Abstract

A method of operating a microsurgical instrument, such as a vitrectomy probe (10), is disclosed. The probe includes a port (16) for receiving tissue and an inner cutting member (14). A flow of tissue is induced into the port with a vacuum source, and the member is actuated in a cyclic manner to open and clo se the port over a plurality of cut rates. A duty cycle of the member is varied with the cut rate to vary the flow of the tissue into the port. The method yields the ability to adjust flow rate into the probe and aperture of the probe for a given cut rate so as to effectively perform a wide variety of vitreoretinal surgical objectives.

Claims (19)

1. A method of operating a microsurgical instrument, said instrument comprising a port for receiving tissue and a member, comprising the steps of:
inducing a flow of said tissue into said port with a vacuum source;
actuating said member in a cyclic manner to open and close said port over a plurality of cycle rates; and varying a duty cycle of said member with said cycle rate to vary said flow of said tissue into said port.
2. The method of claim 1 wherein said actuating step comprises reciprocating said member along a longitudinal axis of said instrument using a pneumatic source to move said member from an open port position to a closed port position, and a spring to move said member from said closed port position to said open port position.
3. The method of claim 1 wherein said actuating step comprises reciprocating said member around a longitudinal axis of said instrument using a first pneumatic source to move said member from an open port position to a closed port position, and a second pneumatic source to move said member from said closed port position to said open port position.
4. The method of claim 1 wherein:
said actuating step comprises using a valve, an electrical signal that opens and closes said valve, and a pneumatic source coupled to said valve to create a pulsed pneumatic waveform; and said varying step comprises, for a given cycle rate, varying a pulse width of said electrical signal so that a maximum pressure of said pulsed pneumatic waveform is substantially equal to a pressure of said pneumatic waveform at which said member closes said port.
5. The method of claim 4 wherein said varying step comprises decreasing said pulse width with increasing cycle rate.
6. The method of claim 4 wherein said varying step comprises increasing said pulse width with decreasing cycle rate.
7. The method of claim 1 wherein said microsurgical instrument is a vitrectomy probe, said member is an inner cutting member, and said cycle rate is a cut rate.
8. The method of claim 1 wherein said microsurgical instrument is an aspiration probe, and said member is a sealing member.
9. The method of claim 1 wherein said actuating step comprises reciprocating said member along a longitudinal axis of said probe using a linear electrical motor.
10. The method of claim 1 wherein said actuating step comprises reciprocating said member along a longitudinal axis of said probe using a solenoid.
11. The method of claim 1 wherein an open size of said port varies with said cycle rate.
12. The method of claim 1 wherein said actuating step comprises controlling said member with a variable controller operatively coupled to said instrument.
13. The method of claim 12 wherein said variable controller comprises a foot pedal having a generally vertical range of motion.
14. The method of claim 13 wherein moving said foot pedal in a downward direction decreases said cycle rate.
15. The method of claim 14 wherein moving said foot pedal in a downward direction increases an open size of said port.
16. The method of claim 13 wherein moving said foot pedal in an upward direction increases said cycle rate.
17. The method of claim 16 wherein moving said foot pedal in an upward direction decreases an open size of said port.
18. A method of operating a microsurgical instrument, said instrument comprising a port for receiving tissue and a member, comprising the steps of:
inducing a flow of said tissue into said port with a vacuum source; and actuating said member in a cyclic manner to open and close said port over a plurality of cycle rates, wherein an open size of said port varies with said cycle rate.
19. The method of claim 18 wherein said microsurgical instrument is a vitrectomy probe, said member is an inner cutting member, and said cycle rate is a cut rate.
