和/或自动体内监测和/或涉及使用体外血糖(“BG”)计和分析物测试条对一种或多种分析物进行不连续监测。实施方式包括组合的或可组合的装置、系统和方法和/或在体内连续系统和BG计系统之间的数据传输。
因此,实施方式包括分析物监测装置和系统,其包含用于体液中的诸如葡萄糖、乳酸盐等之类的分析物体内检测的分析物传感器,该传感器可至少部分置于用户皮肤之下。实施方式包括可完全植入的分析物传感器以及仅有部分传感器置于皮肤下而部分传感器留在皮肤上的分析物传感器,其中留在皮肤上的部分传感器用于例如与发射器、接收器、收发器、处理器等连接。例如,这些传感器可以皮下安置在患者体内用于对患者间质液中的分析物水平进行连续或周期性监测。出于说明的目的,除非另外注明,否则连续监测和周期性监测将可替换地使用。传感器的响应可以与血液或其它液体中的分析物水平相关联或转化为血液或其它液体中的分析物水平。在一些实施方式中,可以安置分析物传感器使其与间质液接触以检测葡萄糖水平,所检测到的葡萄糖可以用于推断患者血流中的葡萄糖水平。可以将分析物传感器插入到静脉、动脉或含有流体的其它身体部分。可以配置本发明公开的分析物传感器的实施方式以用于监测一段时间内的分析物水平,所述时间段可以在分钟、小时、天、周或更长的范围内。
所关心的是例如葡萄糖传感器的分析物传感器,所述传感器能够对分析物进行约1小时或更长时间的体内检测,例如,约几小时或更长时间,例如,约几天的更长时间,例如,约三天或更长时间,例如,约五天或更长时间,例如,约七天或更长时间,例如,约几周或至少一个月。基于所获得的信息,例如,时间t0时的当前分析物水平、分析物的变化率等,可以预测未来的分析物水平。预测性报警可以在分析物水平达到未来水平之前通知用户可能要注意预测分析物水平。这能够为用户提供采取纠正措施的机会。
图1示出了数据监测和控制系统,例如,根据一些实施方式的分析物(例如,葡萄糖)监测系统100。仅为了方便起见,主要就葡萄糖监测装置和系统以及葡萄糖检测方法进一步说明本发明公开的实施方式,并且该说明绝不是意欲限制本发明公开的范围。应理解可以配置分析物监测系统以在相同或不同的时间监测多种分析物。
可以监测的分析物包括,但不限于,乙酰胆碱、淀粉酶、胆红素、胆固醇、绒毛膜促性腺激素、肌酸激酶(例如,CK-MB)、肌酸、肌酸酐、DNA、果糖胺、葡萄糖、谷氨酰胺、生长激素、激素、酮体、乳酸盐、过氧化物、前列腺特异性抗原、凝血酶原、RNA、促甲状腺激素、和肌钙蛋白。还可以监测药物的浓度,该药物例如是抗生素(例如,庆大霉素、万古霉素等)、洋地黄毒苷、地高辛、滥用的药物、茶碱和华法林(warfarin)。在监测不止一种分析物的那些实施方式中,可以在相同或不同的时间监测分析物。
分析物监测系统100包括传感器101、可以与传感器101连接的数据处理单元102和第一接收器单元104,第一接收器单元104被配置为通过通信链路103与数据处理单元102进行通信。在一些实施方式中,还可以配置第一接收器单元104将数据传输到数据处理终端105以评价或另外处理或格式化第一接收器单元104所接收的数据。可以配置数据处理终端105以通过可以任选地配置用于双向通信的通信链路直接从数据处理单元102接收数据。另外,数据处理单元102可以包括发射器或收发器以传输和/或接收数据至第一接收器单元104和/或数据处理终端105和/或可选的第二接收器单元106,和/或从第一接收器单元104和/或数据处理终端105和/或可选的第二接收机单元106传输和/或接收数据。

在图1所示的分析物监测系统100的实施方式中,仅显示了一个传感器101、数据处理单元102和数据处理终端105。然而,本领域的普通技术人员将理解分析物监测系统100可以包括不止一个传感器101和/或不止一个数据处理单元102和/或不止一个数据处理终端105。可以在患者体内安置多个传感器已用于在相同或不同的时间对分析物进行监测。在一些实施方式中,可以将由第一安置的传感器获得的分析物信息用于与由第二传感器获得的分析物信息进行比较。这可能对确认或验证从一个或两个传感器所获得的分析物信息有用。如果在关键的疗法相关决定中考虑分析物信息,那么这种重复可以是有用的。在一些实施方式中,第一传感器可以用于校准第二传感器。
分析物监测系统100可以是连续监测系统,或半连续或不连续监测系统。在多组件环境中,可以配置每个组件以使其能够被系统中的一个或多个其它组件唯一地识别从而可以容易地解决分析物监测系统100内各个组件之间的通信冲突。例如,可以使用唯一的ID、通信通道等。
在一些实施方式中,将传感器101物理安置到正在监测分析物水平的用户的体内或体表。可以配置传感器101从而至少周期性地对用户的分析物水平进行采样,并且将所采集的分析物水平转换成数据处理单元102所传输的相应信号。数据处理单元102可与传感器101连接从而使得两个装置均安置在用户的体内或体表,并且至少部分分析物传感器101为经皮安置的。数据处理单元102可以包含如粘附层等之类的固定元件以将其固定在用户身体上。可以使用可与用户连接并且可与单元102配合的支架(mount)(未示出)。例如,该支架可以包含粘附表面。数据处理单元102进行数据处理功能,其中这些功能可以包括,但不限于,数据信号的过滤和编码,其中每个信号对应于所采集的用户分析物水平从而用于通过通信链路103传输至第一接收器单元104。在一种实施方式中,可以将传感器101或数据处理单元102或结合的传感器/数据处理单元完全植入到用户皮层之下。

在一些实施方式中,数据处理单元包括RF发射器206,其同样可操作地与处理器204连接。在一些实施方式中,可以将RF发射器206配置为RF接收器或RF发射器/接收器,如收发器。此外,还可以在数据处理单元102中设置如电池的电源207以向数据处理电源102提供必要的电能。另外,从图中可以看出,可以另外设置时钟208从而向处理器204提供实时信息。
从图2的实施方式中可以看出,传感器单元101(图1)包括四个触点,其中三个为电极-工作电极(W)210、保护触点(G)211、参比电极(R)212和反电极(C)213,其中每个均可操作地与数据处理单元102的模拟接口201连接。在一些实施方式中,可以使用导电材料制成每个工作电极(W)210、保护触点(G)211、参比电极(R)212和反电极(C)213,所述导电材料可以通过,例如,化学汽相沉积(CVD)、物理汽相沉积、溅射、反应性溅射、印刷、涂覆、烧蚀(例如,激光烧蚀)、涂抹、浸渍涂覆、蚀刻等方式应用。材料包括,但不限于,铝、碳(如石墨)、钴、铜、镓、金、铟、铱、铁、铅、镁、汞(作为汞齐)、镍、铌、锇、钯、铂、铼、铑、硒、硅(例如,掺杂多晶硅)、银、钽、锡、钛、钨、铀、钒、锌、锆、它们的混合物、以及这些元素的合金、氧化物或金属化合物。
可以配置处理器204从而在数据处理单元102运行期间产生和/或处理数据处理单元102的各个部分的控制信号。在一些实施方式中,处理器204还包括存储器(未示出),其用于存储如数据处理单元102的识别信息的数据以及与从传感器101所接收的信号有关的数据。可以检索和处理所述存储的信息以用于在处理器204的控制下传输到第一接收器单元104。此外,电源207可以包括可商购的电池。
在一些实施方式中,数据处理单元102的生产方法可以使数据处理单元102处于低功率的非运行状态(即,生产后休眠模式)。以这种方式,可以显著改善数据处理单元102的存放寿命。此外,如图2所示,尽管所显示的电源单元207是与处理器204相连接的,并且因此,配置了处理器204以提供对电源单元207的控制,但是应注意在本发明公开的范围内,电源单元207的配置是用于向图2中所示的数据处理单元102的每个组件提供必要的电能。
再参考图2,在一种实施方式中,数据处理单元102的电源部分207可以包括可通过单独的电源充电单元(例如,接收器单元104中所设置的)充电的可充电电池单元,从而可以为数据处理单元102提供较长使用时间的电能。在一些实施方式中,可以将数据处理单元102配置为电源部分207中不包括电池,在这种情况下,可以配置数据处理单元102以接收来自外部电源(例如,电池、插座等)的电能,如以下进一步详细讨论的。
再参考图2,数据处理单元102的温度检测部分203被配置为监测传感器插入位置附近皮肤的温度。可以使用温度读数来调节从模拟接口201所获得的分析物读数。还显示了与防护迹线(guard trace)(G)211以及数据监测和控制系统100的数据处理单元102中的处理器204相连接的漏电检测电路214。可以配置漏电检查电路214来检测传感器101中的泄漏电流从而确定所测量的传感器数据是否损坏或者来自传感器101的测量的数据是否准确。这种检测会向用户通知。
图3示出了图1中所示的数据监测和控制系统的接收器/显示器单元(如第一接收器单元104)的实施方式的框图。第一接收器单元104包括以下之中的一个或多个:血糖测试条接口301、RF接收器302、输入端303、温度监测部分304和时钟305,它们中的每一个可操作地与处理和存储部分307连接。第一接收器单元104还包含与电源转换和监测部分308可操作地连接的电源306。另外,电源转换和监测部分308还与接收器处理和存储部分307相连接。此外,还示出了接收器串行通信部分309和输出310,它们分别与处理和存储单元307可操作地连接。接收器可以包括用户输入和/或接口组件或者可以不包括用户输入和/或接口组件。
在一些实施方式中,测试条接口301包括葡萄糖水平测试部分以接收血液(或其它体液样本)葡萄糖测试信息或其相关信息。例如,该接口可以包含测试条端口以容纳葡萄糖测试条。该装置可以确定测试条的葡萄糖水平,并可选地在第一接收器单元104的输出端310上显示(或提醒)葡萄糖水平。可以使用任何适合的测试条,例如,为了获得准确的葡萄糖信息,仅需要将很少量(例如,1微升或更少,例如,0.5微升或更少,例如,0.1微升或更小)的样品施加到测试条上的测试条,例如,来自Abbott Diabetes Care Inc.的血糖测试条。可以将通过体外葡萄糖测试装置获得的葡萄糖信息用于多种目的、计算等。例如,该信息可以用于校准传感器101,确认传感器101的结果以提高其可信度(例如,在疗法相关决定中使用通过传感器101获得的信息的情况下)等。
一方面,配置RF接收器302以通过通信链路103(图1)与数据处理单元102的RF发射器206通信以接收来自数据处理单元102的编码数据从而用于,尤其是,信号混合、解调以及其它数据处理。配置第一接收器单元104的输入端303以允许用户根据需要将信息输入到第一接收器单元104中。一方面,输入端303可以包括小键盘的按键、触敏屏和/或语音激活的输入命令单元等。可以配置温度监测部分304以向处理和控制部分307提供第一接收器单元104的温度信息,而时钟305另外向处理和存储部分307提供实时或时钟信息。
图3中所示的第一接收器单元104的各个组件中的每一个均由电源306(或其他电源)提供电能,在一些实施方式中,电源306包括电池。此外,配置了电源转换和监测部分308以监测第一接收器单元104中各个组件的电能使用情况以用于有效控制电源并且它可以在,例如,电能使用使第一接收器单元104处于欠佳运行状况的情况下提醒用户。配置第一接收器单元104中的串行通信部分309以提供从测试和/或生产设备开始的双向通信通路以尤其用于第一接收器单元104的初始化、测试和配置。串行通信部分309还可以用于将数据上传到计算机,如时间标记的血糖数据。可以通过电缆(如USB或串行电缆)、红外(IR)或RF连接制成与外部设备(未显示)之间的通信链路。配置了第一接收器单元104的输出/显示端310以尤其提供图形用户界面(GUI),并且输出/显示端310可以包括用于显示信息的液晶显示器(LCD)。另外,输出/显示端310还可以包括集成的喇叭以输出声音信号,并且提供了在如移动电话、寻呼机等手持电子设备中常见的振动输出。在一些实施方式中,第一接收器单元104还包括场致发光灯,其配置以向黑暗环境中的输出端310的输出可见显示提供背光。
再参考图3,第一接收器单元104还可以包括作为处理和存储部分307的一部分的存储部分,如可编程的永久性存储设备,或者在第一接收器单元104中单独设置,并与处理器可操作地连接。可以配置处理和存储部分307从而在通过通信链路103接收到来自数据处理单元102的编码数据后进行曼彻斯特解码(或其它协议)以及错误检测和修正。
在其它实施方式中,可以配置数据处理单元102和/或第一接收器单元104和/或第二接收器单元106和/或数据处理终端/输液部分105以通过通信链接线路从例如血糖计无线地接收血糖值。在其它实施方式中,用户控制或使用的分析物监测系统100(图1)可以使用,例如,加入到数据处理单元102、第一接收器单元104、第二接收器单元106或数据处理终端/输液部分105的一个或多个中的用户界面(例如,键盘、小键盘、语音命令等)手动输入血糖值。
在美国专利第5262035号;第5264104号;第5262305号;第5320715号;第5593852号;第6175752号;第6650471号;第6746582号和在2003年12月26日提交的名称为“连续葡萄糖监测系统和使用方法(Continuous Glucose Monitoring System and Methods of Use)”的专利申请第10/745878号中提供了其他详细说明,以上每篇专利并入本文作为参考。
图4示出了根据本发明公开的分析物传感器的一种实施方式的示意图。传感器400包括位于基座404上的电极401、402和403。传感器可以完全植入到用户体内或者可以将其配置成仅有一部分置于用户体内(内部)而另一部分位于用户体外(外部)。例如,传感器400可以包括可置于皮肤表面410上的部分和置于皮肤下的部分。在这些实施方式中,所述外部部分可以包括触点(通过布线连接到第二部分的各个电极上)以连接同样位于用户体外的另一装置,如发射器单元。尽管在图4的实施方式中示出了位于基座404的相同表面上的并列布置的三个电极,但是还考虑了其它配置,例如,更少或更多的电极,一些或全部电极位于基座的不同表面上或存在于另一基座上,一些或全部电极堆叠在一起,不同材料和尺寸的电极等。
图5A示出了电化学分析物传感器500的实施方式的透视图,所述传感器具有可置于皮肤表面510上的第一部分(在本实施方式中可以称为主要部分)和包含可置于皮肤下的插入尖端530的第二部分(在本实施方式中可以称为次要部分),例如,所述插入尖端穿透皮肤并进入到,例如,皮下空间520中,从而与用户的生物液体(如间质液)发生接触。将工作电极501、参比电极502和反电极503的触点部分安置到位于皮肤表面510上的传感器500部分上。示出了第二部分以及特别是插入尖段530处的工作电极501、参比电极502和反电极503。如图5A所示,可以设置从尖部的电极至触点的布线。应理解可以在传感器上设置更多或更少的电极。例如,传感器可以包含不止一个工作电极和/或反电极和参比电极可以为一个反电极/参比电极等。
图5B示出了图5A的传感器500的部分的横截面视图。以层状配置或构造来设置传感器500的电极501、502和503以及衬底层和介电层。例如,如图5B所示,一方面,传感器500(如传感器101,图1)包含衬底层504和布置在衬底层504的至少一部分上的第一导电层501(如碳、金等),并且该导电层可以提供工作电极。还示出了布置在第一导电层501的至少一部分上的传感层508。
再参考图5B,将如第一介电层505的第一绝缘层布置或层叠在第一导电层501的至少一部分上,并且此外,可以将第二导电层509布置或堆叠到第一绝缘层(或介电层)505的至少一部分上。如图5B所示,第二导电层509可以设置参比电极502,并且一方面,它可以包含由银/氯化银(Ag/AgCl)、金等构成的层。
再参考图5B,在一种实施方式中,可以将如介电层的第二绝缘层506布置或层叠到第二导电层509的至少一部分上。另外,第三导电层503可以提供反电极503。可以将它布置到第二绝缘层506的至少一部分上。最后,可以将第三绝缘层507布置或层叠到第三导电层503的至少一部分上。以这种方式,可以分层地布置传感器500从而使得每个导电层的至少一部分被各自的绝缘层(例如,介电层)所分开。
图5A和图5B的实施方式示出了具有不同长度的层。一些或全部的层可以具有相同或不同的长度和/或宽度。
在一些实施方式中,可以将电极501、502、503中的一些或全部设置在如上所述的成层构造中的衬底504的相同侧,或者作为另外一种选择,可以以共平面的方式设置从而可以将两个或更多个电极(例如,并列(例如,平行)或彼此相对成一定角度)安置到衬底504的相同平面上。例如,共平面的电极可以包括位于它们之间的适合的间隔和/或包括布置在导电层/电极之间的介电材料或绝缘材料。此外,在一些实施方式中,可以将电极501、502、503中的一个或多个布置到衬底504的相对两侧。在这样的实施方式中,触点垫可以位于衬底的相同或不同侧。例如,电极可以位于第一侧而其各自的接头可以位于第二侧,例如,连接电极和触点的布线可以横穿过该衬底。
在一些实施方式中,可以配置数据处理单元102来进行传感器插入检测和数据质量分析,还可以将与之有关的信息在预定的时间间隔周期性地传输到第一接收器单元104。相反,可以配置接收器单元104来进行,例如,皮肤温度补偿/校正以及对从数据处理单元102接收的传感器数据的校准。
如以上所提到的,分析物传感器可以包括响应于分析物的酶以提供传感组件或者传感层。可以在传感器上对诸如氧的一些分析物直接进行电解氧化或电解还原,并且更具体地,至少在传感器的工作电极上进行。如葡萄糖和乳酸盐之类的其它分析物需要存在至少一种电子传递试剂和/或至少一种催化剂以辅助分析物的电解氧化或电解还原。催化剂也可以用于可在工作电极上直接进行电解氧化或电解还原的那些分析物,例如氧。对于这些分析物,每个工作电极包括在工作电极表面附近或表面上形成的传感层(参见,例如,图5B的传感层508)。在多个实施方式中,仅至少在工作电极的较小的部分附近或部分上形成传感层。
可以使用多种不同的传感层配置。在一些实施方式中,将传感层沉积在工作电极的导电材料上。传感层可以延伸到工作电极的导电材料之外。在一些情况下,传感层还可以延伸到其它电极之外,例如,延伸到反电极和/或参比电极之外(或设置了反/参比电极)。传感层可以与电极材料整合。
直接与工作电极接触的传感层可以含有电子传递试剂以在分析物和工作电极之间直接或间接传递电子,和/或含有催化剂以辅助分析物的反应。例如,可以形成具有传感层的葡萄糖、乳酸盐或氧电极,所述传感层分别包含如葡萄糖氧化酶、乳酸盐氧化酶或漆酶的催化剂以及分别辅助葡萄糖、乳酸盐或氧电解氧化的电子传递试剂。
在包括多于一个工作电极的一些实施方式中,一个或多个所述工作电极不具有相应的传感层,或者具有不包含电解所述分析物所需的一个或多个组分(例如,电子传递试剂和/或催化剂)的传感层。因此,该工作电极的信号对应于背景信号,通过例如扣除该背景信号,可以从得自与功能完全的传感层有关的一个或多个其它工作电极的分析物信号中除去该背景信号。
在一些实施方式中,所述传感层包含一种或多种电子传递试剂。可以使用的电子传递试剂为电解还原和电解氧化离子或分子,其具有比标准甘汞电极(SCE)的氧化还原电势高或低几百毫伏的氧化还原电势。所述电子传递试剂可以是有机的、有机金属的或无机的。有机氧化还原物质的实例为醌和处于其氧化状态的具有醌结构的物质,如尼罗蓝(Nile blue)和靛酚。有机金属氧化还原物质的实例为茂合金属,如二茂铁。无机氧化还原物质的实例为铁氰化物(III)、钌六胺等。
在一些实施方式中,电子传递试剂具有防止或显著降低样品分析期间所述电子传递试剂扩散损失的结构或电荷。例如,电子传递试剂包括,但不限于,氧化还原物质,例如,与聚合物结合的氧化还原物质,所述聚合物反过来又可以布置到工作电极上或附近。所述氧化还原物质与所述聚合物之间的键可以是共价键、配位键或离子键。尽管任何有机、有机金属或无机氧化还原物质均可以与聚合物结合并用作电子传递试剂,但是在一些实施方式中,所述氧化还原物质为过渡金属化合物或络合物,例如,锇、钌、铁和钴化合物或络合物。认为与聚合物组分一起使用的多种氧化还原物质也可以不与聚合物组分一起使用。
一类聚合物电子传递试剂含有在聚合物组合物中共价结合的氧化还原物质。这类介质的实例为聚(乙烯基二茂铁)。另一类电子传递试剂含有离子结合的氧化还原物质。这类介质可以包含与带有相反电荷的氧化还原物质结合的荷电聚合物。这类介质的实例包括与带正电的氧化还原物质(如锇或钌聚吡啶基阳离子)结合的带负电的聚合物。离子结合介质的另一实例为与带负电的氧化还原物质(如铁氰化物或者亚铁氰化物)结合的带正电的聚合物,如季铵化聚(4-乙烯基吡啶)或聚(1-乙烯基咪唑)。在其它的实施方式中,电子传递试剂包括与聚合物配位结合的氧化还原物质。例如,可以通过2,2′-二吡啶锇或2,2′-二吡啶钴络合物与聚(1-乙烯基咪唑)或聚(4-乙烯基吡啶)的配位形成所述介质。
适合的电子传递试剂为具有一个或多个配基的锇过渡金属络合物,所述每个配基具有含氮杂环,如2,2′-联吡啶、1,10-菲咯啉、1-甲基-2-吡啶基二咪唑或它们的衍生物。所述电子传递试剂还可以具有在聚合物中共价结合的一个或多个配基,其中每个配基具有至少一个含氮杂环,如吡啶、咪唑或它们的衍生物。所述电子传递试剂还可以具有在聚合物中共价结合的一个或多个配基,其中每个配基具有至少一个含氮杂环,如吡啶、咪唑或它们的衍生物。电子传递试剂的一个实例包括(a)具有吡啶或咪唑官能团的聚合物或共聚物和(b)与两个配基络合的锇阳离子,其中每个配基包含2,2′-联吡啶、1,10-菲咯啉或它们的衍生物,所述两个配基不必相同。与锇阳离子络合的2,2′-联吡啶的一些衍生物包括,但不限于,4,4’-二甲基-2,2′-联吡啶和单烷氧基-、二烷氧基-和聚烷氧基-2,2’-联吡啶,如4,4’-二甲氧基-2,2’-联吡啶。与锇阳离子络合的1,10-菲咯啉的衍生物包括,但不限于,4,7-二甲基-1,10-菲咯啉和单烷氧基-、二烷氧基-和聚烷氧基-1,10-菲咯啉,如4,7-二甲氧基-1,10-菲咯啉。与锇阳离子络合的聚合物包括,但不限于,聚(1-乙烯基咪唑)(称为“PVI”)和聚(4-乙烯基吡啶)(称为“PVP”)的聚合物和共聚物。聚(1-乙烯基咪唑)的适合的共聚物取代基包括丙烯腈、丙烯酰胺和取代的或季铵化的N-乙烯基咪唑,例如,具有络合至聚(1-乙烯基咪唑)的聚合物或共聚物上的锇的电子传递试剂。
实施方式可以使用电子传递试剂,其具有相对于标准甘汞电极(SCE)的从约-200mV至约+200mV的氧化还原电势。所述传感层还可以包括能够催化分析物反应的催化剂。在一些实施方式中,所述催化剂还可以起到电子传递试剂的作用。适合的催化剂的一个实例为催化分析物反应的酶。例如,当所关心的分析物为葡萄糖时,可以使用的催化剂例如是葡萄糖氧化酶、葡萄糖脱氢酶(例如,吡咯并喹啉苯醌(PQQ)依赖性葡萄糖脱氢酶、黄素腺嘌呤二核苷酸(FAD)依赖性葡萄糖脱氢酶或烟酰胺腺嘌呤二核苷酸(NAD)依赖性葡萄糖脱氢酶)。当所关心的分析物为乳酸盐时可以使用乳酸盐氧化酶或乳酸盐脱氢酶。当所关心的分析物为氧或者当对分析物的反应响应时产生或消耗氧时,可以使用漆酶。
在一些实施方式中,可以将催化剂连接到聚合物上,将所述催化剂与另一电子传递试剂(如上所述,其可以是聚合物电子传递试剂)交联。在一些实施方式中,还可以使用第二催化剂。该第二催化剂可以用于催化从分析物的催化反应中得到的产物化合物的反应。所述第二催化剂可以与电子传递试剂一同操作以电解所述产物化合物从而在工作电极上产生信号。可替代地,可以将第二催化剂设置在干扰物消除层上以催化除去干扰物的反应。
一些实施方式包括在温和氧化电位(例如,约+40mV的电位)起作用的Wired EnzymeTM传感层。该传感层使用了设计在低电位工作的锇(Os)基介质并且它稳定地锚定在聚合物层上。因此,在一些实施方式中,所述传感元件为氧化还原活性组分,其包含(1)通过锚定到聚合物主链上的稳定(双向)配基连接的锇基介质分子和(2)葡萄糖氧化酶分子。将这两种组分交联在一起。
可以将传质(物质传输,mass transport)限制层(未示出),例如,分析物通量调节层,与传感器包含在一起以用作扩散限制屏障从而减少分析物(例如,葡萄糖或者乳酸盐)向工作电极周围区域传质的速率。所述传质限制层在限制分析物向电化学传感器中工作电极的流量中有用,从而使得传感器在较大的分析物浓度范围内线性响应并且易于校准。传质限制层可以包含聚合物并且可以是生物相容的。传质限制层可以起多种功能,例如,所述传质限制层可以提供生物相容层和/或干扰物消除层的功能。
在一些实施方式中,传质限制层是由含有杂环氮基团的交联聚合物(如聚乙烯基吡啶和聚乙烯基咪唑的聚合物)组成的膜。实施方式还包括由聚氨脂,或聚醚聚氨酯,或化学相关材料制成的膜,或由聚硅氧烷制成的膜等。
根据一些实施方式,在醇缓冲溶液中通过聚合物的原位交联形成了膜并且使用两性离子部分、非吡啶共聚物组分以及可选地亲水或疏水和/或具有其它所需性质的另一部分修饰所述膜。可以由含有杂环氮基团的前体聚合物制备所述修饰的聚合物。可选地,可以使用亲水或疏水调节剂对所得的膜的所关心分析物的渗透性进行“微调”。可以使用可选的亲水调节剂来提高聚合物或所得的膜的生物相容性,所述亲水调节剂例如是聚(乙二醇)、羟基或聚羟基调节剂。
可以通过将交联剂和修饰的聚合物的醇缓冲溶液应用到含有酶的传感层上并使该溶液固化约一至两天或其它适当的时间以原位形成膜。可以通过将所述溶液的液滴置于传感器上或者将传感器浸入所述溶液中等方法,将所述交联剂-聚合物溶液施加到所述传感层上。通常,通过所述溶液的浓度、所述溶液所施加的液滴数、将传感器浸入所述溶液的次数或通过这些因素的任意组合来控制所述膜的厚度。以这种方式应用的膜具有以下功能的任意组合:(1)传质限制,即,减少可以到达所述传感层的分析物的通量,(2)提高生物相容性或(3)减少干扰物。
电化学传感器可以使用任何适合的测量技术。例如,可以检测电流或者可以使用电位测定法。这样的技术可以包括,但不限于,电流分析法、电量分析法、伏安法。在一些实施方式中,传感系统可以是光学系统、比色系统等。
在一些实施方式中,所述传感系统检测过氧化氢来推断葡萄糖水平。例如,可以构建过氧化氢检测传感器,其中传感层包含如葡萄糖氧化物、葡萄糖脱氢酶等的酶,并且将其安置在工作电极附近。可以用对葡萄糖选择性渗透的膜覆盖发送层。葡萄糖通过所述膜后就会被酶氧化,并且然后可以通过与分子氧反应氧化还原的葡萄糖氧化酶以产生过氧化氢。
一些实施方式包括过氧化氢检测传感器,它是从通过将两种组份交联在一起制备的传感层构建得到的,例如:(1)氧化还原化合物,如含有侧链Os聚吡啶基络合物的氧化还原聚合物,其相对于SCE具有约+200mV的氧化电势,和(2)高碘酸氧化的辣根过氧化酶(HRP)。这种传感器以还原模式起作用;在低于Os络合物的电势下控制工作电极,从而通过HRP催化剂导致过氧化氢中等程度的降低。
在另一实施例中,可以如下构建电势传感器。通过将下列两种物质交联在一起构建了葡萄糖传感层:(1)含有侧链Os聚吡啶基络合物的氧化还原聚合物,其相对于SCE具有从约-200mV至+200mV的氧化电势,和(2)葡萄糖氧化酶。然后,通过在零电流条件下使该传感器暴露于含有葡萄糖的溶液并且使得还原/氧化Os比达到平衡值从而可以以电势模式使用该传感器。所述还原/氧化Os比以可重复的方式随葡萄糖浓度变化,并且将使得该电极的电势以类似的方式变化。
传感器还可以包括布置在置于用户中的传感器的至少一部分上的活化剂,如抗凝剂和/或抗糖酵解剂。抗凝剂可以降低或防止传感器周围血液或其它体液的凝固,特别是在插入传感器后。凝血块会污染传感器或者不可再现地降低扩散到传感器中的分析物的量。有用的抗凝剂的实例包括肝素和组织纤维蛋白溶酶原活化因子(TPA)以及其它已知的抗凝剂。实施方式可以包含抗糖酵解剂或它们的前体。抗糖酵解剂的实例为甘油醛、氟化物离子和甘露糖。在本文中广泛使用了术语“抗糖酵解”以包括至少延迟活细胞的葡萄糖消耗的任何物质。
可以将本文所述的传感器配置为不需要系统校准或用户校准。例如,传感器可以进行出厂校准而不需要另外进行校准。在一些实施方式中,可能需要校准,但是该校准可以在没有用户干预的情况下进行,即,可以自动进行。在需要用户进行校准的那些实施方式中,可以根据预定的时间表进行校准或者动态进行校准,即,可以根据各种因素在实时的基础上由系统确定时间,所述因素例如是,但不限于,葡萄糖浓度和/或温度和/或葡萄糖变化率等。
可以使用体外测验条或其它校准器来完成校准,例如,小型样本检验条,如需要少于约1微升样本(例如Abbott Diabetes Care Inc.的血糖监测测试条)的测试条。例如,可以使用需要少于约1纳升样本的测试条。在一些实施方式中,可以根据校准事件仅使用一个体液样本校准传感器。例如,用户仅需要割破身体部分一次以获得用于校准(例如,用于测试条)的样本,或者如果第一次所获得的样本体积不够,则可以在较短时间内割破多于一次。实施方式包括对于给定校准,获得并使用多个体液样本,其中每个样本的葡萄糖值基本类似。可以独立地使用从给定校准事件获得的数据进行校准或者与从先前校准所获得的数据(例如,包括加权平均在内的平均数据等)结合进行校准。在一些实施方式中,系统仅需要用户校准一次,在这种情况下,不需要对系统进行再校准。
分析物系统可以包括可选的报警系统,例如,该报警系统基于处理器信息警告患者分析物可能的有害情况。例如,如果葡萄糖为分析物,则报警系统可以在发生以下情况时警告用户,如低血糖和/或高血糖和/或急性低血糖和/或急性高血糖。当分析物水平达到或超过阈值时,会触发报警系统。或者作为另外一种选择,还可以当分析物水平增加或减少方法中的变化率或变化率的增速达到或超过阈值速率或阈值增速时激活报警系统。例如,就葡萄糖监测系统而言,如果葡萄糖浓度的变化率超过可以指示可能发生高血糖或低血糖状况的阈值时,就会激活报警系统。系统还可以包括通知用户系统信息的系统报警,如电池状况、校准、传感器移位、传感器故障等。例如,报警可以是可听到的和/或可视的。可以使用其它感官刺激报警系统,包括激活时加热、制冷、震动或产生温和电击的报警系统。
本发明公开的主题还包括在基于传感器的药物递送系统中使用的传感器。作为对一个或多个传感器信号的响应,所述系统可以提供药物以中和分析物的高水平或低水平。作为另外一种选择,所述系统可以监测药物浓度以确保所述药物保持在所需的治疗范围内。所述药物递送系统可以包括一个或多个(例如,两个或更多个)传感器,处理单元,如发射器、接收器/显示器单元和药物施用系统。在一些情况下,可以将一些或所有组件集成到单一单元中。所述基于传感器的药物递送系统可以使用来自一个或多个传感器的数据以提供控制算法/机制的必要输入从而调节药物施用,例如,自动或半自动地进行调节。举例来说,葡萄糖传感器可以用于控制和调节外部或植入胰岛素泵的胰岛素施用。
再参考附图,如上所讨论的,例如,由于组织创伤等,与分析物传感器有关的灵敏度可能会在传感器插入后的约第一个24小时内衰减,从而会导致分析物传感器的ESA状况。因此,根据本发明公开的实施方式,提供了分析物传感器校准控制以有效地检测传感器ESA状况的发生,正确地对其进行分类,并随后控制ESA状况从而使传感器可能的错误读数最小化并且使传感器开始报告所监测的分析物水平所等待的时间尽可能接近于传感器初始插入的时间。
一方面,可以配置分析物传感器校准控制程序以检测ESA状况的存在、确认所检测的ESA事件和控制所确认的ESA事件期间的校准从而确保最佳的校准灵敏度估计。
一方面,分析物传感器校准控制算法包括三个部分:(1)ESA检测、(2)ESA分类、和(3)ESA控制。以下详细讨论了控制算法的每个方面或部分。
一方面,校准控制算法的ESA检测部分包括传感器信号(例如,分析物传感器的原始电信号)的检测和对其进行ESA状况特征评价。如果基于该评价检测出ESA状况,则请求分析物传感器进行校准(例如,通过促使用户进行手指针刺测试并输入所得的参比血糖测量值)以获得用于确认ESA事件的灵敏度。
在本发明公开的一个方面,传感器校准控制程序的ESA分类方面包括基于校准测量的灵敏度对分析物传感器信号中可能衰减的严重性进行评级。一方面,ESA分类程序可以将传感器信号特征分为以下三类中的一类:(a)无ESA(0)、(b)可能的ESA(1)或(b)很可能的ESA(2),基于此,一方面,校准控制程序的ESA控制部分进行其它处理以,例如,输出所得的监测的分析物水平(例如,在接收器单元104/106(图1)的显示器上),或在给定的时间段内请求其它参比血糖测量以验证不再存在ESA状况或ESA状况不显著。
更具体地,一方面,至少部分基于ESA分类程序,可以配置校准控制算法的ESA控制程序以修正校准灵敏度并且报告或显示传感器所监测的分析物水平,修正校准灵敏度并且暂时报告所监测的分析物水平或延缓报告所监测的分析物水平。以这种方式,在本发明公开的一个方面,提供了一种有效的传感器校准控制方法,基于,例如,使数据得率最大(从初始插入开始尽可能早地报告所监测到的葡萄糖水平)并且使必要的校准尝试次数最小(例如,进行体外血糖测试的需要),所述传感器校准控制方法优化了分析物监测系统的准确性并改善了用户体验。
更具体地,一方面,可以配置校准控制算法的ESA检测程序以通过评价各种信号特性来检测可能的ESA事件,所述信号特性包括传感器输出、温度和/或从传感器插入开始所经过的时间。在一个方面,ESA检测程序对传感器信号评价了ESA事件中所存在的或与ESA事件有关的那些特征的类似特征,包括例如,在第一个24小时内传感器信号的降低或衰减。ESA检测程序的阈值可以根据ESA事件概率以与其它可测量的量的相关方式的先验知识和/或根据对ESA事件本身可能性的实时修正而改变。先验知识的实例可以包括ESA状况的概率与从传感器寿命开始(即传感器插入)起所经过的时间之间的相关性。
当ESA检测程序确定传感器输出表现出ESA状况特征的概率较高时,一方面,可以请求进行另一种校准量测(即手指针刺测试)以用于对ESA事件进行分类和确认。根据基于ESA检测程序的ESA状况存在的确定性或可能性,ESA状况检测后的校准请求时间和传感器信号报告可能是不同的。
例如,在一种实施方式中,如果ESA检测程序确定ESA状况的概率较高,则可以立即请求校准(例如,通过促使用户进行另一手指针刺测试)并提供所获得的参比血糖测定值,并且不向用户报告表示所监测到的分析物水平的传感器数据,而是将其保留(例如,通过使接收器单元104/106中的显示器禁用、暂停或不活动)到可以进行校准测量以确认ESA状况存在为止。另一方面,如果ESA检测程序确定ESA状况存在的可能性不太确定,则可以有条件地向用户报告或显示对应于所监测的分析物水平的传感器数据,并且如果衰减信号特征(可能指示ESA状况的可能性)持续了一段预定的时间后,则可以在随后排定的时间请求其它校准。
在一个方面,可以配置分析物传感器校准控制算法的ESA分类程序以将传感器信号特征分成基于传感器存在ESA状况的可信度的三个水平。可以配置该程序以通过考察原始传感器信号(Sr)的幅度以及从参比葡萄糖测量所获得的传感器信号的灵敏度来评价该传感器信号,对于所述参比葡萄糖测量,考虑了先前参比测量的幅度(Si)和变化(dSi)。可以检查分配给三个算法分类水平(0、1或2)中每一个的各个信号测量(Sr、Si和dSi)的阈值以将灵敏度分配到三种ESA分类中的一类。
所述三种分类指示了分析物传感器存在ESA状况的可信度或可能性。例如,无ESA(水平0)指示传感器存在ESA状况的可能性为零。可能的ESA(水平1)指示在该校准事件中传感器有可能存在ESA状况。另外,很可能的ESA(水平2)指示在当前校准事件中传感器很有可能存在ESA状况。例如,当用户进行手指针刺测试以提供参比血糖测量时,在每次校准测量时对这些测量值进行检查,从而导致对每个校准事件进行适当的分类。由于ESA信号特征的概率随从初始佩戴传感器算起所经过的时间而改变,因此ESA分类程序的阈值也会随时间而改变。由于当先前已观察到该传感器的信号扰动时,给定校准会导致ESA检测的升高,因此该阈值还会根据任何给定传感器的先前校准量测的结果而改变。
在本发明公开的一个方面,基于传感器存在ESA状况时的可信水平,传感器校准控制算法的ESA控制程序具有三种结果。对于分类为无ESA(水平0)的校准,ESA事件将不可能导致不准确的结果,并且因此,基于从校准事件获得的灵敏度确定并报告了对应于所监测的分析物水平的传感器数据。
对于分类为可能的ESA(水平1)的校准,灵敏度估计在有限的时间段内可能是有效的,并且因此,在验证的基础(在预定的时间段内,例如,两小时或任何其它适合的验证时间段)上根据从校准事件所获得的灵敏度可以确定并向用户报告对应于所监测的分析物水平的传感器数据,在此之后,可以促使用户进行另一次校准以确认从校准所得到的灵敏度的持续有效性。
对于分类为很可能的ESA(水平2)的校准,对应于所监测的分析物水平的传感器数据将可能会包含显著的衰减或错误,并且因此,将传感器数据的报告或输出暂停预定的一段等待时间,并且在此期间使传感器信号得到恢复(例如,从暂时的衰减恢复)。在预定的一段等待时间结束时,可以请求用户进行另一次手指针刺测试以进行另一次校准从而验证ESA状况不再存在或者已不显著。
以这种方式,根据本发明公开的各种实施方式,提供了分析物传感器校准控制,其有效地处理了分析物传感器信号从而最大程度地准确报告了所监测的分析物水平,并且使发生信号衰减事件时提供传感器错误或不正确读数的可能性最小。
在本发明公开的一个方面,可以将本文所述的程序和算法中加入到分析物监测系统100(图1)的接收器单元104/106、发射器单元102或数据处理终端/输注部分105。更具体地,根据本发明公开的实施方式,可以提供一个或多个信号检测器,其配置用于进行一些共享的或所有本文所述的程序从而通过,例如,一个或多个处理器、状态机等(可以包括集成电路、专用集成电路(ASIC)和/或硬件和软件组件的组合,其包括如随机存取存储器(RAM)、只读存储器(ROM)、固态驱动器等的存储器设备)为ESA检测、ESA分类和ESA控制提供传感器校准控制。
更具体地,在一种实施方式中,可以使用多个信号检测器来进行本文所述的校准控制程序。可以配置第一信号检测器,其基于血糖测量或其它参比信息以及来自传感器分析物监测系统的分析物传感器数据对ESA状态进行检测。可以配置第一信号检测器的结果以确定来自分析物监测系统的所监测的传感器信号是否处于ESA状况。
可以配置第二信号检测器以监测分析物监测系统的采样数据(例如,一分钟数据或任何适合的采样率)。在一个方面,可以配置第二信号检测器以指导分析物监测系统从而提醒用户输入当下的或排定的血糖测量值(例如,基于使用血糖计的手指针刺测试)从而确认ESA状况是否存在,并结合第一信号检测器使用,即,基于参比血糖测量对分析物监测系统的ESA状况进行检测。
在一个方面,配置了第一和第二信号检测器以产生多个ESA水平中的一个,例如,三个ESA水平-水平0:无ESA、水平1:可能的ESA和水平2:很可能的ESA。如以上所讨论的,在一个方面,可以将与传感器的可能ESA状态有关的水平2状况称为无显著信号衰减,但是基于与传感器有关的所检测或监测的状况,在如两小时(或任何其它适合的时间段)的预定时间段内需要或必须对可能的ESA状况进行验证。
第一信号检测器和ESA分类模块
当在活动的ESA检测阶段校准尝试通过数据状况验证程序时,运行基于手指针刺测试(或参比血糖测量)的ESA检测器(ESA_FS)。更具体地,ESA_FS检测器在第一基线校准时启动了它的活动(例如,从传感器插入的时间算起的约1小时或更短的时间)。当ESA状况的可能性最大时,它在传感器寿命(例如,从初始插入算起的约第一个24小时)的最初阶段仍保持活动。
在一种实施方式中,所述第一信号检测器在活动的ESA状况检测阶段起到了“ESA分类”模块620(图6)的作用。另外,如果其它信号检测器不怀疑ESA状况,却在活动的ESA状况检测阶段进行了适当的手指针刺血糖测量,那么第一信号检测器还起到“ESA检测”模块610的作用。
ESA_FS检测器使用了两种测试,一种是相对的而另一种是绝对的,两种测试中的任何一种均可以基于任何参比血糖测量来检测活动的ESA检测阶段内的信号衰减水平(ESA_FS水平)。如果两种测试的水平不同,则可以选择两种水平中的较高水平。更具体地,在一种实施方式中,所述相对测试将基于最近的手指针刺血糖测试的最近的即刻灵敏度值,Si(k),与先前的即刻灵敏度值,Si(k-1),以及用于计算组合灵敏度的最近的即刻灵敏度,Si(m),进行比较。选择Si(k)、Si(k-1)和Si(m)的值从而在那些情况(在时间指数k、k-1和m时)下通过了校准后置条件验证。一方面,手动校准取决于ESA_FS检测器进行的测试,但是所得的即刻灵敏度不会作为先前的值使用。
基于所述相对测试,形成了两个比值,Si(k)/Si(k-1)和Si(k)/Si(m)。如下所示,使用这些比值指定ESA_FS水平的两种阈值:
(1)ESA_FS水平2(很可能的ESA状况),如果:

否则,ESA_FS的所述相对测试产生了指示不存在ESA状况的水平0输出。
根据本发明公开的方面,所述绝对测试将灵敏度水平Si(k)与调整到分析物传感器标称灵敏度Snom的灵敏度阈值进行比较。与所述相对测试中的相同,可以选择Si(k)从而使其通过校准后置条件验证。如下所示,分配了ESA_FS水平:
如果:Si(k)/Snom<Kmin_Abs_ESA_FS[2],则为ESA_FS水平2(很可能的ESA状况),
如果非ESA_FS水平2,并且:
Si(k)/Snom<Kmin_Abs_ESA_FS[1],则为ESA_FS水平1(可能的ESA状况),
其中<Kmin_Abs_ESA_FS[2]小于等于Kmin_Abs_ESA_FS[1],并且另外,这两个参数中的每一个均可以是预定的或编程的。
否则,ESA_FS的所述绝对测试就产生水平0输出(指示未检测到ESA状况)。
当ESA状况的可能性最大时,以上相对和绝对测试的阈值是有效的。当ESA检测器在超出绝对最近时间(当超出该时间时,系统将忽略ESA检测)而保持活动时,可以假定ESA的可能性与从传感器插入开始所经过的时间相关,并且不同的可能性使得在ESA检测最大化和校准请求次数最少之间做出不同的权衡。
第二信号检测器
分析物监测系统中的第二信号检测器基于从分析物传感器信号特征对ESA状况进行推断。信号特征的一个实施例为低葡萄糖值的检测。当该检测器报告非零ESA水平时(例如,存在信号衰减(ESA)),则有两种可能性:系统处于ESA或用户处于(或接近)低血糖。
在一种实施方式中,可以将第二信号检测器配置为包括“ESA检测”模块610(图6)的功能,并且第二信号检测器不只用于对整个系统的“ESA控制”模块630进行所检测的ESA状况分类。当所述第二信号检测器产生非零输出时,以整个系统的“ESA控制”模块630所确定的方式预期参比血糖测量。
为了使噪音的影响最小,将预定个数的分析物传感器的最近的未过滤葡萄糖样本,GCAL,取平均值以得到葡萄糖值GESA_CGM。
基于葡萄糖值GESA_CGM,所述检测器报告三种可能的ESA水平中的一种:
如果GESA_CGM<Gmin_ESA_CGM[2],则报告ESA水平2(很可能的ESA状况),
如果非ESA_FS水平2,并且
GESA_CGM<Gmin_ESA_CGM[1],则报告ESA水平1(可能的ESA状况),
其中Gmin_ESA_CGM[2]小于或等于Gmin_ESA_CGM[1],并且另外,对应于已编程到系统中的预定的值或参数。
否则,ESA的所述绝对测试就产生指示未检测到ESA状况的水平0输出。此外,如果不能获得所有最近预定个数的未过滤葡萄糖样本GCAL,则可将所述第二信号检测器配置为报告零水平。
ESA事件控制器
在一种实施方式中,可以通过“ESA控制”模块630(图6)来控制所述第一信号检测器和共存的所述第二信号检测器的不同作用。ESA控制会受到以下因素的影响,包括:每个检测器的灵敏度和特异性、过去或先前校准以及参比血糖测量时间的历史、近期排定的校准和包括可用性的其它方面。
在传感器处于指示不存在ESA状况的水平0状况的情况下,可以配置第二信号检测器以在第二基线校准(即分析物传感器的第二排定校准时间段)后开始运行,所述第二基线校准可以是第一绝对校准(从初始传感器插入事件算起)后排定的浮点校准(从确定无ESA状况时算起),并借此向用户报告所测量到的葡萄糖水平。一方面,可以在与系统中信号检测器的最早允许终止有关的预定时间暂停所测量的葡萄糖水平的显示或输出。
为了避免时间间隔较短的手指针刺血糖测量,当在任何校准尝试后,在成功的基线校准(不允许异步稳定性请求)后所经过的时间不到预定的空闲时间(例如,30分钟)时,或者当在任何排定的校准尝试前,在下一次排定的基线校准(不允许异步稳定性请求)前所经过的时间不到预定的空闲时间(例如,30分钟)时,可以忽略第二信号检测器的输出。
一方面,在活动的ESA检测阶段,所述第一信号检测器用于确定是否存在ESA状况。在一种实施方式中,假定在传感器开始时(即初始插入传感器时)不存在ESA状况。一方面,如下说明了这些状态的过渡、进行和保留。例如,一方面,当确定最近的ESA_FS水平大于第一信号检测器先前测量到的最大允许输出水平(认为是不存在早期信号衰减的指征)时,发生了向确定ESA状况存在的过渡。例如,在本发明公开的一个方面,在认为水平0和水平1对衰减缓和是不够严格的情况下,可以仅将水平2设置为触发向确定ESA状况存在的状况过渡。
一方面,当分析物监测系统确定传感器处于ESA状况时,可以配置接收器单元104/106(图1)以禁用所测量或检测到的葡萄糖水平的输出或显示。此外,可以配置接收器单元104/106从而在过渡到确认无ESA状况的状态前,在提醒用户另一确认(例如,通过请求另一次手指针刺测试)前,在通过参比血糖测量确认存在ESA状况后,将禁用(或暂停或不活动)的输出/显示保持预定的一段空闲时间。
此外,另一方面,可以配置接收器单元104/106从而在传感器处于ESA状况时不请求稳定性校准验证。然而,如果确认,则可以使用任何用户自动或自起始的手指针刺血糖测量以过渡到不存在ESA状况的状态。另外,当认为传感器处于确认的非ESA状况时,第二信号检测器显示水平0(反映无ESA状况)并且通过信号前置条件验证,可以配置分析物监测系统的接收器单元104/106(图1)以请求参比血糖测量从而确认完全不存在ESA状况和/或起始校准。
一方面,当新的手指针刺血糖测量显示ESA_FS水平(例如,以上所讨论的第一信号检测器的输出)小于或等于第一信号检测器的先前测量的最大允许输出水平(认为是不存在早期信号衰减的标志)时,发生了向与不存在ESA状况有关的状态的过渡。例如,要报告或显示的葡萄糖结果需要成功的校准。
当确定不存在ESA状况时,一方面,可以进一步处理传感器信号以确定稳定性和可能的误差。例如,一方面,当第二信号检测器第一次发生水平2输出(即,很可能的ESA状况)时,会产生稳定性校准的即刻请求。在第二信号检测器第一次发生水平2输出后,在最近的成功校准后,在请求另一测量前,在从具有稳定性验证程序的上次参比血糖测量起的时间间隔(例如,约1至2小时或其它适合的时间间隔)时检查分析物传感器信号。另外,非零水平(即水平1或水平2-可能或很可能的ESA状况)触发稳定性校准请求。另外,一方面,如果先前的ESA_FS水平大于0,则在从上次血糖测量起的时间间隔时请求参比血糖测量,并且使用新的参比测量确定ESA_FS。
现在参考附图,图6是说明根据本发明公开的一种实施方式的整个分析物传感器校准控制的流程图。如图所示,根据本发明公开的一个方面的分析物传感器校准控制系统包括ESA检测模块610和ESA分类模块620和ESA控制模块。如以上所讨论的,配置了ESA检测模块610以检测插入或佩戴分析物传感器后的初始时间段内(例如,从初始传感器插入算起的第一个24小时)的早期信号衰减事件的发生。
当检测到与分析物传感器有关的信号衰减时,配置一种实施方式中的ESA分类模块从而对所检测的信号衰减状况进行适当的分类。其后,根据所检测的ESA状况的类型-例如,无ESA状况检测、有可能的ESA状况检测或可能的ESA情况检测,配置ESA控制模块630以启动一个或多个过程从而确认所检测和所分类的信号衰减状况。并且另外,为了进行其它处理以有效地控制与分析物传感器运行有关的校准算法从而使得可达到的最大可报告数据得率,提供了分析物传感器的改善的可用性以用于连续或间歇监测和输出所监测的分析物水平,如患者或用户的葡萄糖水平的波动。
图7是说明根据本发明公开的一个方面的图6的早期信号衰减(ESA)检测程序的流程图。参考图7,例如,说明了ESA检测模块610(图6)所进行的ESA检测程序。更具体地,对于分析物传感器的每个所监测的信号(如对于分析物传感器的每个一分钟数据),起始了新的时间步长(701)并且确定是否应该或者已经启动了ESA检测器(702)。如果确定尚未启动或者不应启动ESA检测器,则该程序根据从传感器所接收的下一信号(例如,从分析物传感器所接收的下一分钟的信号)等待下一时间步骤(714)。
参考图7,如果确定ESA检测器应该启动或确实已启动(702),则确定它是否应该终止(703)。如果确定应该终止ESA检测器(703),则结束该程序(704)。然而,如果确定不应该终止ESA检测器(703),则确定是否已激活待定的参比血糖测量请求计时器(705)。如果确定已激活待定的参比血糖测量请求计时器(705),则在一种实施方式中,产生了参比血糖测量请求并向用户输出非零宽限期(706)。此后,暂停ESA检测器并且系统等待所请求的参比血糖测量。
另一方面,如果确定未激活待定的参比血糖测量请求计时器(705),则检索到或获得分析物传感器信号信息以及其它相关信息(707)。也就是说,除了例如传感器代码灵敏度、即刻灵敏度、从传感器插入开始所经过的持续时间以及从先前传感器校准开始所经过的时间之类的其它相关信息以外,获得了例如传感器原始电信号、相关的温度信息、传感器反电压数据。在本发明公开的范围内,可以获得与分析物传感器的操作和特征有关的其它相关数据。
再参考图7,此后,确定了存在ESA状况的概率(709),并且将该结果与一个或多个阈值进行比较以确定是否存在高ESA条件概率(710)。如果确定存在高ESA条件概率,则请求具有非零宽限期的参比血糖测量数据(711),并且暂停ESA检测程序以等待所请求的参比血糖测量数据。另一方面,如果确定不存在高ESA条件概率(710),则确定是否存在中等ESA条件概率(712)。如果确定不存在中等ESA状况,则该程序继续等待下一传感器数据(714)。另一方面,如果确定存在中等ESA条件概率(712),则将待定的参比血糖测量计时器设置为在未来的预定的相对时间激活,除非在激活的计时器终止前检测到成功的校准(713)。
以上述的方式,在本发明公开的一种实施方式中,可以配置ESA检测模块610(图6)以检测从传感器插入开始的初始时间段内的分析物传感器信号的衰减。

再参考图9,此后确定是否存在ESA分类结果或输出(906)。如果不存在,则进行实时葡萄糖信息的输出或报告并且向用户提供葡萄糖水平数据(914),并且此后等待接收与下一时间步长有关的下一分析物信号(918)。另一方面,如果存在ESA分类结果(906),则确定ESA分类是否与当前分析物信号有关(例如,与当前时间步长有关)(907)。如果与当前分析物信号无关,则该程序继续向用户显示或输出所监测的分析物水平(914),并等待接收与下一时间步长有关的下一分析物信号(918)。

以如上所述的方式,根据本发明公开的各种实施方式,说明了提供有效分析物传感器校准控制的方法、装置和系统,其监测传感器信号的早期衰减并处理所监测的信号从而通过向用户提供大量有用且准确的所监测的葡萄糖水平信息使传感器数据得率最大化。初步研究结果
以手指针刺葡萄糖测量作为参比,在正常(N=10)、T1DM(N=1)和T2DM(N=2)受试者中使用48个传感器插入进行了初步研究。当比较使用传感器的第一天的前10个小时和剩余的从10小时至24小时时间的平均绝对相对偏差(MARD):分别为13.8%(12.8-14.995%CI)及12.6%(11.6-13.695%CI)时,结果并没观察到变差。在前10个小时中,用户可获得7.5小时±2.2小时(平均值±标准差)的葡萄糖数据,从而显著地使可获得的数据得率最大化,并且在传感器插入后不久就提供可报告的葡萄糖信息。
第二初步研究包括上述系统性能的评价,其包含两个地点,每个地点有47位佩戴两个传感器(腹部/手臂)的患者。从传感器插入后1小时开始,以1分钟的间隔收集连续的葡萄糖读数。在佩戴传感器的5天中,在两次就诊检查之间的26小时内,每15分钟使用标准实验室参考(YSI 2300)获得静脉血葡萄糖测量。用单独的血糖计对每位受试者进行毛细管BG测量(平均每天20次)。
传感器系统和YSI之间的平均和中值绝对相对偏差分别为14.5%和10.7%。此外,结合了速率和点信息的连续葡萄糖误差栅格分析(Glucose-Error Grid Analysis)得到了93.9%的准确读数和另外3.2%的良性误差(benign error)。
传统Clarke误差栅格(Clarke Error Grid,CEG)区域A性能为77.1%(6229/8084)。这包括IV胰岛素激发期间的低血糖和血糖高变化率期。当血糖变化率在±1mg/dL/min内时,区域A性能为82.0%(4672/5699)。在全部5天内性能保持恒定,其中第1天区域A中的数据为80.7%,第5天区域A中的为74.1%(p=0.4503)。此外,克拉克误差网格区域A性能与毛细管血糖测量的比为81.2%(3337/4108)。
通过阈值或计划报警(30分钟设置)检测了70mg/dL(n=119)时91.6%的低血糖事件。通过阈值或计划报警检测了240mg/dL(n=144)时97.2%的高血糖事件。阈值或计划报警的假报警率在70mg/dL时为25.2%而在240mg/dL时为21.2%。
基于上述内容,第二初步研究的结果证明了FreeStyle连续葡萄糖监测系统的良好性能,并且在佩戴传感器的5天中,从传感器插入后约1小时开始显示数据。
以上述的方式,根据本发明公开的各种实施方式,分析物传感器校准控制使由于ESA状况而出现错误分析物传感器结果的情况最少并且使不表现ESA信号特征的传感器的可报告分析物传感器数据最多。因此,在本发明公开的方面中,校准控制算法适用于任何皮下安置的分析物传感器,其能够表现出ESA信号特征,并且在分析物传感器灵敏度可能偏离真实传感器灵敏度时启用校准控制。
因此,一方面,该方法包括在预定时间段内监测低于与分析物传感器的分析物水平有关的预定阈值的信号水平,并且当在预定时间段内未检测到所监测的信号水平时报告与分析物传感器有关的分析物水平。
所述预定时间段可以包括不到约1小时。
另一方面,所述方法可以包含接受血糖测量,并根据所接受的血糖测量校准分析物传感器。
另外,预定的阈值可以与一个或多个紧急的低血糖状态或与预定的信号衰减水平有关。
同时,报告分析物水平可以包含存储分析物水平、确认分析物水平或输出分析物水平中的一个或多个。
上述的各种方法包括分析物监测系统100(包括结合图6-9所说明的方法和程序)的数据处理单元102、接收器单元104/106或数据处理终端/输注部分105(图1)在软件应用进行环境中所进行的方法,它们可以体现为使用面向对象的语言开发的计算机程序,所述面向对象的语言允许用模块对象对复杂系统建模以产生代表真实世界、物理对象和它们相互关系的抽象概念。可以将进行本发明方法所需的软件存储在数据处理单元102、接收器单元104/106或数据处理终端/输注部分105的存储器或存储设备(未出示)中,所述软件可以由本领域普通技术人员开发并且可以包括一个或多个计算机程序产品。

对所检测的分析物传感器信号衰减的分类可以至少部分基于预定的多个信号衰减状况。
一方面,多个信号衰减状况可以包括可报告信号状况、条件可报告信号状况和不可报告信号状况。
另一方面可以包括当所检测的分析物传感器信号衰减包括可报告信号状况或条件可报告信号状况时,输出与基于所检测的分析物传感器信号所监测到的分析物水平有关的数据。
输出与所监测到的分析物水平有关的数据可以包括当所检测的分析物传感器信号衰减包括条件可报告信号状况时,输出预设时间段的数据。
所述预设时间段可以不超过约2小时。

另一方面,存储器用于存储指令,其中当一个或多个处理器进行所述指令时,所述指令可以使一个或多个处理器请求参比数据并基于所述参比数据确定与分析物传感器有关的灵敏度值。
参比数据可以包括体外血糖测量数据。
此外,存储器用于存储指令,其中当一个或多个处理器进行所述指令时,所述指令可以使一个或多个处理器至少部分基于所确定的灵敏度值来校准分析物传感器。

Claims (42)
1.一种方法,包括:
以与分析物流体接触的方式安置分析物传感器;
检测安置后预定时间段内分析物传感器信号的衰减;
至少部分基于所述信号的一个或多个特征对所述分析物传感器信号中检测到的所述衰减进行分类;并且
进行信号处理以产生与所述预定时间段内的所述检测的分析物传感器信号有关的可报告数据。
2.根据权利要求1所述的方法,其中,所述分析物传感器信号与所监测到的分析物水平有关。
3.根据权利要求1所述的方法,其中,所述分析物传感器信号中检测到的所述衰减与早期信号衰减状况有关。
4.根据权利要求1所述的方法,其中,所述预定时间段不超过约24小时。
5.根据权利要求1所述的方法,其中,对所述检测到的分析物传感器信号衰减的分类至少部分基于预定的多个信号衰减状况。
6.根据权利要求5所述的方法,其中,所述多个信号衰减状况包括可报告信号状况、有条件可报告信号状况和不可报告信号状况。

基于所述参比数据,确定与所述分析物传感器有关的灵敏度值。
14.根据权利要求13所述的方法,其中,所述参比数据包括体外血糖测量数据。

16.一种方法,包括:
在预定时间段内,监测低于与分析物传感器的分析物水平有关的预定阈值的信号水平;和
当在所述预定时间段内未检测到所述监测的信号水平时,报告与所述分析物传感器有关的分析物水平。

仅当确定所述传感器稳定时,将葡萄糖相关信息报告给所述个体;
其中,在确定所述传感器稳定前,不报告所述葡萄糖相关信息。
22.根据权利要求21所述的方法,其中,使用参比数据确定传感器稳定性。
23.根据权利要求21所述的方法,其中,所述参比数据包括所述个体的血液样本。
24.根据权利要求21所述的方法,其中,所述参比数据得自葡萄糖测试条。

用于存储指令的存储器,当所述一个或多个处理器执行所述指令时,使所述一个或多个处理器以与分析物流体接触的方式安置分析物传感器,检测安置后预定时间段内分析物传感器信号的衰减,至少部分基于所述信号的一个或多个特征对所述分析物传感器信号中所检测到的衰减进行分类,并进行信号处理以产生与所述预定时间段内的所述检测到的分析物传感器信号有关的可报告数据。
28.根据权利要求27所述的装置,其中,所述分析物传感器信号与监测的分析物水平有关。
29.根据权利要求27所述的装置,其中,所述分析物传感器信号中的所述检测到的衰减与早期信号衰减状况有关。
