本发明涉及一种用于选择性治疗特定组织或器官局部的释放药物的医用装置以及制造这种药物涂布装置的方法。
许多疾病并不同时影响整个生物体而仅仅是局限于某些特定的组织,甚至通常是局限于非常有限的个别组织区域或器官局部。这样的实例比如肿瘤、关节和血管疾病。
这类疾病的药物治疗通常是通过口服或静脉给药进行的。口服或静脉给药分布于整个机体内,并在许多情况下在健康组织和器官中引起不希望的副作用,特别是当接受治疗的疾病处于严重期时。这些都使得其治疗应用受到限制。患病组织的选择性治疗既可以通过在保持给药途径不变的同时利用特定结合于该患病组织的药物(例如抗体)实现,也可以通过选择性给药直接注射入患病组织中或者通过导管向供养患病组织的血管进行供给而实现。在选择性给药的情形中,大多因为药效短和侵入式给药途径而产生问题,而任意地重复给药是禁止的。如果药物通过供养患病组织的血流进行选择性给药,还有一个问题就是当血液或活性成分溶液快速流经血管时药物不能被充分吸收。
迄今为止,出现通过各种缓释活性成分的药物制剂、释药植入体或者用于较长时间作用的选择性途径例如植入导管等,以试图解决上述问题。
已知可以将插入体内的医用装置尤其是导管的表面使用能够增强滑动性能或者防止血液凝结但不具有治疗效果的物质进行涂布。
另外,给导管配备将药物注入动脉壁的特定装置,例如经邻近血管壁的位置的导管壁上的穿孔采用针头或者注射高压。
另一原理是基于,将动脉壁和通过导管给药的活性成分制剂之间的接触时间按一下方式延长,即或者在相应的时间内阻塞血液流动,例如采用在两个气囊之间具有隔室的双气囊导管,所述隔室用药物溶液填充;或者通过气囊外壁的长形隆起物之间的空隙,由此可以维持血液以限制的量流经气囊所通过的通道。

在本发明上述提供的医用装置中,所用药物是紫杉醇以及其它紫杉烷类、雷帕霉素及其相关物质、他克莫司及其相关物质、肾上腺皮质激素、性激素及其相关物质、他汀类、埃博霉素类、普罗布考、前列腺环素类、血管生成诱导剂。
在本发明上述提供的医用装置中,所述亲脂性药物以干燥固体或油状物的形式出现在相应产品的表面上。
在本发明上述提供的医用装置中,所述药物的无定形结构有效剂量为粒径为<0.1微米至5微米,这样尽管活性成分微小的水溶性但由于所述结构表面积大而迅速溶解。
在本发明上述提供的医用装置中可以将所述亲脂性药物包埋入易溶于水的基质物质中以实现与装置表面良好地结合和提高组织的吸收率。
在本发明上述提供的医用装置中,所述基质物质由分子量<5000D的低分子亲水性物质组成。
在本发明上述提供的医用装置中可以将所述亲脂性药物吸附到颗粒上或者与低分子基质涂布到装置表面上。
在本发明上述提供的医用装置中,所述表面另外用影响特殊属性例如装置的滑动性的物质或者防止血液凝结的物质进行涂布。
本发明还提供一种制备上述医用装置的方法,其特征在于:将所述亲脂性药物和辅剂在溶液、悬浮液或乳液介质中通过浸渍、涂抹、喷雾或者借助容积测量设备涂布到所述装置的表面,去除过量介质和松散的粘附在表面上的物质。
该方法可以使用相同或不同的溶液、悬浮液或乳液介质和/或辅剂多次进行涂布过程以再现地提高活性成分含量。
该方法可以使用乙醇、异丙醇、乙酸乙酯、二乙醚、丙酮、二甲亚砜、二甲基甲酰胺、甘油、水或其混合物用作为溶液、悬浮液和乳液介质。
该方法可以使用基本上已准备好投入使用的经打褶的气囊用作为药物载体,所述气囊不带支架或者装配有支架、在灭菌之前或之后进行涂布。
该方法可以将所述气囊在展开状态下用相应的亲脂性药物涂布,且所述气囊使用特别润滑的任选地用生物可相容的润滑剂润湿的工具打褶。
该方法可以将与气囊导管相结合的支架安装于涂布之前或之后。

本发明以简易制备方法提供了改良的载药气囊导管或者类似的医用装置,它们具有多方面的通用性并且能使活性成分直接释放。出人意料地与当前公认观点相反,没有从惰性基质(聚合物、水凝胶、微囊等)中持续释放的活性成分,或者无需或不用活性成分的特定化学或物理状态。因此,不需要制造或控制贮存式制剂所使用的复杂技术。
根据本发明,导管上的气囊具有药物涂层是特别有用的,因为在血管或体内其它通道被气囊扩张之后,为了预防由气囊压力形成的内腔的再狭窄或闭塞、以及限制肿瘤生长或促进包括形成侧支循环的康复进程,都在治疗措施中是经常需要的。这可以通过药物在直接贴近气囊表面的区域施展药效而实现。在通向靶位的路径中(通常是高速流经血液的动脉),药物紧密粘附在气囊上直至气囊膨胀,之后在气囊与组织的短暂(经常只有几秒钟)接触时间内以有效剂量释放并被组织吸收,从而避免在气囊收缩后药物被即刻恢复的血流冲洗掉。
根据本发明,涂布的对象是用于引导导管的导丝、针头和导管以及至少在短时间内对患病组织施加压迫的导管局部。优选的导管材料是聚酰胺类、聚酰胺混合物和共聚物、聚对苯二甲酸乙二酯、聚乙烯及共聚物、聚氨基甲酸酯、天然橡胶及其衍生物。因为按照活性成分μg/表面积mm2计算剂量,导管或气囊的用于药物治疗的区域的长度和直径对于其应用而言并不具有决定性的重要作用。例如,直径为2-4mm、长度为1.0-4.0cm的气囊通常用于冠脉扩张。直径粗至>20mm、长度长达>10cm的气囊可用于其它血管。涂布表面可以是光滑的(即不具有用于承载活性成分的特殊结构)、粗糙的或者以任意的方式具有结构,而特殊的表面结构并不是活性成分粘附的条件,但也不会阻碍粘附。活性成分粘附于气囊表面仅仅是通过选择适宜的溶剂以及任选加入某些影响粘附的添加剂来实现的。出人意料地,甚至在完全光滑的气囊表面上这种粘附作用也是强烈的。
所有表面还可以附加地用改善产品滑动性能、防止血液在表面凝结或者改善该医用产品其它特性的物质涂布。但是所述用于涂布的材料并不一定要被释放入环境中,且所述附加涂层不会显著限制用于治疗靶组织的活性成分的释放并由此显著降低产品效能。
通过膨胀一段1cm至大约10cm长度的极细塑料管形成气囊导管。然后将该膨胀后的具有极薄内壁的气囊膜沿着导管轴打褶数次并在导管轴周围紧密缠绕,以使膨胀区域在打褶的状态下的直径仅仅是比导管其余部分略微粗一些。气囊膜的紧密打褶是为了便于使气囊导管通过引导口(Einf ü hrungsschleusen)、引导导管以及例如血管严重变窄的部分。
导管的气囊可以在打褶或展开的状态下被涂布,在此在每一种情况下均得到完整且足够均匀的表面涂层,即使当气囊导管表面在展开状态下被涂布然后重新打褶时活性成分也会牢固粘附于气囊导管表面上。
在展开状态下进行涂布气囊的制备而不产生涂层的损坏,例如通过采用被预先打褶和弯曲的气囊膜,所述打褶弯曲结构不会因为膨胀而消失,并且当气压从气囊中泻出后,气囊膜至少可以松散地重新打褶而不需要外部力量作为主要动力。只有在这样的重新打褶之后,预先形成的褶皱才会由外压或经真空而压缩。绝对不需要利用该褶皱来贮存活性成分。另外,通过非常光滑的材料利用微弱的机械压力可以完成打褶,所用工具也可以例如用光滑的、生物可相容的液体润湿,其中活性成分在该液体中不溶解或者至少不会很好地溶解。
根据本发明另一变形方案,将已经打褶的气囊导管的气囊通过在低粘度的活性成分溶液中浸渍而进行涂布。溶剂和活性成分渗入这些极其致密的褶皱中,在那里它们形成令人惊奇的均匀涂层和剂量可重现的涂层,并且不会被后续步骤所破坏。粘附在外表面上的溶液或者干燥溶剂后得到的粘附在外表面上的涂层可以保留在原位,或者通过另外的步骤除去,以使仅保留了遮蔽在气囊褶皱内部的活性成分部分。
涂布后,在打褶气囊的情形下,可以将支架(stent)推至气囊导管上并在其上面进行坚实的挤压。最后还需要例如使用环氧乙烷进行灭菌。
如上所述设计的工作步骤极其简单,不易受干扰,并且也能使用机械、化学和物理敏感性涂布材料进行。据观察,利用这种方法进行涂布不会引起任何不希望的褶皱与褶皱之间的松弛或粘附现象,同时按照这种方法涂布的活性成分粘附得足够的紧密而不会在途径中被血流冲洗掉,而当气囊在目标组织中膨胀时可以释放出绝大部分的活性成分。
适宜的药物是具有强亲脂性、几乎不溶于水以及当与任何组织成分结合时作用剧烈的药物。当药物的丁醇比缓冲水溶液(pH7)分配比例为0.5、优选为1、尤其优选为5时,或者当其辛醇比缓冲水溶液(pH7)分配比例为1、优选为10、尤其优选为大于50时,这样的药物被称为具有亲脂性。作为替代方式或者除此之外,这种药物应该以大于10%、优选大于50%、特别优选大于80%的百分比与细胞成分可逆或不可逆地结合。优选的是抑制细胞增殖或发炎进程的物质、或者抗氧化剂例如紫杉醇以及其它紫杉烷类、雷帕霉素及其相关物质、他克莫司及其相关物质、肾上腺皮质激素、性激素(雌激素、雌二醇、抗雄激素类)及其相关物质、他汀(statine)类、埃博霉素(epothilone)类、普罗布考、前列腺环素、血管生成诱导剂等。
所述物质优选以干燥固体或油状物的形式出现在各种医用产品表面上。优选具有最小粒径(通常<5微米、优选<1微米、尤其优选<0.1微米),尤其优选的是具有极细粒径的无定形非晶状结构,尽管所述药物基本上很小的水溶解性,所述结构一旦与组织发生接触之后由于其表面积大而迅速溶解而非以微囊起作用,也就是说它是自发并且迅速地溶解。以最小或无定形颗粒形式存在的有效剂量已经足够了;更大的颗粒几乎不对活性成分在组织中的浓度起作用但是也不会带来任何干扰。剂量取决于所希望达到的效果以及所用药物的效能。剂量可以高达5μg/mm2,但是该数值并不构成上限值。较小的剂量更容易掌握。
在提高组织的吸收率的情形下,通过将水溶性差的强亲脂性活性成分包埋入极易溶于水的基质物质中以实现与导管、针头或导丝表面良好地结合。适宜的基质物质是低分子量的(分子量<5000D,优选<2000D)亲水性物质例如用于各种医学诊断步骤中的体内用造影剂和染料、糖及其相关物质例如糖醇类、低分子量聚乙二醇、生物可相容的有机和无机盐类例如苯甲酸盐、水杨酸的盐及其它衍生物等。造影剂的实例有碘化X-射线造影剂和顺磁螯合剂,染料的实例有靛青绿、荧光素和亚甲蓝。辅剂也可以延长产品的贮藏能力,带来特殊的附加药理学效应或者有助于质量控制。
在本发明另一实施方案中,可以将药物活性成分吸收到颗粒上或者涂布到具有低分子基质的适宜医用产品表面上。适宜的颗粒同样是生物可相容的已知诊断剂例如亚铁盐以及各种超声检查用造影剂。
任意类型的辅剂都可以按照低于或高于活性成分的剂量使用。
利用上述药物和辅剂的溶液、悬浮液、或者乳液涂布医用产品。适宜的溶液、悬浮液或乳液介质例如乙醇、异丙醇、乙酸乙酯、二乙醚、丙酮、二甲亚砜、二甲基甲酰胺、甘油、水或其混合物。溶剂的选择是根据活性成分和辅剂的溶解性、待涂布的表面的润湿程度和对在溶剂蒸发后残留的涂层和颗粒的结构的影响以及其对表面的粘附力以及在极短的接触时间内活性成分向组织的迁移。
涂布的实现可以通过浸渍、涂抹、借助容积测量设备(Volumenmesseinrichtungen)的涂布或者在各种温度下、视情况溶剂蒸汽饱和(大气中)下进行喷雾。该过程可以多次,视需要也可以使用不同的溶剂和辅剂重复进行。
已打褶的气囊导管的气囊可以通过将其浸渍于含有活性成分的溶液中或者通过其它手段出人意料地均匀、可再现且可控剂量地涂布,而不会损坏导管的功能。当气囊被重复浸渍于不饱和的活性成分溶液中时,先前涂布的活性成分不会完全被溶解,相反,气囊的活性成分含量反而以可再现的方式增加。
涂层溶液中的过量溶液或松散地附着在表面上的过量物质,它们可以利用简单的方法除去而不会削弱涂层的性能。
根据本发明所定义和制造的各类医用装置与组织进行短时间即几秒钟、几分钟、或几小时的接触。在一些情况下希望直接用药物产品进行药物治疗该组织,例如防止作为对损伤的反应的过量生长或抑制肿瘤生长、促进新血管生成或者消除炎性反应。在所有这些情形中,利用上述方法可以获得令人吃惊的相当长时间的局部高药物浓度。其主要优势在于上述产品和方法的突出的多方面的应用可能性。
优选的应用是用于降低由气囊导管扩张引起的血管壁过度增殖。这在视需要植入的血管支架的区域同样是通过将支架用药物涂布而实现的,但是仅仅是针对被支架所覆盖的血管区域。涂层后的气囊导管也可以治疗支架前方和后方短距离内需要接受治疗的区域,它们可以治疗那些已经植入支架的区域而不必重新植入支架以及那些没有植入或者不能植入支架的血管。与长期释放药物的支架相比,有利的是较好的康复同时很好地抑制了过度增殖以及微小的血栓症风险。
下面将参照实施例就气囊导管的涂层、在血流中涂层的粘附、再狭窄抑制作用以及导管中的活性成分含量方面对本发明几个实施方案进行描述。
实施例1:
使用紫杉醇的乙酸乙酯溶液涂布膨胀后的气囊导管
将由BMT,Oberpfaffenhofen/慕尼黑,德国制造的、产品名为JokerLite、气囊尺寸为2.5mmX20mm的气囊导管膨胀至最大,然后全部长度在乙酸乙酯、18.8mg紫杉醇/ml、+1%药用橄榄油中浸渍1分钟,干燥:紫杉醇含量为39微克(用乙醇萃取后HPLC测得)。
实施例2:
用紫杉醇的乙酸乙酯溶液涂布褶皱气囊导管
将由BMT,Oberpfaffenhofen/慕尼黑,德国制造的、产品名为JokerLite、气囊尺寸为2.5mmX20mm的气囊导管全部长度在打褶条件下于乙酸乙酯、18.8mg紫杉醇/ml、+1%药用橄榄油中浸渍1分钟,然后干燥:紫杉醇含量为69微克。
实施例3:
用紫杉醇的乙酸乙酯溶液涂布褶皱气囊导管
a)将由BMT,Oberpfaffenhofen/慕尼黑,德国制造的、产品名为Joker Lite、气囊尺寸为2.5mmX20mm的气囊导管全部长度在打褶条件下于乙酸乙酯、16.6mg紫杉醇/ml中浸渍1分钟,然后干燥4小时:紫杉醇含量为54微克。
b)步骤相同,但是增加2次5秒钟的浸渍时间,每次在溶液A(=3.33ml乙酸乙酯+100.0mg紫杉醇)中浸渍后干燥时间为1小时;紫杉醇含量为126微克。
c)步骤相同,但是增加4次5秒种的浸渍时间,每次在相同溶液中浸渍后,干燥时间为1小时:紫杉醇含量为158微克。
实施例4:
用紫杉醇的丙酮溶液涂布气囊导管
将350mg紫杉醇溶解于9.0ml丙酮中;使由BMT,Oberpfaffenhofen/慕尼黑,德国制造的、产品名为Joker Lite、气囊尺寸为2.5mmX20mm的气囊导管膨胀至最大,全部长度浸渍1分钟后移走。溶剂在室温下干燥12小时。然后使气囊紧缩并用PTFE-涂布工具按照常规方法进行打褶。任选地,可以用适宜于支架的尺寸在气囊上形成褶皱:气囊上含有29微克的紫杉醇。
实施例5:
用紫杉醇的丙酮溶液涂布气囊导管
a)将打褶后的由BMT制造的、产品名为Allegro、气囊尺寸为2.5X20mm的气囊导管在含有0.15ml乙醇+4.5μl Ultravist300(由Schering AG,柏林,德国制造的X-射线造影剂)+1.35ml丙酮+0.8mg苏丹红+30.0mg紫杉醇的混合物中浸渍:导管的褶皱气囊部分浸渍5次,第一次浸渍1分钟,然后干燥3小时,然后每隔1小时浸渍4次每次5秒钟;随后将支架折皱,带有支架的导管用环氧乙烷按照常规方法灭菌:紫杉醇含量为172微克,利用HPLC检测不到活性成分的分解产物。
b)使用甘露醇的饱和水溶液替代Ultravist300。
c)使用水杨酸钠的饱和水溶液(pH7.5)替代Ultravist300。
d)根据(5a)向配备的溶液中加入5mg乙酰水杨酸。
e)根据(5a)向配备的溶液中加入5mg甘油。
实施例6:
在血流中活性成分的粘附
使用12支由BMT制造的、产品名为Allegro、气囊尺寸为2.5X20mm的气囊导管。各将6支导管的褶皱气囊部分分别在[0.15ml乙醇+4.5μlUltravist300+1.35ml丙酮+0.8mg苏丹红+30.0mg紫杉醇]中或者在[1.5ml乙酸乙酯+0.8mg苏丹红+31.0mg紫杉醇]中浸渍5次,第一次浸渍1分钟,干燥时间为3小时,然后每隔1小时浸渍4次每次5秒钟;随后每组中的各3支褶皱气囊在37℃的50ml人血中轻轻移动5分钟并取出用于测量紫杉醇含量;在血液中移动5分钟后的平均值(每一种涂布方法中n=3)相对于未曾在血液中的3支对照导管而言有所降低。

实施例7:
在猪的冠状动脉的血管成形术和植入支架后检测再狭窄抑制作用
将由BMT制造的Joker Lite型、3.5X20mm或3.0X20mm的褶皱气囊导管分别在下面的溶液中浸渍1分钟:
溶液A)3.33ml乙酸乙酯(EA)+100.0mg紫杉醇
溶液B)0.45ml乙醇+100μl Ultravist-370+4.5ml丙酮(Ac)+150.0mg紫杉醇
然后在室温下干燥过夜。第二天分别再进行浸渍步骤1次(低剂量=L)或4次(高剂量=H),每次间隔1小时每次5秒钟。
在溶液(B)中浸渍2次后活性成分含量平均为250μg,在溶液(B)中浸渍5次后活性成分含量平均为500μg,在溶液(A)中浸渍5次后活性成分含量平均为400μg。
使用这些用紫杉醇涂布后或者未被涂布的导管将支架植入总共22只猪的左前壁或侧壁冠状动脉中,使血管轻微地过度扩大以通过组织过度增生刺激再狭窄。这些动物5周后再次进行血管造影术,利用自动操作的计算机程序测量血管造影照片中的血管狭窄度。

在植入支架和从动物撤走后,将实施例8的长度大约为3cm的气囊从气囊导管上割断并置于1.5ml乙醇中。使用HPLC检测紫杉醇含量。对所有可利用的涂布气囊以及选择未被涂布的气囊进行测量。

实施例9:
将普罗布考以100mg/ml的浓度加入至丙酮中;按照在前面实施例中描述的方法使用该溶液涂布气囊导管。
实施例10:
将雷帕霉素以10mg/ml的浓度溶解于二乙醚中。按照在前面实施例中描述的方法涂布导管的气囊部分;从涂布溶液中移走之后,尽可能立即将气囊保持水平位置并沿着其纵轴方向连续转动。
实施例11:
将埃博霉素B以2mg/ml的浓度溶解于乙酸乙酯中;按照在前面实施例中描述的方法使用该溶液涂布气囊导管。

Claims (11)

1.用于药物释放以选择性治疗患病的组织局部或器官局部的气囊导管,其特征在于,紫杉醇以干燥固体物质附着在所述气囊表面,所述紫杉醇在组织接触后即刻以直至5μg/mm2的剂量释放,所述紫杉醇以粒度<5μm的无定形颗粒存在。
2.根据权利要求1的气囊导管,其特征在于,所述导管的材料选自聚酰胺类、聚酰胺混合物和共聚物、聚对苯二甲酸乙二酯、聚乙烯及聚乙烯共聚物、聚氨基甲酸酯和天然橡胶。
3.根据权利要求1的气囊导管,其特征在于,待附着紫杉醇的气囊表面是光滑的、粗糙的或以任意的方式具有结构。
4.根据权利要求1的气囊导管,其特征在于,所述紫杉醇以粒度<1μg的颗粒存在。
5.根据权利要求4的气囊导管,其特征在于,所述紫杉醇以粒度<0.1μg的颗粒存在。
6.根据权利要求1-5任一项的气囊导管,其用于短暂的组织接触。
7.根据权利要求1-5任一项的气囊导管,其用于冠状动脉扩张。
8.根据权利要求7的气囊导管,其特征在于,所述气囊具有2-4mm的直径和1.0-4.0cm的长度。
9.根据权利要求1-5任一项的气囊导管,其与支架连接。
10.根据权利要求1-5任一项的气囊导管的用途,其用于降低由气囊导管扩张引起的血管壁的增生。
11.根据权利要求10的用途,其用于不能植入支架的血管。
