背景技术

发明内容
在本发明的一些实施例中,测量装置包括:(a)光学测量部件,其产生一阵光,并且响应于反射光来评定患者的生理参数,例如患者血管中的红血球运动;(b)心跳测量部件,其感测患者的心功能参数;以及(c)积分器单元,其接收来自心跳测量部件的输入并且触发光学测量部件的操作。光学测量部件典型地、但不一定包括非侵入性视网膜扫描仪。
光学测量部件所发射的光典型地包括可见光或近红外线光和/或具有在400-1000nm范围内的波长。
因此,提供了根据本发明的实施例的装置,包括:

具体实施方式
图1是根据本发明的实施例的表示用于非侵入性测量患者血管中的血流量的测量装置2的方框图。出于说明的目的,作为实际例子,本说明书主要涉及在眼睛的视网膜8上执行的这种测量。测量装置2典型地包括光学测量部件,其包括诸如用于观察视网膜血管的眼底照相机或者检眼镜的血管成像光学器件6、闪光器14以及成像仪4(例如CCD照相机)。通过例如数字帧抓取器的图像获取接口12,将图像数据发送到“心跳相位(phase)和触发积分器”单元10。积分器单元10借助于闪光器14来控制图像获取和照明计时。在一些应用中,提供显示监视器16,用于观察自动图像分析的结果和允许交互式图像分析,并且打印机18产生分析结果的硬拷贝输出。

心跳测量部件3典型地具有例如TTL脉冲的数字输出(C),其具有与其自身心跳周期的可确定关系的时域特性。例如,时域特性可以是TTL脉冲从低电压到高电压过渡的瞬间(moment),并且这可以指示QRS波群的末端。或者,心跳测量部件3的输出连续变化,以提供关于心跳周期的更完整信息。在此情况下,积分器单元10典型地分析部件3的输出,以确定在心动周期内的重复、固定的点。
利用对患者心动周期的知识,对闪光器14的操作计时,以致在患者心跳周期的已知相位期间,释放光并且获取图像。这些实施例解决的技术挑战是:通过所有血管的血流量率实质上随着心跳周期的过程变化。由于供专利`901中所述装置使用的一些采样周期比心跳周期短的多,因此在此描述的实施例具有在心跳周期内控制何时启动闪光器14并且进行测量的能力。对于一些应用,独立于心跳周期启动闪光器1,并且随后对获取图像的分析也存取来自心跳测量部件3的数据,以便拒绝那些不是在心动周期的指定窗口期间获取的图像。
在一个实施例中,在心跳周期的已知特征之后的固定时间,例如感测QRS波群的同时,或者在QRS波群最高峰之后的1-100ms、100-300ms或300-900ms内,来启动闪光器14。
然而,使用该简单算法可能获得一定程度的不准确度,这是因为与患者心率比较慢时相比,当患者心率过快时,可能在心动周期稍后的相关相位上启动闪光器14。因而,例如,在本发明的实施例中,尽管在前一QRS波群后的固定时间启动闪光器14产生了可接受的结果,其中期望在收缩期之前不久启动闪光器14(例如在QRS波群之前大约100-200ms),使用下文所述的技术能够在准确度上得到改善。
如本发明的这些实施例所提供的,在收缩期前测量血流速度,提供了稳定的血流速度的基线水平,即在心动周期中的最慢时间。对于一些应用,期望测量在心动周期期间的血流速度的变率,在这种情况下对测量器件2进行编程,以在一些心跳期间的收缩期之前而在其它心跳期间的收缩(例如在QRS波群之后0-100ms)之后来启动闪光器14。在一个实施例中,心跳测量部件3操作在数据获取期间,但并不用作对数据获取的触发。相反,独立于心动周期获取数据。在数据后处理期间,根据其测量在心动周期内的位置接收(bin)速度测量。
图2是根据本发明的实施例的表示用于在固定于心动周期特定相位的时间上开始数据获取的技术的采样图。测量装置2(在该例中,积分器单元10或者心跳测量部件3)通过测量两个最近的心跳测量部件的输出来评定患者的瞬时心率。(或者,使用平均值或其它组合的最近心跳周期持续时间)。在图2中所示的第一心跳的末端处,测量装置2评定瞬时心率。该心率在附图中用其反向的持续时间B1来表示。典型地,预先对测量装置2编程以在心动周期的指定相位,即在该周期中的φ点处启动闪光器14,其中φ可定义为,例如紧随QRS波群的心动周期的大约10%或者75%。在心跳测量部件3感测QRS波群和部件3输出脉冲之间的时间A典型地是固定的。因而,在测量瞬时心率F1时,并且典型地在第二心跳的早期,积分器单元10计算数据获取触发时间(T),在该时间上将启动闪光器14并且开始图像获取。如果随后测量的第二心跳具有的持续时间B2靠近B1(例如,MIN(B1,B2)/MAX(B1,B2)<大约10%或20%),那么存储在时间T获取的图像数据。如果测量的第二个心跳具有的持续时间B2不类似于B1(例如,1-MIN(B1,B2)/MAX(B1,B2)>大约40%或50%),那么典型地拒绝或者留出(丢弃)在时间T获取的图像数据。或者,此外,如下文所述,识别出在B1和B2之间小但非平凡的偏差,并且对已记录数据应用纠正算法。
现在参照图3和图4,其是根据本发明的实施例的表示用于对速度数据进行纠正的技术的采样图。有时,积分器单元10确定心跳周期,在其期间所发生的数据获取比先前测量的心跳周期稍长或者稍短。换言之,B2(在ECG 2上,脉搏2和脉搏3之间的时间)比B1(在ECG1上,脉搏1和脉搏2之间的时间)稍长或者稍短。因而,由于第二心跳的2与第一心跳的1并不相同,因此将发生相应的相位误差。由于血流速度随心动周期函数而变化,因此该相位误差可能在血流速度测量中引入一些测量误差。即,由于心率的变化而在测量数据中存在一定量的“抖动”。
在减少相位误差的第一方法中,积分器单元10计算(典型地但并非必须在已经获取所有数据后的后处理期间)在心跳周期的持续时间B2,在其期间实际取得数据。据此,以及已经获得的信息,积分器单元10计算数据获取实际发生在心跳周期中的什么相位。将该测量结合先前搜集的有关在受试者中血液速度通常如何按照心跳周期函数变化的知识(例如,如图4所示),积分器单元10纠正测得的速度,使得若预先知道第二心跳的持续时间并且已经在第二心跳中的期望相位处进行测量,则大致反映将要测量的速度。
因而,例如,如果第二心跳比预期的短,那么事后会发现速度测量是在第二心跳中比期望晚的相位进行的。在图4所示的采样数据中,测得的速度比在期望相位处的速度低。为了估计期望相位处的速度,对该测得速率应用纠正因子。在图4所示的实例中,已知预定的基于人群的速度校准曲线,并且其用于产生纠正因子。
在减少相位误差的第二方法中,其可以与第一方法结合实施,评定心率的短期趋势,并且将其作为到积分器单元10的算法的输入,该算法预测随后心跳的长度。下面参照图5该方法的实例进行描述。
图5是根据本发明的实施例的表示用于减少相位误差的技术的采样图。对于许多个体(尤其是惯于久坐的患者),心跳周期时间并不随机地增加和减少,而是按照患者呼吸周期的当前相位的函数大致上是正弦式地。图5示出了反映此现象的采样数据。因而,通过分析患者心率的近期历史(例如,在图中从率R到R1),积分器单元10识别心率中周期性的上升和下降,并且预测依次下一个心跳(图中的R2)会比前一个快还是慢,以及大约快慢多少。
因外,对于某些应用,基于如上参照图2所述的瞬时心率的预测由一个近期的短期心率变化所取代或者补充(例如,以每个策略加权50%)。
图6是根据本发明的实施例的表示用于响应操作员命令对数据获取进行计时的技术的采样图。典型地,在检查确认患者准备好并且测量装置2准备好以及其相对于患者放置好之后,操作员输入命令,以便开始数据获取。期望避免在操作员输入命令以获取图像(例如,通过按开始按钮)和图像的实际获取之间有长时间的延迟。长时间的延迟有时易于使所获取数据退化(例如,患者的眼睛可能移动)。本发明的一些实施例不但提高心跳同步,如此处所述,而且使在操作员命令和数据的获取之间的延迟最小。
根据本发明的实施例,积分器单元10计算以连续、滚动的基础触发闪光器14的潜在次数。积分器单元10预先知道在接收到操作员命令时下一次数据获取将发生的时间,并且使本身持续准备好,如果可能的话,以在当前心跳周期期间,或者在下一个心跳周期期间获取数据。如果没有接收到操作员命令,那么积分器单元在到达触发点时(在图6中的时间1)并不触发光学测量部件以获取数据,如果在下一个心跳中接收到操作员命令(在图6中的时间A),那么不必要等待直到来自心跳测量部件的下一个脉冲,因为已经设置好触发器(到图6中的时间2)。这样,光学测量部件持续的预先准备好,并且因此从操作员命令到图像获取的等待时间通常是一个心跳周期或更少。
图7是根据本发明的实施例的表示用于减小在操作员命令和启动光学测量部件之间的等待时间的技术的采样图。在某种情况下,当适于启动光学测量部件以获取数据时,最好在心跳周期期间设置一个以上的“目标点”。例如,有长期的上述ECG尖峰信号,在其期间血流量率相当小地变化。在图7中,三个目标点被确定为1、2以及3。如图7中的采样图所示,在第一心跳期间没有接收到操作员命令,不过在1之后不久就收到了操作员命令。因此,产生触发以便在2处开始数据获取。利用类似于以上参照图4所述的那些的微小的正规化调整,或者简单地通过接受所引入的微小附加变化,在许多情况下,积分器单元10可以将在操作员命令和启动光学测量部件之间的等待时间减小。
应当注意的是,对于一些实施例,目标点占据了连续的时间范围,例如,从1延伸到3,而不是少量的离散点。对于其它实施例,若干目标点分布在心动周期的不同部分,以便于对例如最大和最小血流速度进行测量。
对于实施上述技术的实施例(例如,参照图7所述的实施例),积分器单元10典型地存储患者在数据获取期间的实际瞬时心率以及获取数据时心跳中的点(例如,QRS波群之后的毫秒数量)。
图8是根据本发明的实施例的表示用于提高相位准确度的技术的采样图。对于一些应用,心跳测量部件3包括心电图机(ECG)或者有关心脏活动的连续变化迹线的其它发生器,在一个实施例中,积分器单元10将在当前心跳周期(无论如何定义)的开始处开始的迹线的状态与在最近的完整周期期间输出的参考迹线、或者一些近期迹线的组合相比较。例如,比较可以包括在时间上延伸参考迹线直到其能最佳匹配当前部分迹线,以及记录需要延伸多少以获得该匹配。或者,将当前部分迹线延伸到能最佳匹配参考迹线。这种延伸的实例示于图8中。积分器单元10排列出延伸后的参考迹线和当前部分迹线,并且评定(a)当前心跳到最后一次有多长,以及(b)在当前心跳期间的什么时间制动光学测量部件以开始数据获取。
应当注意的是,尽管在此所述的实施例特别地涉及用于血流速度测量目标的血管图像获取的心动周期同步,而且本发明的范围包括数据获取与例如呼吸周期的其它可测量周期性生理参数的同步。(例如,可以使用上述的技术加以必要的变更,使对血氧水平的测量或者呼吸机的控制与患者自然呼吸周期同步。)类似地,本发明的范围包括将此处所述的技术应用于方便对在光学上可及的血管床上血流量的同步测量,这些血管床包括在:
·视网膜、结膜、巩膜外层以及舌(以及,通常任何表面可及的血管床);
·食道、胃、小肠以及结肠(以及,通常胃肠道的内表面或者任何血管通道);
·心脏、脑、肝脏(以及,通常任何手术可及器官的表面);
·任何能够通过提供照明和图像捕获的技术或装置(例如,导管、内窥镜、显微内窥镜或者腹腔镜)光学上接近的血管床。
本发明的范围包括用于减少在同步中的“抖动”和等待时间的其它技术,其对于阅读过本专利申请的本领域普通技术人员而言是显而易见的。
本领域技术人员应当领会到的是,本发明并不局限于以上所述和所示特定内容。更确切地,本发明的范围包括上述各种特征的组合和亚组合,以及现有技术中没有但是本领域技术人员根据阅读前述描述将发生的对其的改变和修改。
Claims (26)
1.装置,包括:
心跳测量部件,其感测患者的心功能参数,此外响应地产生心功能参数信号;
光学测量部件,其通过向所述患者的组织发射400-1000nm的光并且接收从所述组织反射的光来获取数据;以及
积分器单元,其接收所述心功能参数信号,此外作为响应,启动所述光学测量部件,以获取所述数据。
2.根据权利要求1所述的装置,其中,所述光学测量部件包括眼底照相机。
3.根据权利要求1所述的装置,其中,所述光学测量部件包括检眼镜。
4.根据权利要求1所述的装置,其中,所述心跳测量部件包括心电图机。
5.根据权利要求1所述的装置,其中,所述心跳测量部件包括脉搏血氧计。
6.根据权利要求1所述的装置,其中,所述心跳测量部件包括光学密度计。
7.根据权利要求1所述的装置,
其中,所述心功能参数信号包括指示心跳的数字脉冲,
其中,所述心跳测量部件产生所述数字脉冲,但并不在每次心跳时产生指示所述心跳的测量参数的附加信息。
8.根据权利要求1所述的装置,其中,所述组织包括所述患者的视网膜,并且所述光学测量部件接收从所述视网膜反射的光。
9.根据权利要求1所述的装置,其中,所述组织包括从由下列各物构成的群组中选出的组织,所述群组包括:结膜、巩膜外层、舌、表面可及的血管床、食道、胃、小肠、结肠、胃肠道的内表面、血管通道、心脏、脑、肝脏、外科手术可及器官的表面以及通过导管、内窥镜、显微内窥镜或腹腔镜可进入的血管床,
其中,所述光学测量部件接收从所选组织反射的光。
10.根据权利要求1所述的装置,
其中,所述心功能参数信号包括指示单次心跳的多个参数的变化迹线,
其中,所述心跳测量部件产生所述变化迹线。
11.根据权利要求10所述的装置,其中,所述积分器单元响应以下两项而估计所述当前尚未结束的心跳的持续时间:
(a)指示前一完整心跳的参数的变化迹线的一部分,
(b)指示当前心跳的参数的变化迹线的一部分。
12.根据权利要求1所述的装置,其中,所述积分器单元指定用于响应来自所述心跳测量部件的计时数据而启动所述光学测量部件的时间,所述计时数据指示先前心跳的持续时间。
13.根据权利要求12所述的装置,其中,所述积分器单元在当前心跳期间接收来自操作员的操作员命令,所述操作员命令指示已准备好获取数据,并且响应指示所述先前心跳的所述持续时间的所述计时数据,来启动所述光学测量部件,以在所述当前心跳期间获取数据。
14.根据权利要求12所述的装置,其中,所述积分器单元在当前心跳期间接收来自操作员的操作员命令,所述操作员命令指示已准备好获取数据,并且响应指示所述先前心跳的所述持续时间的所述计时数据,来启动所述光学测量部件,以在紧随所述当前心跳之后的心跳期间获取所述数据。
15.根据权利要求12所述的装置,
其中,所述指定时间包括多个指定时间;
其中,所述积分器单元指定适合于响应所述计时数据而启动所述光学测量部件的多个时间部件;以及
其中,所述积分器单元接收来自操作员的指示已准备好获取数据的操作员命令,并且响应所述操作员命令,来启动所述光学测量部件,以在一个所述指定时间获取所述数据。
16.根据权利要求12所述的装置,其中,所述积分器单元响应指示与多个前一心跳的持续时间有关的趋势的计时数据来指定所述时间。
17.根据权利要求12所述的装置,其中,所述积分器单元指定用于响应来自所述心跳测量部件的计时数据而启动所述光学测量部件的时间,所述计时数据指示前一心跳的持续时间。
18.根据权利要求17所述的装置,其中,所述积分器单元执行以下操作:
(a)在当前心跳期间,在所述指定时间启动所述光学测量部件;
(b)接着确定所述当前心跳的持续时间;
(c)如果所述前一心跳的持续时间与所述当前心跳的持续时间的差别小于阈值,那么根据第一协议来处理由所述光学测量部件获取的数据;以及
(d)如果所述前一心跳的持续时间与所述当前心跳的持续时间的差别大于所述阈值,那么根据第二协议来处理由所述光学测量部件获取的数据。
19.根据权利要求18所述的装置,其中,为执行所述第一协议,所述积分器单元将所述获取数据指定为好数据。
20.根据权利要求18所述的装置,其中,为执行所述第一协议,所述积分器单元处理和输出所述获取数据,以便显示给操作员。
21.根据权利要求18所述的装置,其中,如果1-MIN(Dp,Dc)/MAX(Dp,Dc)小于0.20,那么所述积分器单元确定所述前一心跳的持续时间(Dp)和所述当前心跳的持续时间(Dc)的差别小于所述阈值。
22.根据权利要求18所述的装置,其中,如果ABS(Dp-Dc)的小于50ms,那么所述积分器单元确定所述前一心跳的持续时间(Dp)和所述当前心跳的持续时间(Dc)的差别小于所述阈值。
23.根据权利要求18所述的装置,其中,为执行所述第二协议,所述积分器单元丢弃所述获取数据。
24.根据权利要求18所述的装置,其中,为执行所述第二协议,所述积分器单元纠正所述获取数据,并且输出所述纠正的数据,以便显示给操作员。
25.根据权利要求24所述的装置,其中,所述积分器单元分析所述获取数据,以产生血流速度指标,并且利用因子来纠正所述血流速度指标,所述因子基于在一个心动周期的多个相位的典型血流速度之间的已知关系。
26.装置,包括:
周期性生理参数测量部件,其感测患者的周期性变化参数,此外响应地产生周期性变化参数信号;
光学测量部件,其通过向所述患者的组织发射光并且接收从所述组织反射的光来获取数据;
积分器单元,其接收所述周期性变化参数信号,此外作为响应,启动所述光学测量部件,以获取所述数据。
