Abstract

技术领域

背景技术

对脊柱实施的外科手术之一涉及使用椎间盘假体替代(或更换)一个或多个椎间盘。椎间盘替代中涉及的诸问题中的一个是恢复脊柱对准。在各种脊柱手术(包括涉及椎间盘替代的那些手术)中需要改进的方法和假体以用于恢复脊柱对准。
发明内容

为了更好地理解本发明的原理,下面将参考图中示出的实施例,并使用特定语言描述所述实施例。但是要理解的是这不是对本发明的保护范围的限制,本发明所涉及技术领域内的普通技术人员通常容易想到的在图示的装置中的那些替代和进一步修改,以及在此说明的本发明的原理的那些进一步应用,也在本发明的保护范围内。
本发明总的涉及一种特别构造用于脊柱的颈椎区域的人造面关节。所述人造面关节优选地包括上构件和下构件,所述上构件和下构件彼此配合以大体上模仿相邻椎骨之间正常的生理运动。人造面关节的植入不是必然需要去除天然面关节的相当大的部分。相反,上构件和下构件优选地构造成只替代天然面关节的关节连接表面部分,且天然面关节的剩余部分维持原样。而且,人造面关节不是必须构成体内类型的装置,而是相反可以安装在椎间盘空间的外部。另外,人造面关节可以用于脊椎颈段的单侧或双侧治疗。
参照图1,在一个实施例中,上构件20和下构件21每一个优选地包括大体上平坦或轻微弯曲的关节连接部分22、25,所述关节连接部分22、25分别限定关节连接表面;和安装凸缘部分30和31,所述安装凸缘部分30和31分别从关节连接部分横向延伸,以便通过一个或多个接骨螺钉36、37、40和41后部连接到各个脊椎骨的侧块32和35。上/下构件的关节连接部分和安装部分优选地具有相对薄的板状构造。与限定球窝式构造相反,人造面关节的上构件和下构件的配合关节连接表面优选大体上是平的或平坦的或轻微弯曲的。配合关节连接表面中的一些轻微的弯曲,尽管不是优选(less preferred)的,也认为在本发明的保护范围内。要理解的是,在具有上构件和下构件的实施例中,这种非常轻微的弯曲优选地在两个构件之间匹配,并且这种弯曲可能限制可能的关节连接的范围。如此,上关节连接部分与下关节连接部分之间的接触面可以描述为具有“屋瓦式”交叠布置。

本发明的用于修复相邻椎骨的颈段面关节的方法,包括:设置上构件,所述上构件具有大体上平坦或轻微弯曲的第一部分和安装部分;设置下构件,所述上构件具有大体上平坦或轻微弯曲的第二部分和安装部分;将天然面关节的关节连接表面只去除足以允许以交叠的关系将第一和第二部分插入其间的量;然后将第一和第二部分插入到所述表面之间,且第一和第二部分为交叠的关系;通过将安装部分连接到相邻椎骨的后弓上,所述构件优选地固定到相邻椎骨上。
去除天然面关节的天然关节连接表面的步骤可以利用各种手术器械或工具完成。应该选择所使用的器械,以便使得可以达到关节的想要的精确尺寸。所述器械可以是摆锯或锯。并且可以使用高速圆锯。

Claims (27)
1、一种用于修复颈椎面关节的假体,包括:

下构件,所述下构件具有大体上平坦或轻微弯曲的第二部分以及安装部分;
所述安装部分适于连接到相邻的颈椎,且所述第一和第二部分在相邻颈椎的面之间彼此关节连接。
2、根据权利要求1所述的假体,其中所述平坦或轻微弯曲的第一部分以大约45度的角度连接到所述上安装部分。
3、根据权利要求1所述的假体,其中所述平坦或轻微弯曲的第二部分以大约135度的角度连接到所述下安装部分。
4、根据权利要求3所述的假体,其中所述平坦或轻微弯曲的第一部分以大约45度的角度连接到所述上安装部分。
5、根据权利要求1所述的假体,其中所述平坦或轻微弯曲的第一部分以45度的角度连接到所述上安装部分。
6、根据权利要求1所述的假体,其中所述平坦或轻微弯曲的第二部分以135度的角度连接到所述下安装部分。
7、根据权利要求6所述的假体,其中所述平坦或轻微弯曲的第一部分以45度的角度连接到所述上安装部分。
8、根据权利要求1所述的假体,进一步包括第一骨固定部件,所述第一骨固定部件用于将所述上构件连接到椎骨;第二骨固定部件,所述第二骨固定部件用于将所述下构件连接到椎骨;和柔性系绳系统,所述柔性系绳系统连接所述第一骨固定部件和所述第二骨固定部件。
9、根据权利要求9所述的假体,其中所述柔性系绳系统包括柔性的细长部件,所述柔性细长部件固定到所述第一骨固定部件和所述第二骨固定部件。
10、根据权利要求9所述的假体,其中所述骨固定部件是螺钉。
11、一种用于修复颈椎面关节的假体,包括:
具有大体上平坦或轻微弯曲的部分和安装部分的构件,所述大体上平坦或轻微弯曲的部分,适于替代两个相邻颈椎之间的关节连接的面关节的面中的一个面的关节连接表面。
12、根据权利要求11所述的假体,其中所述平坦或轻微弯曲的部分以大约45度的角度连接到所述安装部分。
13、根据权利要求11所述的假体,其中所述平坦或轻微弯曲的部分以45度的角度连接到所述安装部分。
14、根据权利要求11所述的假体,其中所述平坦或轻微弯曲的部分以大约135度的角度连接到所述安装部分。
15、根据权利要求11所述的假体,其中所述平坦或轻微弯曲的部分以135度的角度连接到所述安装部分。
16、一种用于修复相邻椎骨的颈椎面关节的方法,包括以下步骤:
(a)设置上构件,所述上构件具有大体上平坦或轻微弯曲的第一部分,以及安装部分;
(b)设置下构件,所述下构件具有大体上平坦或轻微弯曲的第二部分,以及安装部分;
(c)将面关节的面的关节连接表面只去除足以允许以交叠的关系将所述平坦或轻微弯曲的部分插入所述面之间的量;
(d)将所述平坦部分插入到所述面之间,且所述平坦或轻微弯曲的部分为交叠的关系;和
(e)通过将安装部分连接到相邻椎骨的后弓上,将所述构件固定到相邻椎骨上。
17、根据权利要求16所述的方法,其中所述平坦或轻微弯曲的第一部分以大约45度的角度连接到所述上安装部分。
18、根据权利要求16所述的方法,其中所述平坦或轻微弯曲的第二部分以大约135度的角度连接到所述下安装部分。
19、根据权利要求18所述的方法,其中所述平坦或轻微弯曲的第一部分以大约45度的角度连接到所述下安装部分。
20、根据权利要求16所述的方法,其中所述平坦或轻微弯曲的第一部分以45度的角度连接到所述上安装部分。
21、根据权利要求16所述的方法,其中所述平坦或轻微弯曲的第二部分以135度的角度连接到所述下安装部分。
22、根据权利要求21所述的方法,其中所述平坦或轻微弯曲的第一部分以45度的角度连接到所述下安装部分。
23、一种用于修复第一椎骨与第二椎骨之间的颈椎面关节的方法,包括以下步骤:
(a)设置一构件,所述构件具有大体上平坦或轻微弯曲的部分,以以及安装部分;
(b)将第一椎骨的天然面表面只去除足以允许以与第二椎骨的天然面表面成交叠的关系插入所述平坦或轻微弯曲的部分的量;
(c)以与第二椎骨的天然面表面成交叠的关系插入所述平坦部分;和
(d)通过将安装部分连接到第一椎骨的后弓上,将所述构件固定到第一椎骨上。
24、根据权利要求23所述的方法,其中所述平坦或轻微弯曲的部分以大约45度的角度连接到所述安装部分。
25、根据权利要求23所述的方法,其中所述平坦或轻微弯曲的部分以45度的角度连接到所述安装部分。
26、根据权利要求23所述的方法,其中所述平坦或轻微弯曲的部分以大约135度的角度连接到所述安装部分。
