技术领域
本发明涉及一种用于向气体输送水蒸汽的装置和方法。具体地讲,本发明涉及一种低流量蒸汽传送筒(low flow vapor transfercartridge),其可选地允许水进入大约每分钟1升到大约每分钟8升范围内的气体流速下的气流,而不会传送液态水。低流量筒起到全部或部分抵挡细菌的屏障的作用,同时允许水蒸汽通过。
背景技术
已经认识到输送氧气和富氧空气到病人的呼吸道时会经常引起病人不舒服,特别是在空气输送较长时间时。还已经认识到输送具有相对较低的绝对湿度的空气可导致呼吸系统不适。
已经提出过一些装置来克服这些问题。美国专利文献2003/0209246A1描述了用于呼吸道治疗的装置和方法的实施例,其适合加热及湿润空气,目的是向病人的呼吸道输送加热和湿润的空气,所述文献被全部以引用的方式结合于此。如美国专利文献2003/0209246A1中公开的这类装置代表了对现有技术装置的一种改进。

发明内容

具体实施方式
在下述详细说明中,将描述本发明中与给病人输送湿润气体有关的特征。图3提供了系统实施例的完整示意图。图1和2示出了可在如图3中示出的系统中使用的蒸汽流动筒的实施例。

虽然本发明的代表性实施例参照的是将加热和湿润的空气输送到病人。但是本发明不限于输送空气。同样,空气或其它气体可在输送到病人之前被加热或不被加热(但优选是被加热)。最后,根据本发明的代表性实施例通过向气体输送水蒸汽实现湿润空气或其它气体。但是,也可考虑,除了水蒸汽,还附加地输送其它蒸汽到气体,或者用其它蒸汽代替水蒸汽。
下面将详细描述一种热—湿交换筒,其优选为不必将系统从IV输液架拆开或移除就能被触及以实施维护。根据本发明的装置可使用的交换筒的一个例子是由Vapotherm,Inc.提供的部件号为VT01-B的部件。这种容器也可考虑其它结构,目的是增加表面积并减少压降。一种优选筒的中空纤维具有大约55到大约60微米厚度的壁。当然也可使用其它的中空纤维。
现在参照图3描述适于输送加热及湿润气体的另一种装置的代表性特征。参照图3中提供的示意性图,装置400包括供应单元组件402和输送管组件404,后者适合可拆卸地连接到供应单元组件402。供应单元组件402设有进口406,用于从壁式气源(wallsource)或者从压缩机、气罐或者其它供应源接收气体。在较低流量应用中,最优选地使气体具有大约1升/分钟到大约8升/分钟的流速,但是也可考虑用更高或更低的流速。
进口406的下游是气体截流电磁阀408,用以在需要时阻止气体流动。设置交换器410,以致利用水和气体逆向流过交换器410来湿润气体。泄漏检测器412和压力传感器414设置在交换器410的下游。接着,气体向外行进通过输送管组件404,目的是供给加热、湿润的气体,如标记“A”处所示。
供应单元组件402被构成可从水袋416接收水。迫使水流过供应单元组件402的泵418可以设有12伏DC电源。压力传感器420设置在泵418的下游以检测系统中水的压力。接下来,水在加热器422中被加热,所述加热器可设有115伏AC电源。以“W”表示的水首先被输送通过供应单元组件402而进入输送管组件404。水W优选地在大约0.6升/分钟的流速及大约8psi的压力下从供应单元组件402输送出。但是,也可考虑更高或更低的流速和压力。

图1示出了用于向气体输送水蒸汽的低流量筒100的代表性实施例。筒100被构成用在诸如图3中示出的系统400等中。所述筒100用于湿润通过筒100的中空纤维膜135的气流140,同时暖水在中空纤维膜135的外部循环。筒100包括多个中空纤维膜135,每个纤维膜限定一个通道,用于气流140从通道的上游端流动到通道的下游端。在大约10psi的代表性压力等级和大约17℃到大约27℃的进口温度下,气体从进气口115进入筒。以大约每分钟1升到大约每分钟8升的流速将空气输送通过中空纤维膜135。希望在5升/分钟时经过中空纤维膜135的压降小于大约100mmHg。
在优选进口压力最高为大约90mmHg的条件下,水从进水口125进入筒100,并且在大约33℃到43℃的温度下接触中空纤维膜135的外表面。水流过中空纤维膜135外表面之间的空间,并且通过中空纤维膜135中的孔(在120处)将蒸发的水输送给膜135内的气流140。
随着气流140变得湿润,其向下游移动到通道的端部。在该端部空气从出气口110离开筒。水循环通过筒100的内部区域(在中空纤维膜外表面周围),所述循环水从出水口130离开筒100。
本发明的一个代表性实施例将水蒸汽向呼吸气体的传送限定到在呼吸气体中没有液态水存在的程度,同时优选地维持大约100%的相对湿度。在每分钟5升的进气口流速下,相对湿度的等级可选地降低到大约95%,但不允许液态水通过。
在另一个实施例中,优选的筒100被设计成可使中空纤维膜205的壁的尺寸优选为,在最高为大约每分钟8升的流速下允许足够的水蒸汽通过以使气体完全充满蒸汽,而同时抑制形成液态水。通过下述步骤限定性能:向中空纤维膜205的外部作用水压;以及测量:(1)液态水串套通过中空纤维膜205的时间;(2)在已经发生渗透之后水流过中空纤维膜205的速度。在优选实施例中,以下性能被认为是适当的:在47mmHg的静压力下,渗透至少花费一个小时,并且之后的水流量小于0.21ml/min。
图2示出了低流量筒200的容腔结构。外罩210封罩着由温度受控的循环水围绕的中空纤维膜205。在代表性实施例中,外罩210可以部分由聚碳酸酯材料形成。外罩210包括:进水口225,其被定位成适于将水引向中空纤维膜205的外表面;出水口230,其被定位成适于将水从外罩引出。水分子扩散通过中空纤维膜205进入到内腔,在内腔中被沿着中空纤维膜205流过的呼吸气体吸取。外罩210具有进气口215,其被定位成适于将空气引向中空纤维膜205的通道中的每个的上游端;出气口220,其被定位成适于将空气从中空纤维膜205的通道中的每个的下游端引出。
筒200被构成可将水蒸汽向呼吸气体的传送限定到在呼吸气体中几乎没有或者完全没有液态水存在程度。根据优选实施例,在呼吸气体中没有液态水,并且所述筒被构成可维持大约100%的相对湿度。
在较低流量应用中(例如,呼吸气体的流速为大约1升/分钟到大约8升/分钟),过去很难减少或防止液态水流入呼吸气体。具体地讲,由于呼吸气体的低流速,水蒸汽可积聚在呼吸气体中导致在呼吸气体中形成液态水。
但是,现在已经发现可通过减少中空纤维膜205外表面的累加表面积减少或者消除液态水流入呼吸气体。尽管发现这种表面积的减少有利于减少液态水通过,但表面积的过度减少可阻止水蒸汽充分通过以适当加湿呼吸气体。最优选地,希望提供的相对湿度等级在大约95%到大约100%的范围内,而同时不允许液态水通过。
现在已经发现,水蒸汽扩散通过中空纤维膜205进入呼吸气流的优选速度取决于下述两个主要变量中的至少一个,即可供扩散的表面积和中空纤维膜205的壁的两侧压力梯度。压力梯度是中空纤维膜205外部水压和内部气压之间的差异。水压可被认为恒定。在气体低流速下气压也低,因此梯度有利于更多的水扩散进入气流中。此外,低气流使得携带水蒸汽的能力减弱。组合的效果是趋向于扩散通过中空纤维膜205的水多于作为蒸汽被运走的水。结果,气流可积聚成液态水滴,接着被运送到病人的通风道中。为减少进入气流的水量,已经发现可以通过减少用于扩散的表面积实现。通过减少中空纤维膜205的数量来实现减小该筒100、200的表面。具体地讲,已经发现使用具有总面积在大约90平方厘米到大约110平方厘米范围内的大约250个中空纤维膜可以实现在有效范围内的水传送性能。已经发现前述的表面积对于在大约1升/分钟到大约8升/分钟之间的流速特别理想。
通过减少中空纤维膜205的数量可选地减少筒200的表面积,由此,减少了用于气流和蒸汽交换的中空纤维膜205的总表面积的量。或者,可选地减小中空纤维膜205的外径和/或长度以减少表面积。因此,通过选择纤维膜的数量、改变纤维膜的尺寸或者是上述两种方式的结合可实现表面积的减少。
在一个代表性实施例中,一个阵列的中空纤维膜205可由聚合材料形成,包括聚砜。但是它们也可以由非聚合材料形成。中空纤维膜205连接到安置在外罩210端部的盖245上。聚氨酯密封材料235可被布置在筒200的每个端部,以致将中空纤维膜205牢固地设置在外罩210内,目的是完全或基本上防止空气和液态水在筒中混合。
外罩210的进气口215和出气口220通过安置在外罩210端部的盖245限定。所述端盖245在外罩210的两端紧固在外罩上以封闭筒200的内部,但是它们可选地拆卸下来。端盖245可由聚碳酸酯或相似类型的材料组成。每个端盖245包括倒钩形软管连接部240。因此,这些倒钩形软管连接部240被设置在筒200的两端以致使筒200容易连接到整个系统上(例如,连接到导管上,通过该导管气体可被输入或输出筒)。因此,这些倒钩形软管连接部240在一端形成进气口215并在相反端形成出气口220。筒200可选地设有路厄连接器250,以构成每个进水口225和出水口230,并且将筒200接通到另一个外部装置上(例如,连接到导管上,水通过该导管被输入或输出筒)。

在另一个代表性实施例中,所述筒200具有大约6英寸到大约6.5英寸范围内的长度以及大约1英寸到大约1.5英寸范围内的宽度。筒200的端部被希望设有3/8英寸的倒钩形软管连接部240。筒侧向的连接器优选为大约5/8英寸长并具有大约1/4英寸的内径。提供这些尺寸是为了举例的目的,应该认为可以选择多种不同的形状、尺寸和结构,并且本发明不限于任何特定的尺寸和形状。
为了测定筒对于输送蒸汽是否最优,可进行“穿透”测试。例如,图4示出了筒100的穿透测试。通过侧向端口对筒100的水套施加水压,所述侧向端口用于经进水口125和出水口150实现水的连通。通过筒100中央上方的储水箱405的水的高度测定压力。筒100被夹持在读数单位为0.1gm的灵敏电子秤421上方。任何通过中空纤维膜的水从出气口110冒出落入放置在秤421上的容器415内。通过进水口125从储水箱405输送到筒100的水优选地通过操纵阀门411而进行控制。
参见图4,用于测试的特定压力为47mmHg,其相当于64cm的水柱。储水箱405的高度被调节到筒100中央上方64cm。记录第一滴水到达容器415的时间以及从穿透开始最初30分钟内水收集的速度(gm/hour)。穿透发生在第一滴水从出气口110冒出时。

Claims (15)

1、一种用于向气体输送水蒸汽的装置,包括:
多个中空纤维膜,每个纤维膜限定一个通道,用于使气体从所述通道的上游端流动到所述通道的下游端;
封罩着所述中空纤维膜的外罩,所述外罩具有进气口,其被定位成适于将空气引向所述中空纤维膜的所述通道中的每个的上游端,以及出气口,其被定位成适于将空气从所述中空纤维膜的所述通道中的每个的下游端引出,所述外罩还具有进水口和出水口,所述进水口被定位成适于将水引向所述中空纤维膜的外表面,所述出水口定位成可将水从所述外罩引出;

(b)使中空纤维膜的外表面接触处于33℃到43℃的温度的水;
(c)将中空纤维膜的总表面积维持在90平方厘米到110平方厘米之间。
5、根据权利要求4所述的方法,还包括下述步骤:以80%到100%的相对湿度提供气体。
6、根据权利要求4所述的方法,还包括下述步骤:以每分钟5升的流速、95%的相对湿度提供气体。
7、根据权利要求4所述的方法,还包括下述步骤:将通过中空纤维膜的压降维持为在每分钟5升的流速下小于100mmHg。
8、一种用于给病人输送湿润气体的系统,所述系统包括:
水接收器具;
气体接收器具;以及
与所述水接收器具和所述气体接收器具流体连通的装置,其被构成适于向气体输送水蒸汽并且包括:
多个中空纤维膜,每个纤维膜限定一个通道,用于使气体从所述通道的上游端流动到所述通道的下游端;
封罩着所述中空纤维膜的外罩,所述外罩具有进气口,其被定位成适于将空气引向所述中空纤维膜的所述通道中的每个的上游端,以及出气口,其被定位成适于将空气从所述中空纤维膜的所述通道中的每个的下游端引出,所述外罩还具有进水口和出水口,所述进水口被定位成适于将水引向所述中空纤维膜的外表面,所述出水口被定位成适于将水从所述外罩引出;
所述中空纤维膜的总表面积在90平方厘米到110平方厘米的范围内。
9、根据权利要求8所述的系统,其中,所述中空纤维膜被构成为:在没有气流且静态水压为47mmHg时,可在至少一个小时内阻止水穿透。
10、根据权利要求8所述的系统,其中,所述每个中空纤维膜被构成为:在初始水穿透之后,在47mmHg的静态水压下,水流量不超过每分钟0.21毫升。
11、根据权利要求8所述的系统,所述水接收器具为适于连接到水源的导管。
12、根据权利要求8所述的系统,所述气体接收器具为适于连接到气体源的导管。
13、一种用于向气体输送水蒸汽的装置,包括:
多个中空纤维膜,每个纤维膜限定一个通道,其用于使气体从所述通道的上游端流动到所述通道的下游端;
封罩着所述中空纤维膜的外罩,所述外罩具有进气口,其被定位成适于将空气引向所述中空纤维膜的所述通道中的每个的上游端,以及出气口,其被定位成适于将空气从所述中空纤维膜的所述通道中的每个的下游端引出,所述外罩还具有进水口和出水口,所述进水口被定位成适于将水引向所述中空纤维膜的外表面,所述出水口被定位成适于将水从所述外罩引出;
其中,所述中空纤维膜被构成为:在没有气流且静态水压为47mmHg时,可在至少一个小时内阻止水穿透;
其中,所述中空纤维膜被构成为:在初始水穿透之后,在47mmHg的静态水压下,水流量不超过每分钟0.21毫升。
14、根据权利要求13所述的装置,其中,所述中空纤维膜的总表面积在90平方厘米到110平方厘米的范围内。
15、根据权利要求14所述的装置,其中,所述中空纤维膜具有100平方厘米的总表面积。
