发明内容
本发明提供一种为了治疗和/或诊断的目的通过将电信号或电磁场施加到胃的组织而治疗和/或诊断胃失调的装置、系统和方法。本发明也提供一种基于感测一个或多个参数而治疗和/或诊断胃失调的装置、系统和方法,该装置、系统或方法根据这些参数也就是在要求时刺激胃。诊断或治疗目的可以包括但不限于控制食欲、饱食、饮食习惯和/或肥胖,治疗反胃,促进或加速胃里的食物或流质的混合或分解,控制、促进或加速食物或流质穿过胃以及进入小肠的运动,以及刺激胃以延迟食物从胃进入小肠的通行。刺激可以影响平滑肌收缩、与胃关联的神经,以及/或者胃的生物化学或分泌物。

外壳11包括电子电路29。如图3中所示,刺激器10的电子电路29也包括与分立装置通信的遥测电路。
电子电路29接收感测信息,通过电极14,15将刺激电子脉冲提供给胃壁,以及与外部单元例如读取器、记录器或控制器遥测通信。刺激器10可以与这里所描述的电子电路29通信,或者与控制刺激器10的分立控制器(例如,图4的外部装置60中的控制器70)通信。
导线12包括分立的阻抗电极16,17,它们经由沿着导线12延伸的分立连接器而电连接到电子电路29。布置阻抗电极16,17,以当它们被询问时感测胃里食物的容纳物的阻抗。容纳物的阻抗提供关于已吞咽的食物或流质的信息。例如,脂肪典型地具有比碳水化合物高的阻抗。可以在有了食物已被咽下的指示之后,通过控制器30(图3)或控制器70询问阻抗电极。如这里所述的,这种指示器可以包括,例如温度的改变、pH的改变、胃收缩的改变,以及压力的改变。也可以以周期性为基础询问阻抗传感器。因此,可以将感测和功率支出限于需要感测的那些时候。

传感器18a-c,19每个都可以包含一个或多个传感器,它们提供关于病人的状况的反馈或与病人的肠胃系统相关的信息。例如,传感器可以包括加速计,它探测病人的总运动和/或呼吸运动,从中可以确定病人的觉醒状态。可以通过连到电子电路29或其他控制装置的导线或者通过遥测或其他通信模式,将传感器18a-c或19连接到电子电路29或连接到另外的控制装置。
布置传感器18a,18b和18c,以直接感测关于胃的信息。传感器18a-c可以包括但不限于以下的一个或多个:温度传感器,收缩传感器,压力传感器,应变仪,pH传感器,加速计,光学传感器。如上所述,传感器18a-c电连接到电子电路29或者与电子电路29通信。(可选地,它们可以与分立的或外部的控制器通信,例如与响应于一个或多个传感器所感测的信息而控制刺激脉冲的控制器70通信。)
刺激器10包括锚钉123和外壳11。锚钉123包括具有可扩展末端125的伸长元件124以及以抗腐蚀金属导体环形式的刺激电极126。可以使用刺激电极代替电极14,15的一个,或者除了它们之外还使用刺激电极。在伸长元件124上包含应变仪121,并且将它通过外壳11电连接到电子电路29。将应变仪121布置成接近锚钉123上的电极126,其中锚钉123用来将电极锚定到胃壁中。应变仪用作这里所描述的收缩传感器。例如,在此引用作为参考的美国专利6,535,764号中描述了这种刺激器的构造和植入技术。电子电路29通过电极将感测的刺激电子脉冲提供给胃壁,并且与外部单元例如读取器、记录器或控制器遥测通信。
图2说明根据发明的刺激器的可选配置。刺激器20包括以皮下方式植入病人内的外壳21。刺激器还包括从外壳21延伸通过腹部并到达胃100的导线22a,23a,其中从胃100的外部将在导线22a,23a的端部的电极22,23植入到胃肌肉层中。植入刺激器外壳21以及在腹腔镜辅助下将电极22,23植入胃100中的方法一般地对于本领域技术人员是众所周知的。

在使用中,一旦部署了刺激器(例如,10或20),那么通过电子电路29提供电刺激。电子电路29能够产生多种类型的可编程波形,这些波形将刺激提供给肠道的平滑肌衬。本领域技术人员众所周知有许多不同类型的电刺激程序和策略,它们能够用于通过电路29提供电刺激参数,而主焦点就是给胃提供电刺激参数。在一种实施方案中,电刺激的焦点是促成食物的胃潴留以产生饱食感觉。电刺激的另一个焦点可以是干预胃的天生蠕动,天生蠕动在这里意指典型地也用来分解食物以及/或者将物质移向腔房或移出胃的胃运动。另一个焦点是通过刺激胃而促成饱食感觉。另一个焦点是通过刺激胃而控制与胃或饥饿相关的分泌物。
在图3中说明电子电路29的实施方案。刺激器的电子电路29布置在外壳11中。电子电路29可以是以标准化芯片的形式,这种芯片可以与一种或多种传感器一起使用,这些传感器包括但不限于这里所描述的那些。电子电路29或类似的电子电路也可以包含在系统的分立植入的传感器或部件中。因此,可以配置多种部件,以通过遥测或类似的信号通知与其他部件通信。
电路29包括用于控制电子电路29的操作的微处理器或控制器40,内部时钟41,以及用于给电路29的各个部件供电的电池装置44例如一对锂碘电池。同样地,控制器40和电池装置44连接到电路的主要部件的每个,这将是本领域技术人员易知的。电池44使其输出提供给直流-直流转换器44a以提供更高的电压,该电压用于电刺激脉冲。直流-直流转换器44a是常规的,并提供刺激所要求的输出电压。内部时钟41也可以包括与微处理器40通信的实时时钟部件。实时时钟部件可以用来控制刺激,例如通过仅在一天的特定时间做出刺激或允许刺激。实时时钟部件也可以为探测的事件提供日期/时间戳记,这些事件被存储为存储设备中的信息。可以通过仅保存对应于所关注事件的信息来保护存储器,其中所关注事件的信息连同事件发生的时间/日期一起保存。

电路29也可以通过A/D转换器(具有放大器)46a,46b,46c,46d分别连接到一个或多个传感器18a-c和19,或者25,26,24a,24,121。A/D转换器将来自传感器的典型模拟电信号转换成传送给控制器40的数字信号。合适类型的这些传感器可以包括但不限于这里所描述的类型的传感器。在各个位置的这种传感器通过导线或者通过可选的通信方式例如遥测的无线通信或者间接地通过分立控制器例如控制器70而连接到电子电路。
控制器40通过用于寻址RAM50中的位置的地址总线50a以及用于传送信息给RAM存储器50并从那里传送来信息的双向数据总线50b而连接到RAM50。RAM存储器50包括事件存储器48,它临时存储由传感器18a-c,19,24a,24,25,26或者电极14,15;16,17;或22,23所记录的数据。RAM 50还包括可编程存储器49,可以例如通过外部编程器60对它编程。存储在可编程存储器中的数据可以包括电刺激操作模式的规范(例如,波形、用于定步、诱发、干预或反转收缩的,用于干预天生活动的,用于控制生物化学或与胃相关的分泌物的刺激类型,或者其他类型的刺激)以及各种过程或响应参数(例如,用于开启或关闭各个感测或刺激功能、参数修改、用于识别病人的胃肠道的各种状况的协议或过程,以及用于对这种识别做出响应的协议或过程)。这些数据以及过程/协议元件,包括对感测数据做出响应的响应元件也可以位于可独立于电子电路29而布置的其他控制器装置中。可以响应于感测的信息而执行编程,或者可以通过外部控制器自动执行,或者当需要时由治疗医师等执行。从传感器或电极获得的提供给控制器40的感测数据可以存储在RAM50中的事件存储器48中。与连续地发送相对,可以经由遥测线圈45将存储在事件存储器48中的数据间歇地发送为数据突发,以便节省电池电能。时钟也可以将存储在事件存储器中的数据做上标记或者打上日期/时间戳记。处理器也可以基于预先确定的阈值或特性来选择事件,它们将要被存储作为重要事件,而其他事件被过滤且不被存储。
电极14,15或22,23通过A/D转换器46e和46f连接到微处理器40。A/D转换器46e将电极14,15或22,23所感测的电EMG信号转换成代表EMG电活动的数字信号,其中该数字信号传送给微处理器/控制器40并存储到RAM50中的事件存储器48中。并且,A/D转换器46f将电极14,15或22,23所感测的并通过下面描述的阻抗电路53提供的电信号转换成代表组织阻抗的数字信号,其中该数字信号传送给微处理器并存储到RAM50的事件存储器48中。

可以对刺激器10或20进行编程,以响应于所感测的参数而传送电刺激。传感器18a-c,19,24a,24,25,26,121或电极14,15,16,17,22,23依赖于它们的具体位置而可以包括(但不限于):可以感测指示食物或流质的吞咽的温度改变或温度变化率的温度传感器;可以用来确定什么时候食物已被吞咽的pH传感器;可以用来确定食物的存在和/或组成物的光学反射器/传感器;可以用来感测运动模式例如收缩的存在、强度或频率的压力传感器;可以提供关于胃收缩和对刺激的局部响应的信息的收缩传感器;可以提供关于胃的容纳物的信息的阻抗传感器,以及/或者可以确定什么时候存在特征EMG模式以确定对象的觉醒的阻抗传感器;确定对象的活动水平或觉醒的运动传感器;提供关于与胃相关的生物化学组成物例如分泌物的信息的生物化学传感器。
响应装置可以包括至少一个传感器以及至少一个响应元件。从感测的信息,响应元件确定状况的存在,例如食物的存在;食物的吞咽;吞咽食物的类型;对象的活动水平;对象的觉醒;每日周期或日程的时间;胃的收缩等。
响应元件可以结合多个感测参数,以确定状况或环境的存在或者状况或环境的存在的可能性。响应元件可依据此确定治疗的方针,包括协议、刺激参数,以及是否做出刺激。在一种变化形式中,响应元件可以通过刺激而做出响应,以干预胃收缩,以减慢、停止或反转倾向于将食物移动通过胃的天生蠕动收缩。
例如,指示食物吞咽的可能性的温度改变以及指示对象没有在睡觉或者不是高度活跃的加速计的组合确定可以促使响应元件刺激胃,以将食物保留预先确定的一段时间。加速计可以确定指示睡眠状态的可能性的低水平活动,但是可能被感测到已咽下食物从而需要刺激的温度传感器驳回。当温度指示食物吞咽的可能性时,可以以类似的方式使用PH。定时器也可以给定的时刻确认正在吃食物的可能性,或者在某个时刻,例如当胃正在本能地清洗胃时,这典型地在晚上期间进行,定时器可以制止刺激而不管正在吞咽食物。
响应元件可以接收来自一个或多个传感器的输入,并且作为响应,响应元件可以询问另一个传感器的信息以确定行动方针。这可以用来节省电池或电能消耗。这也可以用来使用多于一个传感器确认状况或环境的存在。例如,一个或多个传感器可以提供食物已被咽下的信息。一旦做出该确定,那么可以触发另一个传感器,以确定已咽下什么类型的食物。例如,阻抗传感器可以通过测量胃的容纳物的阻抗而确定胃的容纳物的特性。光学发射器/传感器可以感测胃的容纳物的光反射/透射特性。可以将该信息记录到存储装置中以及下载。并且,当检测到某种类型的食物时,信息可以诱发由响应元件控制的刺激响应。另外,可以提供食物作为具有不同类型食物的标记的饮食养生法的一部分。可以产生某些食物的胃潴留,同时允许其他食物移出胃的运动。
图5说明由温度传感器在一段时间上所感测的示例处理温度信号。将温度传感器放置到胃中,以感测由于对象吞咽物质例如食物或流质而可能出现的温度或温度改变。传感器18a,18b,18c或26可以适合作为放置在胃里的传感器。
如图5中的示例感测信号中所说明的,温度位于y轴上,而时间位于x轴上。从时间t0~t1,所感测的温度相对恒定,处于大约37摄氏度的中心体温。在时间t1~t2之间,病人吞咽了暖食物,并且传感器感测随着时间流逝的特征温度,这由从时间t1~t2的温度信号所指示。从时间t2~t3,温度已再次回到正常。从时间t3~t4,温度的改变比从时间t1~t2更渐进得多并且改变到更小的程度。该温度波动没有触及温度阈值Th(高)或T1(低)。温度的绝对变化以及温度的变化率也小于从时间t1~t2的温度绝对变化和变化率。从时间t4~t5,温度再次近似为正常,其处于37摄氏度。从时间t5~t6,病人吞咽了冷物质,并且传感器感测随着时间流逝的特征温度,这由从时间t5~t6的温度信号所指示。
信号由控制器40处理,或者由电子电路29以遥测方式传送到外部编程器60。控制器40或控制器70可以以多种方式处理信号,以确定一个时段中的特征信号是否指示已吞咽了食物或其他物质。例如,从感测的温度信号中,可以得出或确定信号的随着时间流逝的温度变化或温度的绝对变化。如果变化是相当快的以及具有相当大的程度,那么可以确定已吞咽了食物或物质。因此,使用一个或多个温度参数,例如所感测的实际温度,温度的变化或温度变化率,可以做出对象已吞咽了物质的确定。另外地或可选地,可以观察所感测温度信号的其他特性,以推断该信号是物质的吞咽、某种类型的物质(例如,流质或食物)的吞咽、或物质的阈值量的特征。例如,可以将所感测的信号与特征信号比较,从该比较中,必要量的特征物质吞咽信号或与特征物质吞咽信号的相关程度可以促使处理器断定已吞咽了必要量的食物。然后,响应元件通过刺激或不刺激胃,通过增加、降低刺激,或者通过改变刺激参数而做出响应。
图6A和6B说明来自多轴加速计的一个轴的示意加速计信号。y轴对应于所检测的运动,而x轴表示时间。参考图6A,在时间ta0~ta1之间,活动状态是显示对应于睡眠和睡眠期间的呼吸的特征运动的最小值。在该时段期间,关闭刺激。从时间ta1~ta2,感测到活动的突发,然后在ta2~ta3之间再次感测到突发。从时间ta3~ta4,信号回到特征睡眠和睡眠呼吸模式。在该示范信号中,活动的突发对应于可能在睡眠期间发生的总运动。其中,可以通过观察在总运动发生之后的短时间内运动返回到典型的睡眠模式来确定这一点。对电子电路29的控制器40(或感测数据所供往的其他控制器或处理器)进行编程,以识别睡眠呼吸运动以及对应于睡眠期间的运动的总运动。程序可以将多个信号参数比较,并且找出某种程度的相关性,以从中做出状况的确定。这种状况可以包括例如睡眠、休息而不是睡眠、睡眠期间的总运动,或使力水平。例如,睡眠呼吸模式可以包括对应于睡眠的呼吸速率或模式。如果总运动相对简短并且信号返回到睡眠呼吸模式,那么可以对控制器进行编程,以将总运动识别为“睡眠期间的总运动”。如果运动相当快并且处于相对较高的幅度,那么可以对控制器进行编程,以确定存在较高的使力水平。
参考图6B,在时间tb0~tb1之间,活动状态是最小值以及睡眠的特征。在该时段期间,关闭刺激。从时间tb1~tb2,感测到活动的突发,并且从时间tb2~tb3再次感测到总运动。从时间tb1~tb2以及从时间tb2~tb3的运动是觉醒期间的运动特征。信号不返回到特征睡眠运动模式。对控制器进行编程,以将该总运动或这种总运动在一段时间的持续识别为觉醒状态期间的活动。控制器也可以将总运动与探测的来自胸腔运动的呼吸进行比较,以确认对象的觉醒状态的判断。从时间tb2~tb3的运动更快,并且具有指示更大使力水平的更大幅度。如果活动持续相当长的时间,那么处理信号的控制器或处理器可以例如将活动水平识别为高的使力水平。然后控制器可以相应地控制信号,以在高使力期间将信号关闭。可以对控制器进行编程,以识别加速计曲线的多个参数,包括运动的变化率、运动信号的幅度,以及具有特征信号的其他类型动作。
除了将特征例如食物的吞咽包含在编程装置响应内以外,也可以感测胃的天生特性并用来做出与刺激相关的决定或者以控制或修改刺激。例如,可以周期性地观察天生的胃收缩。没有食物摄取时,如果收缩增加,那么可以确定对象正在变得饥饿或将要变饥饿。可以对响应装置编程,以通过控制胃收缩例如通过干预收缩,而对这种指示性收缩模式做出响应。可选地或另外地,生物化学传感器可以用来识别与饥饿相关的生物化学物质例如激素(例如Ghrelin)或者其他胃分泌物例如酶或酸性组成物的存在、不存在或数量/浓度。响应装置可以通过做出刺激而对信息做出响应。也可以将信息存储和发送给外部装置,或者在随后时间下载,以允许对象做出其他应对或者随着时间的过去观察对象模式。
参考图7,说明了光学传感器80,它从胃里的刺激器10的外壳11延伸出来。光学传感器80包括末端81,配置该末端81以照明和感测胃里的食物或其他物质的光学特性。光学传感器80通过探测对应于胃里物质的各种组成物的光吸收、反射或激发特性而使用光谱学。
根据发明的一种实施方案,传感器光源发射和探测某些波长的光或光的不存在。根据该实施方案,传感器80包括至少一个光源,例如发射白光或者一种或多种特定波长光的LED或光纤源。光源84由控制器控制,控制器将短的光脉冲引导到肠道中或胃的容纳物处。传感器80还包括用于感测反射或透射光的至少一个传感器。反射或透射光指示作为某些材料或物质的光特征的某些波长的特定反射或吸收。当光电二极管感测到或没有感测到足够量的对应于特定波长的光时,确定出对象的激发特征以及/或者光电二极管所敏感的特定波长(无反射)光的吸收。传感器80通过处理电路59连接到处理器40(图3)。在接收到来自处理器40的指令时,处理电路59加强光纤或LED。处理器59也接收来自传感器的光感测部分的对应于感测的反射或透射光的信号,并且将这些信号转换成传送给处理器40的数字信号。处理器40或处理器70从电子电路29接收相关数据,基于所感测的反射或透射光而确定是否存在某些物质或组成物。
响应于感测到具有特定组成物的食物的存在,装置可以做几件事情的一件。例如,装置可以基于探测到的食物组成物而刺激胃,以保留食物。如果探测到不是预先批准的饮食养生法的一部分的物质例如脂肪、碳水化合物或碳水化合物类型(例如,简单的或复杂的),那么装置可以刺激胃以提供饱食感觉或轻微不舒服的感觉。
在一种实施方案中,用标记物例如荧光标记物产生预先批准的食物。如果吃下未经批准的食物,那么装置通过刺激胃以产生不舒适的感觉而做出响应。这样,将个体培养成憎恶未经批准的食物。
可以通过传送控制信号给处理电路59的控制器40而询问光学传感器80,处理器40询问传感器80以感测胃的容纳物。为了节省装置电能,可以仅当例如用如上所述的温度传感器感测到食物的吞咽时才传送该控制信号。在一种实施方案中,在感测到物质的吞咽之后立即询问传感器80,使得可以在物质被消化酶初次分解之前感测物质的组成物。为了饮食跟踪的目的,装置也可以记录可以在一段时间之后下载的感测信息的日志。
参考图8,说明了光学传感器80的末端81的一种变化形式81a。末端的变化形式81a包括光发射器探测器821-n,发射器/探测器821-n的每个包括光纤或光传感器(例如,光电二极管),它们发射以及探测特定的相应波长的光λ1-n(其中n是正整数)。发射器/探测器821-n可以可选地包括LED和光传感器。发射器/探测器821-n照亮胃的容纳物,然后对于特定波长的光λ1-n探测胃的容纳物的作为结果的光反射或激发特征。由处理电路59将感测的光转换成典型信号,其中处理电路59以本领域技术人员易知的方式处理感测的光信息。
参考图9,说明了光学传感器80的末端81的一种变化形式81b。末端81的变化形式81b包括形成开口87的凹入部分86,其中开口87用于临时容纳胃100的容纳物的一部分。开口87包括近端84a和远端85a。末端81的该变化形式81b包括位于凹入部分86上在开口的远端85a处的光电二极管探测器85,以及在凹入部分86上在开口87的近端84a处的LED光源84。在该实施方案中,使用了白光源。将光源84和光电二极管探测器85布置成彼此相关,使得由光电二极管探测器85接收和感测从光源84发射出的光。光电二极管探测器85可以包括探测器或滤光器的阵列,其中每个探测器或滤光器探测特定波长或波长范围的光。可选地,具有预先确定波长的多个LED发射器(例如,具有滤光器)可以用来照亮胃。特定波长的吸收可以用来确定各种组成物的存在或不存在。
探测器的阵列连接到处理电路59,其中处理电路59连接到处理器40。配置处理电路59,以例如基于选择的诊断模式选择对应于待探测波长的传感器或滤光器。处理器40可以选择要感测的特定物质或食物等。这可以编程到处理器40中,或者可以用刺激器系统在治疗或诊断过程中修改。处理器40指示处理电路59,以促成光发射,然后感测透过位于开口87中的物质的光。
在使用中,将胃中的食物或其他物质收聚到光从那里透过的开口87中。光电二极管传感器85感测透过开口87中的食物或物质的光。选择光电二极管探测器85,以探测不同波长的光。食物具有可用来识别食物的组成物的特征光吸收。可以从信号中过滤掉胃组织的光特征以及相应的光吸收或反射信号,只留下对应于胃的容纳物的信号。作为结果的感测光信号由处理电路59处理,其中处理电路59将对应于光电二极管探测器85所感测的透射光的典型数字信号传送给处理器40。
如上所述,配置电子电路29,以接收指示光学参数的感测信号,例如指示某种食物的存在的信号。将感测的信号传送给处理器40,其中处理器40将代表感测信息的信号经由遥测线圈45传送给外部控制器/处理器70。信息可以是例如将每个传感器的感测光信息结合的复合信号的形式,或者可以是对应每个传感器的时间间隔信号。
在另一种实施方案中,装置通过感测压力而感测收缩。胃里的压力一般地随着收缩速率的增加而增加。例如,大约250-300mm汞柱的压力指示对应于较高的胃活动水平的收缩水平,它将有益于抑制以便控制饥饿。如果病人处于要抑制食欲的状态,那么一旦感测到收缩则刺激胃以减小收缩从而减小饥饿阵痛。可以将压力传感器放置在胃中,比如像传感器18a,18b或26。可以将压力传感器放置在外壳中,其中外壳被构造成具有足够薄的外壁以允许在外壳内感测到压力改变。
在另一种实施方案中,收缩传感器被放置在胃壁中或胃壁上,比如像传感器18a,18b,18c,25或26,121,并且感测胃收缩。响应于感测到收缩,响应元件可以使胃被刺激,以通过例如减小收缩或反转收缩而控制胃收缩。
在睡眠期间,胃经历清洗过程。可以通过睡眠周期的压力改变和emg信号特征来观察该过程。因此,可以监测胃emg,并且当存在这种信号时可以防止刺激。睡眠的其他指示器例如来自加速计的指示可以用来确认对象的睡眠状态。
实时时钟也可以用来例如基于每日日程确定时间,以做出刺激或不做出刺激。例如,实时时钟可以与睡眠传感器结合而确定什么时候关闭刺激,例如当推测对象正在睡眠时,以及当所感测的参数指示病人事实上在睡眠时。可以将刺激关闭一顿饭的时间,以允许对象吃饭而没有蠕动信号的干预,以及允许食物的正常消化。
已关于优选实施方案,特别地关于胃刺激器描述了发明,本发明设想了传感器的多个组合可以用来确定病人的状态或该病人的胃肠道,以及作为响应确定是否、什么时候以及如何刺激胃。多种不同的信号以及信息处理技术可以用来达成刺激协议或其修改。可以在传感器处理器和刺激电极之间使用多种不同的通信方案。并且也可以根据多种技术和装置而改变为装置的植入部分供电的方法。
响应元件或响应装置可以包括放置在一起或分立放置的一个或多个部件。例如,可以将信号处理部件放置在植入的装置中,并且可以将用于确定特定状况的存在的控制器放置在与信号处理部件之间或间接通信的分立部件中。
其他仪器和装置可以用来响应于特定刺激位置处的刺激,而确定局部于刺激位置处的刺激参数。参考图11A和11B,内窥镜仪器480用来绘出胃壁中的电活动,以及用来识别和表征胃壁对各种电刺激参数的响应。仪器480包括细长的柔性元件481,柔性元件481一般地由蛇形管482制成,其中腔管483延伸穿过蛇形管482。将端部效应器484布置在仪器480的末端。端部效应器484包括通过铰链485连接在一起的电极元件486,487。电极元件486,487包括布置在元件486,487的端部的电极488,489。电极488,489通过延伸穿过电极元件486,487的导体490,491而连接到导线492,493,其中导线492,493穿过仪器480中的腔管483延伸到近身布置的手柄499。导线492,493连接到外部刺激器/记录器单元498,其中单元498通过导线492,493将刺激能量提供给电极488,489,并且通过导线492,493记录由电极感测的电活动。机械牵线494连接到铰链驱动装置495,并且穿过腔管483延伸到手柄499。电极元件486,487最初布置在紧靠的位置上。当使用手柄499向端部移动牵线494时,铰链驱动装置495关于铰链485转动电极元件486,487以展开电极元件486,487,从而电极488,489彼此远离。在这种情况下(图11A-11B),可以将电极放置在胃壁上在期望的位置处,以测量和记录电活动、电参数,或者提供电刺激脉冲到胃壁上。一旦将刺激脉冲提供给胃壁,则可以在视觉上或者通过布置在端部效应器484上的感测肌肉收缩的传感器(没有显示)例如应变仪来观察胃的响应(例如,收缩的存在、不存在或程度)。可以通过感测电活动、电参数,或者通过观察其中刺激导致期望响应的位置来确定用于附着刺激装置的理想位置。并且,也可以通过观察位置对于端部效应器484所传送的各种刺激参数的响应来确定装置的理想刺激参数或程序。
图10是说明根据发明一个方面的刺激器的操作的流程图。如本领域技术人员容易明白的,图10中所陈述的刺激器的操作可以用易于认出的结构元件来实现,其中这些结构元件包括例如与存储装置中所保存的程序协作的CUP或处理器。刺激器首先刺激在刺激位置处的组织301。与刺激位置紧密接近的局部传感器感测在刺激位置处的局部收缩响应302。如果收缩还没有达到例如预置或预先编程到刺激器中的合适收缩阈值,那么重复刺激301和感测302步骤。如果收缩已经达到阈值303,那么在阈值处或高于阈值的预置量处设置刺激305。然后,刺激遵循某个协议例如这里所描述的协议,其中在某组环境下以及为了某个目的例如为了控制食欲而提供刺激306。在预先确定的时间,一旦发生预先确定的事件,或者在编程器的要求下(例如,由对象或供应商通过外部控制器),对指示可能需要重新调整刺激的标记进行检查307。这种标记可以包括预置的时间流逝、预先编程的一组情况的出现(例如基于定义刺激协议或识别环境的算法,其中该算法可能指示没有有效地传送刺激)。如果标记指出不需要刺激的重新评估308,那么刺激器返回到步骤306,其中再次遵循刺激协议。如果需要刺激的重新评估308,那么如上所述地重复刺激步骤301、感测步骤302,以及刺激参数设置步骤305。根据发明的刺激器可以包括对于独立确定的参数独立地测试和设置的多个电极以及传感器。
虽然已关于优选实施方案描述了发明,但应当明白可以在下面权利要求的范围内做出变化和修改。这种修改可以包括换入以基本相同的方式执行基本相同的功能的元件或部件,以获得与本发明可以用属于下面权利要求的范围内的修改实现的基本相同的结果。
Claims (14)
1.一种可植入的胃传感器装置,包括:
被配置为放置在胃内部的传感器,胃内部被胃壁包围,所述传感器包括温度传感器并且被配置为感测对应于胃的容纳物的信息,其中胃的容纳物包括由对象吞咽的物质;以及
处理器,所述处理器连接到所述传感器,并且被配置为接收对应于胃的容纳物的信息以及由在胃内部当中所感测到的温度的变化率来确定何时所述物质被对象吞咽。
2.根据权利要求1的可植入的胃传感器装置,其中对应于由对象吞咽的物质的信息包括对应于胃里的食物的存在的信息。
3.根据权利要求1的可植入的胃传感器装置,其中对应于由对象吞咽的物质的信息包括对应于胃里存在的食物的组成物的信息。
4.根据权利要求3的可植入的胃传感器装置,其中所述传感器包括阻抗传感器,并且其中对应于由对象吞咽的物质的信息包括阻抗信息。
5.根据权利要求3的可植入的胃传感器装置,其中所述传感器包括光学传感器,并且其中对应于由对象吞咽的物质的信息包括光学信息。
6.根据权利要求1的可植入的胃传感器装置,其中所述传感器包括光学传感器。
7.根据权利要求1的可植入的胃传感器装置,还包括与所述处理器通信的响应元件,其中响应元件被配置为对感测的对应于由对象吞咽的物质的信息做出响应。
8.根据权利要求7的可植入的胃传感器装置,还包括被配置为传送电信号给胃的电刺激装置,并且其中所述响应元件被配置为响应于感测的对应于由对象吞咽的物质的信息而控制所述电刺激装置传送电信号。
9.根据权利要求7的可植入的胃传感器装置,其中所感测的信息对应于至少一种类型的食物,并且其中所述响应元件被配置为对感测到该至少一种类型食物的存在而做出响应。
10.根据权利要求1的可植入的胃传感器装置,其中所述处理器被配置为从传感器所接收的温度信息来确定吞咽量,并且进一步包括电刺激装置,电刺激装置被配置为响应于吞咽量超过阈值而将电信号传送给胃。
11.根据权利要求1的可植入的胃传感器装置,其中所述处理器被配置为响应于来自另一个传感器的信号而驱动所述传感器感测对应于胃的容纳物的信息。
12.根据权利要求11的可植入的胃传感器装置,其中所述传感器包括光学传感器,并且所述另一个传感器包括温度传感器。
13.根据权利要求1的可植入的胃传感器装置,还包括被配置为感测对应于对象的活动水平的信息的传感器。
14.根据权利要求13的可植入的胃传感器装置,其中对象的活动水平对应于觉醒状态。
